I stayed here for rehab and was very pleased. The staff were attentive, kind and professional-nurses, therapists and administration were knowledgeable, responsive and proactive with care plans. The facility is clean, welcoming to families, offers engaging activities and good communication; I felt well cared for and would recommend it.

    Summary of reviews

Rossville Rehabilitation and Healthcare Center elicits a mixed assessment from families and former residents. Many accounts highlight strong clinical and rehabilitative strengths: motivated therapists, measurable therapy progress, effective wound care, and helpful discharge planning. Numerous reviews praise specific staff members, friendly front-desk personnel, and administrators who are accessible and engaged. The facility also receives positive comments for its activity program — themed days, church services, poets, and varied recreational offerings — and for episodes of high-quality dining, particularly during special events.

At the same time, reviewers describe persistent operational inconsistencies. Staff responsiveness is a frequent area of concern; several accounts indicate slower responses during nights and weekends and long waits during shift changes. These responsiveness issues are tied to perceptions of understaffing and uneven staffing deployment, which reviewers connected to delays in assistance, delayed or omitted medication doses, and inconsistent adherence to prescribed diets. There are also recurring comments about medication-administration gaps and occasional medication mix-ups, which suggest opportunities for stronger medication-safety processes and oversight.

Cleanliness and facility condition are described in contrasting terms. Many families note clean rooms, maintained floors, and a well-kept first floor; however, other reviewers raise sanitation and housekeeping concerns, odor issues in particular areas, and pest-control concerns. The building’s exterior and some rooms are characterized as dated, and a number of reviewers mention cramped shared rooms or multi-resident rooms as a limitation for privacy and comfort.

Dining feedback is highly variable: while special-event meals and some menu items draw enthusiastic praise, a substantial number of reviewers describe inconsistent meal quality, portion size complaints, and failures to consistently meet dietary restrictions. Activity programming is frequently cited as a strength — staff-led crafts, games, and services are appreciated — but some families express that activity reminders and individual engagement can be inconsistent, especially for residents who need prompting.

Management and communication show a split pattern. Several families commend specific administrators and leaders for being proactive and supportive, and cite positive interactions around care planning and transitions. Conversely, other reviewers describe unresponsive management, poor follow-up on complaints, billing concerns, and at least a few serious allegations involving financial misconduct; those allegations may warrant direct inquiry and verification with oversight bodies. Clinical oversight concerns are also highlighted in reports of limited on-call physician availability and inconsistent monitoring practices.

Notable patterns for prospective families: rehabilitation services and many direct-care staff receive strong praise and can deliver meaningful functional gains, but operational variability—especially around staffing levels, medication safety, and nighttime responsiveness—appears to drive most negative experiences. When evaluating this facility, families may find it useful to ask about staffing ratios by shift, medication-administration protocols, on-call physician coverage, infection-control and housekeeping procedures, dining accommodation processes, and options for private or less-dense rooming. Observing a mealtime and an activity, and seeking recent survey or inspection records, can help clarify whether the strengths highlighted by many reviewers are consistent with the experience you or your loved one require.

About Rossville Rehabilitation and Healthcare Center

Rossville Rehabilitation and Healthcare Center is a large skilled nursing and rehabilitation community that cares for seniors and others who need different levels of medical help over both the short and long term, and you'll find they provide hospice care for end-of-life needs, specialized memory care for folks with memory issues, independent and assisted living, as well as supportive home care services-including both non-medical and Medicare-certified medical care at home if that's needed, so folks have a lot of options all in one place. The center operates with 172 certified beds and takes in residents for long-term stays or those just needing rehab before going back home, while also offering options such as adult day services and support for active adult living, so you get a good variety of services under one roof, and the rooms can be studio, semi-private, or shared with a companion if you prefer. Skilled nursing and intermediate care are available for very frail residents who depend on round-the-clock nursing, and dialysis is on site, with nursing care and rehabilitation managed by a team that aims to meet different personal goals for health and daily life, and their programs are set up to focus on individual needs.

Their nurse turnover rate is 30.6%, which is lower than the state average, but nurse hours per resident per day stand at 3.19, which is below the state average, so while staff may stick around longer, there might be fewer hours per person, and the facility's average rating is 5.7 out of 10-ranking them as the 85th highest rated in their city, while the Centers for Medicare & Medicaid Services gives them 3 stars for nursing home care. There are also some deficiencies to mention: the center has had complaints about protection from abuse and neglect, including issues with failing to report suspected problems, and there were recent deficiencies listed as immediate jeopardy for some nursing and physician services, meaning there were risks to health or safety, plus a deficiency about infection control, and, altogether, inspection reports have noted 53 different deficiencies. On February 2, 2024, a complaint report cited four deficiencies and the center has been fined $157,250 and had payment suspended once after a complaint. Ownership is by Trr Snf Operations Holdings LLC (100%) with indirect interests from several family trusts and Aaron Oppenheimer, and Atlas Healthcare is the facility's affiliate.

Overall, Rossville Rehabilitation and Healthcare Center has most every type of senior care under one roof, with certified beds and a multidisciplinary team ready for long-term care, memory care, rehabilitation, skilled nursing, home care, and hospice, and their programs are aimed at helping residents feel connected and well cared for, but inspection reports and ratings show there are some areas with room for improvement and concerns to keep in mind about staffing, infection control, and resident safety.
