    Excellent caring rehab, strong recovery

My mom was well cared for during her stay - attentive, organized, and genuinely caring staff (Mrs. Glenda handled everything) and professional CNAs. On-site PT/OT helped her make strong recovery progress and regain mobility at home; the facility is very clean, well-kept, quiet, and has a pleasant courtyard. Overall a very good rehab experience that I would recommend for in-patient rehab.

    Summary of reviews

Feedback about Ouachita Healthcare Rehabilitation Center is mixed, with a clear pattern: the facility demonstrates strength in short-term rehabilitation services and has several operational weaknesses that families should evaluate. Many reviewers praise the therapy teams and on-site PT/OT services for helping residents regain mobility and return home. Caregivers such as CNAs and therapists are frequently characterized as caring and effective, and common areas and outdoor spaces are described as clean and pleasant. These positive elements make the center a viable option for goal-directed inpatient rehabilitation.

At the same time, reviewers describe recurring operational issues that affect the overall experience. Communication problems are a consistent theme — families describe difficulty reaching clinicians and administration, unreturned calls, and challenges obtaining clear follow-up after incidents or discharge. Several accounts highlight medication-management gaps, including delays, improper administration, and post-discharge medication issues; these are service-level concerns that merit specific questions during intake and care planning. Staff conduct and responsiveness are reported as variable: while some caregivers are attentive and organized, others are described as having poor bedside manner, appearing distracted, or not responding promptly to call systems.

Dining and daily-care practices are another area of mixed feedback. Some families report meal temperature and timing issues and insufficient time for residents to finish meals; other reviewers note caring assistance from aides during feeding. Facility upkeep appears uneven — many reviewers note clean common areas and a well-kept courtyard, while a subset report room-level maintenance problems such as water damage, scuffed walls, or localized sanitation concerns. Activities and outdoor access are generally seen as positive for resident engagement.

Management and administrative processes are highlighted as a weakness by several reviewers. Concerns include limited responsiveness to phone inquiries, calls being forwarded or disconnected, difficulties with discharge coordination, insurance and relocation support, and perceived gaps in incident reporting and follow-up. There are also worries about infection-control practices and visitation restrictions during outbreak conditions; some families welcomed window visits while others found lockdown communication inadequate. A small number of serious individual accounts — including concerns following a resident's death — indicate the importance of reviewing clinical oversight and escalation procedures.

In sum, Ouachita Healthcare Rehabilitation Center offers strong rehabilitation programming, helpful therapy staff, and a generally clean environment that can support recovery-focused stays. Prospective residents and families should weigh these strengths against reported weaknesses in medication management, staff responsiveness, communication, and administrative follow-through. When evaluating the facility, ask for specifics about medication-safety protocols, staffing ratios and training, call-response metrics, incident-reporting practices, discharge planning and relocation support, and current infection-control/visitor policies to ensure those operational elements meet your expectations.

    About Ouachita Healthcare Rehabilitation Center

Ouachita Healthcare Rehabilitation Center sits in Lakeshore, Louisiana, with room for 167 residents and usually about 159 people living there each day. This center's run by Plantation Management Company and offers skilled nursing care, short-term rehabilitation stays, and hospice care, so families looking for support over the long term or just during tough times can find both options here. They've got general and specialized services for folks who need help with daily living, along with in-house physical, occupational, and speech therapies, and there's a secured Alzheimer's Unit for those who need extra safety, privacy, and places to walk or spend quiet time.

The building includes things like a chapel, library, activity room, cinema room, and an elegant dining area, and there's even a whirlpool bath along with a beauty and barber shop-all these things are included without added fees, and each room comes wired for phone and internet with cable TV as standard. Private suites, private rooms, couples' rooms, and semi-private rooms offer choices for different needs. Family caregivers can get resources and expert advice, too, if they visit the center's website. Staff provide care that tries to support not just physical wellness, but also emotional and spiritual well-being.

In terms of staffing, residents at Ouachita Healthcare Rehabilitation Center receive about 3.34 nurse hours per day, which is a bit less than the state average of 3.8, and nurse turnover is a bit lower here too, at 37.7%, compared to the state's 48.4%. Inspections reported 25 deficiencies in total, including 10 during standard inspections; these cover issues with infection control, pest control, and communication during discharge planning, though most weren't linked to actual harm but have potential for more than minimal harm. The health department gave the center a rating of 2.8 out of 5 based on 24 reviews. Overall, families will find many practical services and different amenities at this center, though it's important to be aware of its recent inspection results and the details shared in the reports.
