    Attentive staff saved my family

I'm very pleased with this facility - the staff are consistently caring and attentive, nurses and CNAs go above and beyond (even providing head/neck support pillows), it's clean and home-like, activities are energetic under an excellent director, and admissions/DON/administrator are accessible and proactive. A nurse's quick intervention literally saved my family member, so I feel personally satisfied with the high level of care.

    Summary of reviews

The reviews for this facility are polarized, with families describing both strong, compassionate clinical care in some cases and significant operational problems in others. Several reviewers praised nursing staff who respond quickly in emergencies and CNAs or nurses who go beyond basic duties. The facility offers on-site therapy and an active activities program; some visitors described a home-like atmosphere and well-presented exterior and common areas. At the same time, other reviewers raised recurring operational concerns that suggest variability in day-to-day performance.

Care quality appears inconsistent. Positive accounts highlight competent nursing care and timely medical interventions, while negative accounts raise concerns about medication management, missed medication updates, and lapses in clinical monitoring. Reviewers also described wound-care and infection-risk concerns as well as weight and feeding problems; these indicate weaknesses in ongoing clinical oversight and nutrition-monitoring processes. The presence of high-acuity residents may contribute to the need for closer clinical supervision.

Staffing and staff conduct are recurring themes. Some families described attentive, caring staff and proactive issue resolution, whereas others reported insufficient staffing, CNAs performing minimal care during some shifts, and frequent turnover among management and clinical leadership. These patterns point to workforce instability and gaps in training or accountability. There are also allegations of theft and isolated claims of inappropriate staff behavior; such claims should prompt direct inquiry with both facility leadership and regulatory authorities.

Dining and daily living services were described variably. Several reviewers reported inconsistent meal delivery or feeding support and concerns about residents losing weight; others commented negatively on food quality. These indicate potential breakdowns in meal-service continuity and resident-specific feeding plans rather than a systemic absence of dining services.

Activities and social programming were generally a relative strength in many accounts. An energetic activities director and regular programming were cited as contributing to resident engagement. Therapy and rehabilitation services are available on-site, though families should verify therapy outcomes during a visit since effectiveness was described unevenly.

Facility condition reports are mixed. Some reviewers said parts of the building and grounds were clean and well-maintained, while others raised sanitation and odor concerns, leaks of nutritional supplements, and general cleanliness issues in resident areas. There were also mentions of surveillance cameras and related privacy concerns, which prospective families may wish to discuss with administration.

Management and communication practices drew consistent criticism from negative reviews: long phone hold times, disconnected calls, inaccessible administrator voicemail, persistent billing problems, and slow responses to family concerns. These operational communication failures compound clinical and staffing issues and make it difficult for families to get timely information.

Given the variability in experiences, prospective residents and families should perform an in-person visit, observe multiple shifts if possible, and ask targeted questions about staffing ratios, medication administration protocols, infection-control procedures, wound and nutrition management, incident reporting, and billing practices. Review state inspection reports and ask facility leadership for recent corrective-action documentation. The facility shows concrete strengths in nursing responsiveness, therapy availability, and activities, but also exhibits operational and oversight gaps that merit careful scrutiny before placement.

    About Jena Nursing & Rehabilitation

Jena Nursing & Rehabilitation sits over on Aimwell Road and 3rd Street in Jena, Louisiana, and you'll find it offers many things for people who need different types of care, and so if someone's looking for skilled nursing, long-term stays, help with memory care issues like Alzheimer's or Dementia, or just a short rehab after a hospital visit, this place has set up services to cover all those needs, and there's always 24-hour supervision and between 12 and 16 hours of daily nursing care, with physicians and respiratory therapists on hand at all hours. They've got a Memory Care community called The Seasons of Jena, set aside with special programming, activities, and extra support for people dealing with memory problems, and they offer palliative care, respite stays for caregivers who need a break, and non-ambulatory care for people who need extra help moving around, and you'll spot specialized caregivers-always trained for this kind of work-offering help and advice to residents and families. As for staying busy and social, inside there's a big activity room with scheduled events every day of the week, plus an arts room, game room, library, a dedicated movie theater, and a spa and wellness room; outside there are walking paths, an enclosed courtyard, another beautiful garden area, and plenty of outdoor programs to encourage people to get some fresh air or relax, and for trips to appointments or outings, there's transportation and parking provided as well.

The staff set up fitness and wellness programs, community events, and family support services so everyone feels included and cared for.

For the basics, housekeeping, laundry, move-in help, and business room services are part of the package, along with both Medicare and Medicaid certification in place for people handling the financing. Jena Nursing & Rehabilitation runs as a single-story facility with 108 beds and keeps the mood comfortable and supportive, aiming for patient-centered service and community wellness instead of anything too flashy, while offering a reliable spot for seniors or others needing steady nursing, rehabilitation, or memory care in this corner of Louisiana.
