    Compassionate professional staff, excellent care

I placed my mom here and I'm very happy with the compassionate, professional staff - nurses, CNAs, NP Samantha and admin Megan were especially helpful during the transition. Care and therapy were excellent and helped her progress, meals were tasty, and rooms and common areas were clean and comfortable. The grounds are lovely, activities are engaging, and communication is timely and organized. I'm grateful we chose this community.

    Summary of reviews

Overall impression: Tabor Hills presents as a campus with notable strengths in therapy-led rehabilitation, a continuum of care (including a memory-care wing), and a generally clean, landscaped environment. Review narratives frequently highlight effective rehabilitation outcomes, attentive nursing and therapy staff, and a range of social and personal-care amenities that support daily life and socialization. At the same time, reviewers describe a mix of experiences that point to variability in how those strengths are delivered across wings and shifts.

Care and staff: Clinical and caregiving staff receive consistent praise for compassion, attentiveness, and rehabilitation skill; families describe successful PT/OT outcomes and staff who support residents' dignity. However, there is a clear pattern of variability: some shifts or individual employees are perceived as less engaged, and reviewers describe delays in responding to call lights and occasional lapses in attending to basic needs. Staffing models that include agency therapists and variable scheduling appear to contribute to uneven continuity of care. There are also specific operational concerns about medication administration and diabetes-related tasks that families should clarify at admission.

Dining and activities: Dining is frequently cited as an asset—many reviewers describe above-average meals, generous portions, and a broad selection. A minority of accounts point to inconsistent food quality and occasional adverse dining-related effects, so dietary accommodations and monitoring may be important for medically fragile residents. The activities program is broad (salon services, music, manicures, trips, and group outings) and supports social engagement; nevertheless, programming is described as uneven for residents with mobility limitations and on weekends, and some pandemic-era reductions in group activities are still noted.

Facilities and amenities: The campus mixes recently renovated wings and well-appointed private-room bathrooms with older sections that show deferred maintenance (worn carpeting, older cabinetry, door hardware issues). Common areas and grounds are generally regarded as attractive and well cared for, and private-room options are available. Wifi is provided but coverage and strength are inconsistent in parts of the facility, which may affect telehealth and family video calls.

Management, admissions, and operations: Administrative staff and social workers receive positive mention for guidance and support during transitions, and the facility accepts Medicaid with some level of assistance available. Some families perceive admissions tours as sales-focused or insufficiently attentive to dementia-specific concerns; prospective residents should request detailed care-process explanations and observe staff–resident interactions across shifts. Infection-control policies (including frequent testing and visitation restrictions) have been strictly enforced, which reassures some families but has constrained visitation and activities for others.

Notable patterns and recommendations: Prospective residents and families will find strong rehabilitation services, a compassionate core staff, clean public spaces, and varied programming attractive. Important trade-offs include occasional variability in day-to-day caregiving responsiveness, reliance on agency staff that can affect continuity, uneven maintenance between wings, and inconsistent wifi and dining experiences. Before committing, families should (1) tour both renovated and older wings, (2) ask for current staffing ratios and use of agency personnel, (3) review medication- and diabetes-management procedures, (4) confirm activity availability for less-mobile residents and weekend programming, and (5) discuss costs and Medicaid assistance. These steps can help align expectations with the facility's operational realities.

    About Tabor Hills Healthcare Facility

Tabor Hills Healthcare Facility has a long history, starting all the way back in 1894, first as an orphanage and then as a home for seniors, and now it sits on 52 landscaped acres where people can find Independent Living, Supportive Living, and a Skilled Nursing/Rehabilitation Healthcare Facility all on one campus, which makes it a spot where seniors can get care for many different needs whether that's independent living, supportive services, short-term rehab, long-term care, or even memory care. The mission at Tabor Hills is to help each resident reach and keep their best physical, mental, and emotional well-being, so there's a lot of focus on dignity, self-determination, and making sure everyone feels safe and comfortable, plus there's always a staff member around to help with things like bathing, dressing, medication, transfers, and walking or wheelchair assistance, and for folks who need a little more, they offer restorative nursing, therapy, wound care, and support from nurses like RNs and personal care assistants. Daily life includes scheduled activities, social programs, outings, arts and crafts, health and wellness classes, movie nights, community and resident-run events, plus good old-fashioned walks on the paths, gardening, using the fitness center, visiting the library, or playing cards in the game room, and for families, there are support services and visiting options. The private rooms have their own bathrooms, cable, telephones, air conditioning, kitchenettes, and high-speed Wi-Fi, and the facility has studios, single rooms, two-bedrooms, and semi-private styles all with set monthly rates, which means people can find a setup that matches their needs and budget. Meals get served restaurant-style in a dining room, with lots of choice thanks to a professional chef named Krys who manages special dietary requests for things like diabetes and food allergies, and there's also all-day dining so folks can eat when they want. For healthcare, Tabor Hills offers nursing home care, skilled nursing, sub-acute and outpatient therapy, and services for people who need extra support after surgery or illness, with about 43 of 105 certified beds available as of June 2025, and they take Medicare, Medicaid, and long-term care insurance. The campus runs housekeeping, laundry, dry cleaning, transportation, and guest parking, and there's an emergency alert system, sprinklers, a move-in coordinator, and a dedicated social services department, plus residents can use washers, dryers, salons, maintenance, and kitchens. The staff, known for being friendly and attentive, manage support services and help coordinate care and activities, and each part of the campus, including Independent Living, Supportive Living, and Healthcare, has dedicated phones and offices, with nursing stations named 1West, 2West, and 2East. There's also a Senior Day Services program for folks who want a safe place to socialize, exercise, and take part in cognitive activities during the day, and respite care is available if families need a break. Tabor Hills makes sure to follow all Federal and State laws and got special recognition for their vigilance during COVID-19 from Telligen, earning the Blue Ribbon in COVID-19 Vigilance Award, so safety and quality are high on their list. The community includes Suportive Living, Independent Living, and a Healthcare Facility offering sub-acute care, and with programs from Symbria Wellness and a social services team, there's a lot aimed at helping everyone stay as healthy and happy as possible, and folks can pick the level of care or support that's right for them.
