    Excellent rehab care, attentive staff

I stayed for post-op rehab and was very pleased - the nurses, CNAs and therapy team (Myra, Shon, Brittany, Ivy and many others) treated me like family, listened to my concerns, and intensified PT/OT when needed so I left walking and with a full range of motion. The facility felt like home, was clean and safety-focused, staff went above and beyond (even arranging calls and visits), and I'm grateful - I would choose Nazareth again for rehab or nursing care.

    Summary of reviews

Nazareth Home - Highlands Campus presents a mixed but largely positive profile centered on strong rehabilitation services and a generally compassionate care culture. Across reviews there is consistent praise for the facility’s physical and occupational therapy programs — staff are described as skilled, encouraging, and outcome-focused, with many residents achieving measurable improvements in mobility and independence. Therapy staff and some named individuals received repeated commendations for individualized plans, intensive progress when needed, and strong outpatient follow-up. Families and residents frequently highlighted a warm, homelike atmosphere and the sense of being treated like family by many caregivers.

Clinical and direct-care strengths are complemented by facility-level positives: reviewers commonly noted clean, well-maintained rehab rooms and communal areas, an active calendar of activities, supportive spiritual services, and amenities such as an aviary and pleasant grounds. Meal service and dining were favorably mentioned, and staff have been recognized for arranging remote visits and facilitating family contact when needed. These operational elements support rehabilitation goals and daily quality of life for many residents.

Despite these strengths, several operational concerns recur and warrant attention. Some reviewers described instances where physician orders or individualized care plans were not consistently followed, which points to gaps in clinical-order adherence and interdepartmental coordination. There are reports suggesting delays in attending to personal-care and incontinence needs and variability in responsiveness from nursing staff; these accounts imply staffing strain or workflow issues that can affect continuity of care. Additionally, a minority of accounts raised concerns about staff conduct or communication tone and about inconsistent family updates, indicating opportunities to standardize communication practices and bedside responsiveness.

In summary, Nazareth Home - Highlands Campus appears to excel in rehabilitation services, individualized therapy, and creating a supportive environment for many residents. Prospective residents and families should weigh these strengths against the noted operational issues: ensure clear care-plan documentation, ask about current staffing levels and coordination processes between therapy and nursing, and clarify family communication pathways during a tour or pre-admission discussion. Doing so will help set expectations and identify whether the facility’s strengths align with the resident’s clinical and personal needs.
