    Caring staff and excellent therapy

I placed my mom here and have been very pleased - the nursing staff are caring, hands-on, and genuinely attentive to residents as individuals. Therapy is top-notch (her gait and weight improved), rooms are very clean, and the small, resident-focused community offers engaging activities and family dining. It's not the fanciest place, but the staff consistently exceed my expectations and have made this a loving, comfortable fit for our family.

    Summary of reviews

Care quality: Clinical and therapy strengths are a clear pattern in the feedback. Reviewers consistently praise nursing staff and rehabilitative services for attentive personal care, successful weight and gait outcomes, and individualized therapy progress. There are multiple accounts of staff delivering regular personal-care routines and going beyond baseline expectations to support residents' physical needs. At the same time, the facility appears to struggle with persistent staffing shortages and licensed-nurse coverage gaps, which creates variability in how reliably those strengths are delivered across shifts.

Staff and conduct: Staff are frequently described as warm, caring, and personally engaged with residents' interests and quality of life; many comments highlight cheerful, helpful nurses and support staff. However, there are also notable concerns about turnover, burnout, and instances of unprofessional conduct and accountability lapses. These factors contribute to an uneven experience: where engaged, stable staff are present the care is praised, but staffing instability and occasional conduct issues undermine consistency.

Dining and activities: The facility offers a family-dining option and organized social events, including monthly family-oriented activities, which reviewers view positively when implemented. Therapy and activity programming have produced meaningful engagement for some residents. Conversely, activity provision and meal quality are described as inconsistent; some accounts indicate minimal programming or mediocre food. Prospective families should expect variability in recreational and dining experiences depending on staffing and resource availability.

Facilities and housekeeping: The physical plant is described as older and in need of maintenance in multiple comments, with reviewers citing dated aesthetics and areas requiring attention. Housekeeping and sanitation are uneven: some rooms and areas are described as clean, while others reflect lapses in maintenance and cleaning standards. There are also practical facility shortcomings noted, such as limited outdoor or dedicated staff break space and shortages of basic supplies that can affect routine caregiving.

Management and safety: Management involvement and resident-focused priorities are noted positively by several observers, and some families found the facility to be a good fit under recent ownership changes. At the same time, reviewers raise concerns about administrative responsiveness, office-staff professionalism, and property-security processes. Specific safety-related operational gaps include memory-care environmental controls that may not meet expectations for secure dementia care. There are also mentions of resident property-security concerns that warrant inquiry.

Notable patterns and recommendation: Overall, Evercare of Lebanon appears to offer strong hands-on nursing and therapy when staffing is adequate, within a small, community-oriented setting. The principal trade-offs are operational: chronic staffing shortfalls, variability in housekeeping and activities, aging infrastructure, and episodic management or conduct concerns. Prospective residents and families should prioritize visiting at different times of day and asking targeted questions about staff-to-resident ratios, licensed-nurse coverage, memory-care security measures, housekeeping protocols, supply availability, and recent maintenance plans to assess whether the facility’s current operational capacity aligns with their needs.

    About Evercare of Lebanon

Lebanon Care Center is a medium-sized senior living and nursing facility with a total of 90 beds. The center operates under for-profit, LLC ownership, and is designed to serve residents in need of both short-term and long-term care. The facility is not part of a continuing care retirement community, focusing instead on providing dedicated nursing services to its residents. Participation in both Medicare and Medicaid programs allows for a broader range of individuals to receive care, making it accessible for residents with different financial backgrounds or coverage needs.

The care at Lebanon Care Center encompasses services that are typical of a skilled nursing facility, supporting seniors who may require assistance with daily activities, medical supervision, and rehabilitation after hospitalization. Residents at the center can benefit from specialized rehabilitative services whenever required, with care plans tailored to address individual health needs following comprehensive assessments. These assessments are regularly updated to ensure the evolving requirements of each person are met.

Within the facility, the management structure includes a nursing director and registered nurses tasked with the oversight of resident care. Nurses are responsible for developing and updating care plans, administering medications, and ensuring that therapeutic diets are prescribed and followed as ordered by physicians. Dietitians, either registered or licensed, can be delegated to manage the dietary needs of residents under the direction of the attending physician, within the scope defined by state laws. The center’s food services are designed to meet professional standards, from sourcing ingredients to food storage, preparation, and distribution.

Lebanon Care Center places emphasis on the dignity, self-determination, and communication rights of its residents. Efforts are made to ensure a supportive environment where residents can exercise their personal rights, including being informed about transfers or discharges, and being given appeal opportunities when changes occur. The center aims for a dignified living experience for all who reside there, with particular attention to protective measures against abuse, neglect, and unsafe conditions. Staff strive to maintain a setting free from accident hazards and are tasked with adequate supervision of all residents.

Routine health, fire, and safety inspections are conducted to maintain a secure environment, with the goal of meeting regulatory standards and promptly correcting any identified issues. Lebanon Care Center’s commitment to a high standard of care is reflected in the routine updating of individualized care plans and ongoing staff training. The facility aims to be a supportive and caring environment for seniors requiring skilled nursing and rehabilitative services, providing a comprehensive approach that encompasses medical, dietary, and personal care needs.
