    Professional, clean, friendly, active community

I'm grateful we found this place - the administration, nurses, phenomenal CNAs, housekeepers and cooks are professional, kind and very responsive. Since Axiom took over the community is cleaner, friendlier, and more active with great events and holiday celebrations; my family and I are happy and would highly recommend it, with only minor room for improvement.

    Summary of reviews

Reviews present a mixed but recognizable pattern: many families praise the facilityโ€™s direct-care workforce while others describe operational weaknesses that affect day-to-day resident experience. Positive observations center on frontline staff โ€” CNAs, nurses, housekeepers and kitchen employees โ€” who are frequently described as caring, professional, and attentive. Families mention supportive administration, a friendly community atmosphere, active programming (holiday celebrations, crafts, and other events), and kitchen staff that are appreciated when meals meet expectations.

At the same time, several reviewers raise concerns tied to operational capacity and consistency. Staffing levels are a recurring theme: small staff groups and what reviewers describe as staffing shortfalls correlate with delayed nursing responsiveness, longer waits at the nursesโ€™ station, and overall strain on daily care delivery. Those operational pressures are also associated with inconsistent housekeeping and trash-removal practices and with sanitation and incontinence-care concerns in specific areas. These patterns suggest variability in maintenance and personal-care processes depending on shift coverage and workload.

Dining and activities elicit divergent impressions. Activity programming and event planning are frequently cited as strengths, with families noting engaging holiday events and recreational crafts. Food service receives mixed feedback: some reviewers call the cooks phenomenal and express gratitude, while others cite poor meals and inconsistent food-service execution. This indicates variability in dining quality or in meal-service reliability across times or units.

Management and communication appear to be in transition for some families. Several reviews note improvements since an Axiom takeover and describe positive changes in staff demeanor and resident happiness; others criticize management responsiveness and characterize administrative oversight as uncaring. Combined with reports of high census relative to available staff, these comments point to operational and leadership areas that may benefit from continued attention.

Overall, prospective residents and families will find evidence of strong, compassionate caregiving and active resident programming alongside recurring operational concerns: cleanliness consistency, staffing adequacy, and meal-service reliability. Those considering the facility may wish to ask about current staffing ratios, sanitation and incontinence-care protocols, recent quality-improvement actions since management changes, and to observe meal service and staffโ€“resident interactions during a visit.

About Axiom Healthcare of West Frankfort

Axiom Healthcare of West Frankfort is a for-profit skilled nursing facility managed by Petersen Health Care. The facility has 96 certified beds, though it usually has about 38 residents each day. Residents have access to furnished rooms with private bathrooms, air conditioning, telephones, kitchenettes, cable TV, and Wi-Fi. The staff provides 3.32 nurse staffing hours per resident per day, but the nurse turnover rate is high at 57.4%. The reported care covers non-ambulatory support, help with bathing, dressing, transfers, daily living, and medication management, with a 24-hour call system and supervision. Skilled nursing is available 24 hours a day, seven days a week, but inspection reports show 43 deficiencies, including problems with keeping a registered nurse on duty, holding regular quality meetings, and making sure there's enough food and fluids as well as proper storage, preparation, and serving of food. They've reported one infection-related deficiency.

The facility has special programs for memory care, behavioral health, wound care, respiratory care with oxygen therapy, nebulizers, and tracheotomy support. There are also on-site X-ray services, physical therapy, occupational therapy, and speech therapy. A professional chef handles meals, making sure to support allergies, diabetes, and special dietary needs, though food service issues have been documented during inspections. There's a dining room with all-day dining, community events, daily activities, art programs, movie nights, music, resident-led and group activities, fitness classes, religious programs, and amenities like a barber shop, laundry, and housekeeping. They offer concierge service, move-in help, transportation and parking, emergency alert systems, and outdoor spaces like a garden, walking paths, and outdoor common areas. Residents can enjoy furnished living areas where they can relax and socialize. The facility accepts Medicaid and Medicare payments. Overall, Axiom Healthcare of West Frankfort provides many health services aimed at supporting residents' needs, comfort, and independence, but inspection reports show there are areas that need improvement, especially in food service and nursing.
