    Warm staff, home-like, highly recommended

I moved my loved one to Crossroads and have been very pleased - the staff are supportive, warm, and treat residents like family. Nurses, CNAs, therapists and support staff (special thanks to Michael, Vanessa and Ashlea) are attentive and helpful, and the team kept us informed through therapy progress. The grounds are pleasant, the place feels like home, renovations are improving things, and they offer VA voucher and transportation help. I would choose this facility again and recommend it to others.

    Summary of reviews

Overall impression: Reviews portray a facility with a strongly positive caregiving culture but with several operational and facility-level weaknesses worth evaluating in person. Many families highlighted the competence and compassion of front-line staffโ€”nurses, CNAs, therapists, dietary, housekeeping, and activities teamsโ€”and consistent progress during rehabilitation stays. Therapy services (physical, occupational, speech) and collaborative care-planning were repeatedly cited as strengths, and reviewers noted responsiveness to family communication and coordinated discharge or next-level placement.

Care quality and staff: Clinical and therapy staff are frequently described as caring, attentive, and effective at advancing rehabilitation goals. Families also noted timely attention and individualized end-of-life support when needed. The primary staff culture is characterized as welcoming and familial, which aligns with multiple mentions of staff who โ€˜โ€˜love their jobs.โ€™โ€™ However, there are also comments about inconsistent nursing responsiveness and occasional lapses in oversight; prospective families should clarify staffing patterns, nurse-to-resident ratios, and on-duty supervision during a tour.

Dining and activities: Support services including housekeeping, laundry, kitchen, and activities programs received positive remarks. Transportation to medical appointments and stores and assistance with VA voucher benefits were noted as practical advantages. Pricing was described as higher than alternatives, so cost-benefit considerations and contract details should be discussed with admissions staff.

Facilities and safety: The campus and grounds are described as pleasant and peaceful, and management has planned renovations. At the same time, reviewers raised concerns about aging infrastructure, deferred maintenance, and cleanliness in some communal areas. Security and access-control issues were mentioned (for example, concerns about doors and visitor access), and there were reports of an electronic check-in system that generated unwanted messages. Infection-control practices, particularly around COVID handling, were also a source of concern for some families.

Management and administration: Several comments indicated uncertainty about administrative stability and credentialing; this is an area to verify directly with facility leadership and through state documentation if licensing is a concern. Communication strengths (family updates) were noted, but some reviewers experienced problematic automated communications from check-in technology or unclear visitor policies, including a visitor-fee element that families should confirm.

Bottom line: The facility appears to offer strong, person-centered caregiving and effective therapy services within a pleasant campus environment, but operational issues (facility maintenance, sanitation in common areas, security/access controls, administrative clarity, and some inconsistencies in nursing responsiveness) recur in reviews. Prospective residents and families should arrange an on-site visit, review recent inspection and licensing records, ask about staffing and infection-control protocols, and get a clear written summary of costs and visitor policies before committing.

    About Highlight Healthcare of Woodstock

Highlight Healthcare of Woodstock sits at 309 McHenry Rd., Woodstock, Illinois, and it's a for-profit nursing home that's been running since at least 2025, operating under Yosef Tsadok and Rifka Weinberg since September 2023, offering more than 100 beds and focusing on post-acute rehabilitation, general medical care, memory support, and a wide range of skilled nursing needs, but it's important to know that this place's been named a "special focus facility candidate" due to serious concerns about quality, and past inspections have brought up a good number of problems, including 64 deficiencies-among them, 4 were about infections and there were issues like abuse, neglect, and exploitation, so it'd be wise for families to take a good look at recent reports before making up their mind. Nurse turnover here is high at 45.1% and nurse staffing averages 2.94 hours per resident each day, which is something to think about if quality of care and staff consistency matter, though they do have physicians, restorative nursing assistants, and support for daily activities like bathing, dressing, grooming, toileting, and eating, along with a bunch of therapy options like occupational, physical, and speech therapy, plus diabetic care, dialysis, post-surgery rehabilitation, and wound care services scattered among the offerings. Residents who need memory support for things like Alzheimer's, dementia, or who require skilled nursing for hospice, stroke, cardiac recovery, pulmonary problems such as COPD, tracheostomy care, or who are recovering after surgery or trauma can find care plans made for those needs, and while people can expect help with laundry, meals, medication, and personal support services, details like the entrance, exact building design, or levels of accessibility aren't really specified anywhere; still, folks get meals, nutrition counseling, housekeeping, worship services, and organized outdoor events. The community itself sits in a region of Woodstock with LGBTQ protections against discrimination based on orientation, gender identity, and income, and nearby apartments and townhomes often mention things like pools, patios, fitness centers, internet, pet-friendly rules, laundry rooms, and waterfront views, though which of those fit this facility in practice isn't too clear from the records. Highlight Healthcare of Woodstock isn't currently for sale or rent, and there's no website as of now, but anyone considering this place would do well to look closely at all inspection reports and pay careful attention to the facility's history before making any decisions, because the record shows both a big range of healthcare services and a pattern of quality concerns that deserve a real hard look.
