    Ossian Health Care and Rehabilitation Center, managed by TLC Management since June 2012, has 100 certified beds and provides four types of care: Assisted Living, Memory Care, Skilled Nursing, and Rehabilitation. The center covers different stages of senior care in the same community, so residents can move between care types as needs change without having to leave the facility, and they've done this by dividing the building into four distinct lifestyle communities. They say families are important partners, and care plans get shaped by the wishes and needs of each resident, using a personalized approach that attempts to foster independence, comfort, guidance, and restoration, helping people to live as fully as possible. There's around-the-clock nursing for residents who need Skilled Nursing, and specialized areas for residents with memory loss that help keep them secure. Their staff provides physical, occupational, and speech therapy, and although they've had praise for compassion and individualized plans, there's also a higher-than-average nurse turnover rate at 57.1% compared to the state average of 48.5%, with reported nurse staffing hours per resident per day at 3.42, which is below the state average of 3.7. Past inspections have pointed out two infection-related deficiencies, and there have been complaints about infection control and failing to provide care following residents' orders and wishes. Still, the staff does try to monitor health daily and carry out regular safety checks, aiming to help restore residents to their best potential, whether they're staying short-term or long-term. The living areas have comfortable lounges, family rooms, dining spaces, enclosed courtyards, and gardens, all arranged to be friendly and safe. There's a life enrichment center that holds programs and seasonal activities, including arts and crafts, games, group talks, light exercise, pet therapy, music therapy, and scenic strolls, which help keep people mentally, physically, and spiritually engaged. They provide chef-prepared daily meals in a dining room set with linens and silver, and staff will help based on each resident's preferences, allergies, and dietary needs. Residents can count on personal transportation, laundry, and housekeeping services. Trial or respite stays are possible, and there's an option for guests to dine with residents. Ossian Health Care and Rehabilitation Center operates as a rental community, so you don't have to pay a large buy-in fee to move in, which keeps things flexible for families. They run a Facebook page with updates and community news.
