    Long-term resident praises facility care

I'm a happy long-term resident in this beautiful, very clean facility on 10 acres. The caring, friendly staff and family-run atmosphere (Mike Babitz gave a great tour) - plus excellent in-house nursing, PT/OT rehab, great food, daily activities and intergenerational programs - have done wonderful things for me.

    Summary of reviews

Libertyville Manor Extended Care presents a mixed but distinct profile. Many families and former residents praised the facilityโ€™s rehabilitation services, onsite physical and occupational therapists, and a visible, family-run management approach. The campus and grounds are viewed positivelyโ€”spacious indoor and outdoor areas on a large propertyโ€”and the facility supports a regular daily schedule of activities, including an intergenerational daycare program that several people identified as a meaningful social asset. Dining quality and frequent staff rounds were also noted as strengths, and the facility advertises an in-house nursing capability alongside hospice-friendly support.

Clinical-care impressions are uneven. The facility appears to perform well for short-term rehabilitation and for socially active, lower-dependency residents who can participate in programs. However, there are recurring operational concerns about staffing levels and responsiveness, which reviewers linked to delays in addressing restroom and mobility assistance and variable availability of staff across shifts. Several accounts highlight issues with staff conduct and communication tone; these describe problems with frontโ€‘line interactions and a need for clearer, more consistent family communication and follow-up after concerns are raised.

A notable pattern relates to care for residents with cognitive impairment. Reviewers indicated limited capacity for higher-acuity dementia care and questioned behavioral-management and medication-related practices in those situations. This suggests the facility may be less suitable for individuals whose primary needs are advanced memory support or complex behavioral management without augmentation of specialized staff and protocols.

Facility condition and management stability are additional considerations. While parts of the buildingโ€”particularly the rehab areaโ€”were described as updated and well maintained, other areas were identified as needing renovation. Operational instability has been reported due to property-sale and closure planning, which raises continuity-of-care and transition concerns for families. Prospective residents and families should assess current staffing ratios, dementia-care protocols, and contingency plans for property changes before making a placement decision.

In summary, Libertyville Manor may be a good option for short-term rehabilitation, therapy-driven recovery, and socially engaged residents who benefit from activities and intergenerational programming. For families considering long-term placementโ€”especially when dementia care or 24/7 high-acuity attention is requiredโ€”it is advisable to verify staffing consistency, observe care during multiple shifts, request details on behavioral- and medication-management practices, and confirm any plans related to facility ownership or closure that could affect continuity of care.

    About Libertyville Manor Extended Care

Libertyville Manor Extended Care sits on a peaceful 40-acre campus with landscaped courtyards, a park-like setting, and a clean, well-kept building that always seems busy but organized when you walk through. There are different places to live, with one-bedroom and two-bedroom apartments available, and plenty of help for daily needs like bathing, dressing, grooming, toileting, medication management, and meal reminders, which is helpful for people with health concerns-especially since the nursing staff can handle feeding tubes, diabetic injections, complex medical needs, and wound care, and they do IV therapy and even respiratory care if needed, while the therapists help after surgery, after strokes, or with any kind of rehabilitation like joint replacements or sub-acute rehab. For folks who need more help, Libertyville Manor offers specialized care for Alzheimer's and dementia in their secure memory care program, using therapies that help with confusion and wandering, plus they have support for mental illness and hospice care. People can also use respite care, adult daycare, and long-term skilled nursing, which gives a lot of options for different stages of life, and they've got their Sheltered Care, assisted living, and independent living choices so families don't have to worry about moving their loved ones if their needs change.

Residents get nutritious meals made by chefs and meal planners who put effort into ingredients and taste, and there're on-site amenities like a beauty and barber shop, a library, housekeeping, and a fitness program, along with worship services and many social events, so people can keep active and connected. Staff at Libertyville Manor, which is family owned and privately run, show real friendliness, patience, and kindness, treating everyone as family and making the environment feel warm and home-like, and visitors notice how calm and welcoming the community is because of the experienced and dedicated people working there, from nurses to aides, who never seem rushed even when it's busy. People on tours often comment about how animals like Gander the national service dog visit, and the activities director has won awards for creating programs that help keep residents engaged mentally, physically, and emotionally, including intergenerational activities and outings into the community that folks always seem to talk about afterward. Libertyville Manor is fully licensed, insured, Medicare-approved, and well-accredited, and the philosophy's straightforward: everyone gets treated with dignity and respect, and personal care gets planned according to each resident's needs, so no one feels like they're just a number or left behind. The services-from medication distribution and management to adult rehab, wound care, memory programs, and stroke recovery-let families choose flexible short-term or long-term help within one community that values safety, compassion, and health above everything else, which gives peace of mind even if things change.
