    Compassionate staff, clean comfortable home

I had a very positive experience here: the staff and administration were friendly, knowledgeable and genuinely caring, always eager to assist with compassion. The building felt clean and home-like with cozy rooms, shiny floors and a pleasant smell, rehab and meds were well managed, and I would confidently recommend it to loved ones.

    Summary of reviews

The reviews reflect a highly mixed set of experiences at this facility, with notable strengths alongside persistent operational weaknesses. Care-quality impressions range from accounts of highly attentive, compassionate nursing and effective rehab therapy that enabled functional improvement, to descriptions of inconsistent clinical performance and lapses in medication administration. Several families praised the Director of Nursing and individual caregivers for knowledge and responsiveness, while other families described staffing gaps and inconsistent continuity of care.

Staff behavior and culture appear uneven. Multiple accounts highlight friendly, welcoming administrative staff, engaged unit caregivers, and a generally hospitable atmosphere for short stays. At the same time, reviewers describe variability in staff responsiveness, including slow call-light responses and difficulties getting timely attention for health concerns. There are recurring comments about poor communication and inconsistent handling of medical records and family updates.

Dining and nutrition receive mixed evaluations. The facility employs a certified cook and dietary aide and some residents had satisfactory meal experiences, but many reviewers expressed dissatisfaction with meal quality, limited menu variety, and unappetizing preparation. Some comments also raised pest-control concerns specifically linked to kitchen areas, which amplify dining-related worries.

Facility condition and resident environment are inconsistent. Positive notes describe home-like rooms and clean, shiny floors in maintained sections, yet there are repeated sanitation concerns, localized odor issues, observable deferred maintenance (aging beds, equipment, and furnishings), and pest-control concerns. Safety-related issues were highlighted as operational risks—for example, furniture or bedding that may impede mobility and worn mobility aids—suggesting a need for systematic maintenance and equipment replacement.

Programming and daily engagement appear limited for some residents. Several reviewers noted minimal activities and extended periods of low stimulation, which may affect residents’ physical and mental well-being. Conversely, families who observed active staff engagement reported more positive social and therapeutic interactions.

Management and administrative practices drew polarized feedback. Some reviewers praised an engaged, compassionate administrator and helpful admissions staff; others described what they perceived as an administrative focus on finances, brochure misrepresentation, opaque billing, and inconsistent follow-through on care concerns. There are also serious, individual allegations—such as theft and post-incident concerns—that reviewers cited; these specific claims may warrant verification through regulatory or investigative channels.

Overall pattern:Expect variability. Prospective residents and families may find dedicated, skilled caregivers and effective short-term rehab in some units, but should be prepared to assess staffing consistency, inspect areas for sanitation and maintenance, review dining/food-service practices, and confirm communication protocols and billing transparency before placement. For families considering this facility, targeted questions about staffing ratios, response-time metrics, maintenance schedules, pest-control programs, activity calendars, and documentation practices would help clarify whether the facility’s strengths align with the resident’s needs.

    About Evercare of Swansea

Swansea Rehab Health Care is a medium-sized, for-profit nursing facility with a total of 94 beds, offering both short-term rehabilitation and long-term care services. It operates under corporate ownership and is not part of a continuing care retirement community. The facility participates in both Medicare and Medicaid programs, allowing for a broad range of residents, including those relying on these government insurance options for payment. Its capacity ensures that it can provide care and support to a significant number of individuals, aiming to meet the diverse needs of the community it serves.

The facility provides skilled nursing services, including short-term rehabilitation for those who have had recent hospital stays. Medicare typically covers short-term rehabilitation at this type of facility for up to 100 days following a qualifying hospital stay, subject to specific eligibility requirements. For those in need of long-term custodial care, Medicaid may provide coverage once long-term care insurance and personal resources have been utilized. Residents can expect that payment and cost details are carefully managed to help families understand their financial obligations throughout their loved one’s stay.

Routine health, fire, and safety inspections are conducted at Swansea Rehab Health Care, reflecting its ongoing commitment to a safe and supportive environment. These inspections, performed at least every 15 months, assess critical areas such as food preparation safety, infection prevention programs, and response to resident needs and preferences. Over recent inspection cycles, the facility has encountered various challenges, including maintaining a registered nurse on duty for the required hours, timely reporting of incidents, the adequacy of pest control, and fulfilling residents’ rights to a clean, comfortable, and homelike setting. The facility has also received citations relating to staff licensing, monitoring antibiotic use, and ensuring all treatments and care plans align with physician orders and the personal goals of each resident. Most of these issues have been resolved within the timeframes outlined by inspection authorities, and correction dates are documented for transparency.

Swansea Rehab Health Care’s engagement with both Medicare and Medicaid, together with its sizable bed capacity, positions it as a significant provider within its local area. While it is not a part of a larger retirement community or affiliated with hospital complexes, it strives to deliver essential services for seniors requiring both rehabilitation and ongoing nursing care. The range of care options and participation in federal and state programs support its mission of caring for older adults across various stages of health and rehabilitation needs. Regular inspections and oversight contribute to its ongoing efforts to maintain quality care and address any areas that need improvement, ensuring that residents can expect a responsive and safe living environment.
