    Loving staff, excellent care, recommended

My mom spent two years here and I'm very grateful. The staff-Ashley especially-were loving, attentive and treated residents like family; nursing, rehab and therapy were high-quality and recovery-focused. The facility and Memory Care wing were clean, food and service were good, and the front office was cooperative-I would recommend them.

    Summary of reviews

Reviewer experiences at Willow Crossing Health & Rehab are mixed, with distinct areas of strong performance alongside operational weaknesses that families should evaluate carefully. Many families and former residents emphasized the quality of rehabilitation and therapy services, noting substantive clinical gains and positive rehab outcomes. The Memory Care wing and several common areas were described as well-appointed and clean by some visitors, and hospice coordination and long-term placement continuity were viewed positively in individual cases.

Direct-care staff receive frequent praise for compassionate, attentive interactions; reviewers describe caregivers who form family-like relationships with residents and provide individualized attention. Several comments singled out supportive front-office personnel and named staff who facilitated admissions, visits, or care coordination. These strengths appear to be important drivers of positive experiences, particularly when rehabilitation or palliative support is the primary service need.

Counterbalancing those positives are recurring operational concerns. Cleanliness and incontinence-care practices were described as inconsistent, with associated delays in cleanup and room maintenance. Families also raised concerns about staffing levels and coverage, especially during later shifts, which reviewers linked to delays in attending to residents, postponed bed or shower changes, and occasional missed or delayed clinical tasks. Linked to staffing issues were described lapses in medication administration and missed or poorly coordinated medical appointments.

Dining and dietary service present a variable picture: some residents enjoy meals and regular beverage service, while others described inconsistent food quality, overcooked items, and limited variety or portion issues. Equipment availability โ€” including mobility aids and appropriate beds โ€” and general room maintenance (for example, window seals and routine upkeep) were also cited as areas requiring attention.

Management and supervision emerged as another theme. Reviewers described inconsistent staff professionalism (including on-shift distractions) and raised questions about training, hiring, and retention practices. Families noted instances where administrative responsiveness did not meet expectations, and where operational activities appeared to take precedence over immediate resident needs.

For prospective residents and families: the facility offers clear strengths in rehabilitation, therapy, memory-care environment, and several examples of compassionate caregiving. At the same time, there are consistent indications of variability in cleanliness, staffing, medication coordination, and facility maintenance. When touring or evaluating Willow Crossing, consider asking specific questions about staffing ratios by shift, medication-safety protocols, equipment availability and maintenance schedules, cleaning and incontinence-care procedures, meal planning and dietary accommodations, and how the facility documents and responds to family concerns. These checks can help determine whether the facilityโ€™s strengths align with a prospective residentโ€™s needs and risk tolerance.
