    Clean facility, new owners improving

I like the facility - clean, well-run - and the new owners are clearly making positive changes. My only frustration is how state regulations slow some improvements, but overall I'm very satisfied.

    Summary of reviews

The reviews indicate persistent operational and care-quality concerns at the facility with a small number of positive notes about recent ownership change and some attentive staff interactions. The dominant themes are sanitation and hygiene-related issues, staffing instability, documentation and clinical oversight gaps, and worries about financial transparency and property management.

Care quality appears uneven. Reviewers cite delays in attending to residents, missed or inconsistent responses to call lights, and a lack of routine adherence to elements of care plans (for example, nonuse of bed or chair alarms specified in plans). Clinical documentation and care-plan paperwork are described as incomplete or slow to be updated, and several comments indicate limited on-site physician involvement. Together these patterns suggest weaknesses in clinical oversight and in monitoring systems that support timely, individualized care.

Staffing and staff conduct are frequent concerns. Reports describe workforce dissatisfaction, CNAs vocalizing poor working conditions, and inconsistent staffing levels that affect continuity of care. While some families noted instances of attentive caregiving, the prevailing pattern in the reviews is of uneven staff responsiveness and concerns about the tone and priority given to residents. These staffing dynamics also appear to contribute to operational problems such as inconsistent bathing schedules, delayed incontinence care, and supplies running out.

Dining, supplies, and facility operations show evidence of budgetary pressure. Reviewers mention lowered food quality, routine shortages of essential supplies (for example, briefs/pull-ups), and a perception that cost-cutting decisions affect daily resident comfort. Facility cleanliness and sanitation are recurring issues, including concerns about laundry hygiene and food-storage practices. There are also allegations of employee theft and examples suggesting weak controls around resident finances and property โ€” issues that raise questions about transparency and accountability.

Activities and social programming receive little attention in the reviews; families did not highlight a consistent or notable activity offering, and the lack of commentary may reflect limited emphasis in reviewersโ€™ experiences. Management and administration are singled out for a perceived operational focus on finances over resident-centered priorities, and reviewers describe difficulty obtaining requested documentation and clarity about resident accounts.

For prospective residents and families, the pattern in the reviews points to several areas to verify during a tour and pre-admission conversation: staffing ratios and turnover, procedures for responding to call lights and alarms, care-plan update processes, on-site clinical coverage, supply-management practices, policies for safeguarding resident property and finances, and recent inspection or licensing actions. The facility may be undergoing transition under new ownership, which some reviewers mentioned; that could present opportunities for improvement but also may mean current operational issues are in flux.

    About Mitchell Village Care Center

Mitchell Village Care Center sits out in the rural area of Mitchellville, Iowa, and provides care for seniors who need different levels of support as they age. It's a for-profit nursing home with 65 certified beds and usually about 63 residents filling those spots, with a modern building finished in 1996, so it doesn't look like some of those older homes built long ago. The center offers nursing home care with help from registered nurses, licensed practical nurses, and certified nursing assistants, so residents get a decent mix of direct-care staff around, but skilled nursing services don't run all day and night like in some places-the care's mostly intermediate, designed for folks who don't need constant medical help but can't quite live on their own. Seniors can find support for both short and long stays here, and the staff tries to create a homelike feel while focusing on each person's well-being, whether that person plans on staying or wants to recover and return home.

For people with memory problems, there's a specific Alzheimer's and dementia care program, which tends to cost more than regular care, but has structured support for those tough cases. Mitchell Village Care Center also has adult day services, home health (Medicare-certified), non-medical home care, and rehabilitation with occupational, physical, and speech therapy, so seniors have some options whether they need a full-time place or help at home. Hospice care and end-of-life options are available for those who need comfort toward the last part of life, and respite care lets caregivers take a break every now and then. Assisted living is on site too, with aides lending a hand with bathing, dressing, and daily needs, while staff take care of laundry, housekeeping, and basic help.

The center provides meals made by chefs and meal planners, using quality ingredients-nutritious and balanced plates for every resident. Social services help look after the emotional side of life, and there are councils where residents and families can speak up about their thoughts or needs. There's no hospital attached, and it's not part of a big chain or focused program, so the place stands on its own with local management. Mitchell Village Care Center participates in Medicare and Medicaid, and it's fully sprinklered for fire safety. The staff includes various specialists-doctors, nurses, assistants-and a registered dietician oversees meals. The atmosphere's aimed at being comfortable for most seniors, those who need more or less care, and there's a map if anyone needs help finding their way. Overall, this center tries to keep things straightforward, safe, and manageable for older folks in the area, especially for those who need a place that's not too fancy but gets the job done.
