    Home-like facility with compassionate staff

I chose this small, home-like facility for my mom and have been very pleased - manicured grounds, secure entry, gorgeous views, and a warm, welcoming atmosphere. The staff (reception, nurses, therapists and Life Enrichment team) are attentive, compassionate and professional; care and rehab have been excellent and I'd recommend it to families seeking personal, dignified care.

    Summary of reviews

Autumn Lake Healthcare at Braddock Heights presents a mixed but distinct profile: reviewers frequently praise the facility’s scenic location, small boutique atmosphere and an involved Life Enrichment program, while also raising operational concerns that prospective residents and families should evaluate during a tour.

Care quality and staff: Many accounts highlight compassionate, attentive caregivers, therapists and nursing staff who develop personal relationships with residents. Inpatient rehabilitation and therapy services receive positive comments for individualized and group therapy, and several families report meaningful recovery and improved wellbeing. At the same time, there are recurring operational concerns: reviewers describe staffing shortages, high turnover, and delays in responding to call bells or assisting residents. These issues manifest as prolonged wait times for personal care and sporadic lapses in hygiene assistance. Rehabilitation outcomes are described as strong by some families and limited by others, suggesting variability in clinical effectiveness that may depend on timing, caseload, or staff availability.

Dining and nutrition: Feedback on food service is mixed. The facility hosts social meals and special events that families appreciate, but reviewers also note inconsistent meal timing and occasional reports of poorly cooked or cold food—with weekend service continuity cited as an area of particular weakness. Prospective families should inquire about current meal schedules, kitchen staffing, and weekend coverage during visits.

Activities and social life: The Life Enrichment Department and volunteer-supported events are frequently praised for providing stimulating programming and opportunities for family participation. Reviewers describe engaging, action-packed activities, themed events (for example, holiday teas and family-friendly trick-or-treating) and staff who take initiative to involve residents, which supports social wellbeing in a smaller community setting.

Facilities and maintenance: The property’s location, outdoor seating, manicured lawns and secure entry are consistent strengths. However, there are multiple comments indicating maintenance issues inside the building—peeling paint, elevated temperatures, mold in areas, broken fixtures, and pest-control concerns—alongside reports of long-running audiovisual/connectivity problems in common areas. Room configurations also vary; some rooms lack private bathrooms. Housekeeping reports are mixed: some reviewers describe a very clean, home-like environment, while others describe sanitation and odor concerns in certain units. These contrasting observations suggest variability across wings or shifts.

Management and communication: Several staff members and administrators receive positive mention for helpfulness, rapid greeting on arrival, and good family communication. Conversely, there are complaints about admissions and administrative responsiveness, slow corporate approval for equipment and supplies, and inconsistent enforcement of smoking policies. These managerial and policy-level gaps appear to affect both daily operations and timely access to durable equipment.

Notable patterns and considerations: The dominant pattern is one of strong interpersonal care and a supportive social environment tempered by operational inconsistencies—particularly around staffing, maintenance, sanitation, meal service, and administrative responsiveness. Prospective residents and family members should schedule visits at different times (including weekends), ask about current staffing ratios and turnover, review sanitation and pest-control records, verify in-room bathroom availability, confirm equipment approval timelines, and observe meal service to form a balanced assessment before deciding.

About Autumn Lake Healthcare at Braddock Heights

Autumn Lake Healthcare at Braddock Heights sits in a large, nicely kept chateau with views of Braddock Mountain, and while it's got 65 certified beds, most days you'll find around 55 residents living there, so it's not overly crowded and there's a steady community feel. Skilled nurses and a team of therapists are there day and night to help with medical needs, but nurses spend about 3.32 hours with each resident per day, which falls below the Maryland average of 3.9 hours, and the nurse turnover rate stands higher at 54.3% compared to the state's 41.5%. Over the years, inspectors have cited the center for 54 areas needing improvement, with six specific issues related to infection control, and the records show problems such as not always honoring residents' rights to voice concerns or giving timely help with daily activities.

The facility is part of the Autumn Lake Healthcare network, though it runs on its own under the direct ownership of As Family Vb Holdings LLC and M Meisels Family Holdings LLC, so that's who holds responsibility there, while all healthcare services are provided on-site and there's no public info about the people managing daily operations. The place does have round-the-clock skilled nursing and advanced specialty care programs provided by a team of in-house, board-certified specialists, so people with complex health conditions can get care as needed. Rehabilitation is available every day, with a gym full of modern equipment and plans shaped around each resident's needs, aiming to help folks recover and regain strength.

Residents have choices between triple, quad, semi-private, and private rooms, many of which are bright and homey, with WiFi and cable TV if you want to keep busy in your own space or use an iPad for activities. Meals come with a menu full of healthy, tasty options and can be eaten in a cheerful dining room or in your room if you're more comfortable staying put. The property features open courtyards, a resident garden, and mountain views if you want some fresh air, while lounges inside help people gather and enjoy each other's company.

Autumn Lake offers many activities, ranging from bingo, art, music therapy, and cooking classes to manicures, exercise groups, outings, and holiday parties, and there's a monthly calendar always filling up with something new. Religious services are available if you want them, and a Resident Council gives folks a chance to share their ideas or raise issues. For those with dementia or memory care needs, there are specialized activities and a Life Enrichment Program to boost engagement and quality of life. The staff, even with changes in nursing and some past issues found by inspectors, work to merge care and comfort, hoping residents live each day fully and feel part of an active, caring place. The connection to the Autumn Lake network means resources and ideas are shared with sister centers in states like New Jersey, Wisconsin, Connecticut, West Virginia, and Maryland, which helps support different needs and styles of care.
