    Kind staff, clean facility, reassuring

I'm very pleased with this community - staff from aides to nurses, therapists and leadership are consistently kind, responsive and go above and beyond (special thanks to Florence, Claudia, Ann and Baily). My mother is well cared for, comfortable, enjoys activities and her glider rocker, the food and dining are good, mornings have free coffee, and the facility is exceptionally clean. Overall a warm, supportive place that gave our family real reassurance.

    Summary of reviews

Care quality at this facility appears mixed, with a clear split between families who praise individualized, compassionate caregiving and those who describe delays and missed needs. Positive accounts emphasize attentive nurses, aides who go beyond basic tasks, and skilled therapists who deliver meaningful rehabilitation. Conversely, a recurring operational pattern is delayed personal assistance and slow call-bell response, which families identified as a primary concern for daily care and safety.

Staff and clinical services receive both commendation and criticism. Several reviewers singled out specific clinicians and therapists for professionalism, clear communication, and effective hands-on care; hospice coordination was also described as compassionate. At the same time, reviewers frequently described understaffing, high turnover, and stress on frontline workers, which reviewers linked to slower responses, inconsistent dressing and toileting assistance, and occasional supervision gaps such as residents entering other rooms or requiring repeated prompting.

Dining and activities are similarly inconsistent. Some families report tasty, well-balanced meals, an attractive dining room, and an active schedule of daily programming; other families find meal quality and timing unreliable and beverage options limited. Activity offerings and communal spaces were noted positively by some but absent or insufficient by others, and the facility lacks clearly defined outdoor visiting areas and comfortable sit-and-talk spaces according to several accounts.

Facility condition and cleanliness vary across units and floors. Multiple reviewers praised a clean, well-maintained environment and timely housekeeping, while others noted sanitation concerns in specific areas, dark or cramped rooms, and maintenance needs. These differences appear to be unit- or floor-specific in several descriptions, with some floors cited as consistently better maintained than others.

Management and operational concerns are prominent in the reviews. Themes include chronic staffing shortages, inconsistent interdisciplinary communication between nursing, therapy, and dietary services, and variability in leadership responsiveness—though some families described administrators as responsive and engaged. A regulatory complaint and at least one reviewer filing a complaint with the Department of Public Health were mentioned, and isolated serious allegations (including theft) were raised by reviewers.

Overall, prospective residents and families will encounter strong positives around individualized, compassionate care and skilled therapy in many cases, but should also be aware of recurring operational weaknesses tied to staffing, responsiveness, and consistency across units. Recommended steps for decision-making include an in-person tour at different times of day, direct questions about staffing ratios and call-bell response metrics, review of recent inspection and complaint history, and asking how the facility addresses unit-to-unit variability in cleanliness, activities, and meal service.

About Beaumont Rehab & Skilled Nursing Center - Westboro

Beaumont Rehab & Skilled Nursing Center - Westboro belongs to the SALMON Health and Retirement family and sits as a large nursing home where people get help with daily life and health, focusing on skilled nursing, rehabilitation, long-term support, and various therapies. The facility manages 152 certified beds, and usually cares for around 136 residents a day, offering private suites and shared rooms, each with private bathrooms, air conditioning, and WiFi, which people appreciate for comfort though it does feel like a busy place with a lot of folks coming and going, and the nurse turnover rate sits higher than average. Residents have daily needs met, like help with dressing and bathing, medication management, and meals cooked and served, and there's always someone available with 24-hour supervision and call systems in place, so there's a sense of safety even for those who wander or need more security, especially for memory care.

The center has programs for short-term rehabilitation after a hospital stay, ongoing long-term care for those with chronic illness, and helps all sorts of people with many health problems, using an interdisciplinary team involving physicians, nurses, social workers, therapists, and more, all with skills in things like physical therapy, occupational therapy, speech therapy, and other specialty services such as dental, vision, and psychiatry. There is also the Tapestry Memory Care program-they handle Alzheimer's and dementia in a more focused way. Residents can join therapy sessions every day of the week, including balance exercises, gait training, and wheelchair services, and those who finish their rehab sometimes use SALMON Home Care for continued help at home. There's a commitment to patient-centered care, respecting residents' choices, frequent communication with family, special education for staff, and attention to safety like fall prevention and trying to avoid too many unnecessary medicines.

Amenities cover basics like housekeeping, linen service, transportation to appointments, telephones, internet, cable, and meals, and the atmosphere aims to feel warm and flexible, to help everyone reach the best health and independence possible. There's a special effort to offer dignity, focused quality, and a safe, well-monitored environment, but public inspection reports show the place faced 23 deficiencies, including not always coordinating well with required screenings, missing some trauma-informed, culturally aware care, and one infection-related issue, yet these didn't lead to actual harm though they could have been better. The facility runs under the leadership of Lisa Coppola and Continuing Care Management LLC and keeps a steady focus on improving things like antipsychotic use and resident quality of life. Families find the staff knowledgeable and communicative, but nurse staffing hours come in below the state average, so folks who want more one-on-one attention might notice that. All in all, this is a place with lots of support and programs for older adults needing different kinds of care, where safety, routine, and rehabilitation stand out, though it's not without areas that need work.
