Old Jefferson Community Care Center has a quiet atmosphere where elders and their families can find resources to help with health and wellness decisions, and folks can take part in programs meant to help them regain strength and independence after illness or injury, and there's a real effort to focus on person-centered care where staff tries to get to know each resident and look after their day-to-day needs with respect and compassion, making the place feel comfortable, almost like a big home instead of an institution. You can tell they've got a mix of care options, because they cover independent living, assisted living, skilled nursing, memory care, rehabilitation, and hospice care, depending on what someone needs at different stages, and those rehab to home programs are set up so folks can get back to their own houses if that's an option, with help from trained physical therapists, occupational therapists, and speech therapists down in the fully equipped gym-everybody gets support to reach their highest level of function. They're part of the CommCare® Corporation, so they're focused on serving the community instead of turning a profit, and there's a long history of volunteer involvement, with people coming in to share music, cooking, and stories about travel, plus families and the wider community often join in the activities. They've earned recognition too, like the 2025 American Healthcare Association Bronze National Quality Award. The center's main dining room is a spot for meals with table service and choices, but also for social things like playing wheel of fortune or enjoying time together, and there are refreshment bars with drinks, coffee, soft serve ice cream, and homemade cookies any time you want them, which folks seem to like. The resident rooms and common areas-like living rooms, a library, dining spaces, and a chapel-are comfortable and set up to help people feel at ease. Staff use less intrusive electronic charting systems to keep things running smoothly, so things don't get bogged down with paperwork. There's a steady schedule of life enrichment activities, outings, and social events where families, community folks, and volunteers all take part. People feel they're treated with dignity, and the focus is always on meeting each resident's individual needs in a down-to-earth, home-like setting.

Beautiful facility but unsafe nursing
I loved the look of the place - clean, hotel-like, great activities, decent dining and strong rehab/therapy - and many staff were warm and helpful. But I saw major, recurring problems with nursing: slow/ignored call buttons, understaffing, poor personal care, hygiene issues, infections, bedsores and even hospital transfers after rehab. Communication, responsiveness and some administrators were also unreliable, and long-term care is mainly semi-private rooms. Beautiful facility, but I would not trust them with a frail or medically complex loved one.
