    Outstanding rehab caring staff spotless

I'm very pleased with my loved one's stay-rehab and therapy were outstanding and helped them recover, the administrator and staff were caring and responsive, and nursing/housekeeping were professional. The facility is spotless and odor-free, meals are good, activities plentiful, communication was helpful, and I'd recommend Cloverleaf for rehab and long-term care.

    Summary of reviews

Overall impression Cloverleaf Healthcare is described predominantly as a rehabilitation-focused and long-term care provider with a strong emphasis on therapy services, hands-on caregiving, and facility cleanliness. Many families highlighted positive rehabilitation outcomes, especially from physical, occupational, and speech therapy teams, and several accounts describe residents regaining function and returning home. Long-tenured staff, a welcoming admission process, and an emphasis on family communication are recurring strengths.

Care and staff Care quality is commonly characterized as compassionate and attentive. Nursing staff, CNAs, therapists, and ancillary teams (housekeeping, maintenance, food service) are frequently praised for going above and beyond, and reviewers often note continuity from long-serving employees. That said, there are some contrasting accounts: a minority of reviewers reported concerns about responsiveness, documentation, and clinical monitoring. These concerns point to occasional variability in how promptly clinical issues are identified and communicated to families.

Dining and activities Activities programming is a clear strength โ€” the facility offers frequent, engaging events that families and residents appreciate. Meal service receives mixed-but-overall-positive comments: multiple reviewers enjoyed meals and cited regular family-style dining and pleasant food service, while others requested greater variety and healthier options (more fruits/vegetables and less repetitive menu items). Meal continuity and communication about dining preferences are areas families suggested could be strengthened.

Facilities and operations The physical plant is commonly described as clean, well-kept, and odor-free, with housekeeping and laundry processes noted as thorough. Some operational weaknesses were identified: parking is limited, and room assignments or room sizes have caused discomfort for a few families (including shared or undersized rooms and occasional wrong assignments). These point to capacity and space-management constraints rather than an overall lack of upkeep.

Management and communication patterns Several families praised proactive administrative involvement, tours, and a visible leadership presence that advocates for residents. Conversely, other accounts describe inconsistent administrative responsiveness and variable communication during admissions or when changes occur. Reviewers recommended that families clarify scheduling, daily routines, and preferred communication channels on admission to reduce misunderstandings.

Notable patterns and guidance for families The dominant positive themes are strong therapy outcomes, caring frontline staff, and a clean, activity-rich environment suitable for both short-term rehabilitation and longer stays. The primary operational concerns to weigh are variability in clinical monitoring/response, occasional delays in addressing resident needs, inconsistent communication about schedules and care plans, meal variety limitations, and space/parking constraints. Prospective residents and families should consider asking targeted questions about clinical-monitoring protocols (especially for high-acuity needs), meal options and special-diet availability, room assignment practices, and the facility's preferred family-communication process during tours or pre-admission conversations.

    About Cloverleaf Healthcare

Cloverleaf Healthcare sits at 9325 N S Crawford St in Knightsville, Indiana, and it's a place where you'll see several different services under one roof, because they've got assisted living, skilled nursing, memory care, independent living, and long-term care, and you'll see names like HR Director, Staff Nurse in Rehabilitation and Long Term Care, Social Services Director, and an owner and CEO walking around, which gives a sense they've got hands-on staff. They've earned a 4.6 average patient rating from 9 reviews, which tells you people seem generally happy with the care there, and you'll also find they're a spot for community events like the Servicemen's Breakfast, where each week, former service members and others get together for an 8 a.m. meeting and breakfast at 8:30, where you'll see eggs, bacon, sausage, biscuits and gravy, and some drinks, which is a nice routine for those who like some social time with their mornings. You'll notice the facility focuses on medical help, too, because they offer 24-hour skilled nursing care, short-term rehabilitation, wound care, and even in-house PD Dialysis, so people who need strong medical support can find help without leaving the building. There's specialized memory care programs for folks with Alzheimer's or dementia that feature memory-enhancing activities and all-day supervision in safe, locked areas, and the independent living option means people who want fewer chores and more social activities can have those, though they can ask for support if their needs change. They also support people with everyday things like bathing, meals, and taking medicine. There's not a lot of flash or fancy names for rooms or extra features, but the basics are here-health care, support, nursing staff, and a sense of steady routine, so residents can get the care and help they need, along with some familiar faces every day, plus the place can be lively sometimes thanks to their local events.
