    Compassionate dementia care, excellent rehabilitation

I placed my mom at Clearstream for dementia care and I'm very pleased - the staff are compassionate, attentive, and consistently go above and beyond. Eva (activities director) and the nursing leadership are wonderful; there are lots of inclusive events, strong therapy for stroke recovery, frequent family updates, and a warm, home-like atmosphere. Rooms have been upgraded, residents are engaged and happy, and I confidently recommend Clearstream for rehab and long-term care.

    Summary of reviews

Clearstream Rehabilitation and Nursing Center elicits a mixture of positive and concerning impressions. Many families and visitors highlight strengths in direct resident care, rehabilitation services, and activities programming. Nursing and caregiver staff are frequently described as compassionate, attentive, and person-centered; several accounts describe nurses and CNAs taking extra steps to communicate with families, kneeling to speak with residents at eye level, and delivering individualized rehabilitation after stroke or surgery. Therapy and rehab services receive specific praise for supporting recovery and functional gains.

Staffing and care consistency are a recurrent theme on both sides of the ledger. Positive reports emphasize long-tenured employees, a sense of familial culture among staff, and visible nursing leadership (including a highly regarded director of nursing). Conversely, other commentary points to variable staff engagement, teamwork challenges across departments, and inconsistent overnight supervision. Related operational weaknesses include delays in resident checks, concerns about pain-management practices, and gaps in medication administration protocols. These patterns suggest the facility can provide high-quality care but may have uneven performance tied to staffing levels and shift coverage.

Dining and housekeeping feedback is mixed. Several reviewers described clean rooms and recent facility upgrades, a home-like atmosphere, and well-run holiday or family events. The activities department is repeatedly called out as a strength — inclusive programming, seasonal events, child-friendly visits, and an engaged activities director contribute to a lively social environment. At the same time, there are recurring notes about menu appeal and meal quality, and some reviewers raised sanitation and odor concerns in particular areas. These observations point to generally acceptable maintenance with room for improvement in consistent housekeeping and food services.

Operational and administrative issues appear as a second major pattern. Positive experiences cite effective family updates and rehabilitation coordination, while critiques reference limited office hours for family access, restricted access to personal belongings, inconsistent admission/insurance acceptance decisions, and uneven responsiveness from management or administrators. Communication breakdowns between departments were reported, producing frustration when care coordination or information transfer is required. There are also isolated but serious reports alleging inappropriate staff conduct; these allegations warrant follow-up and verification through facility management or regulatory channels.

Overall, Clearstream shows clear strengths in hands-on caregiving, rehabilitation outcomes, and an active activities program, supported by committed individual staff members and some strong clinical leadership. Offsetting those strengths are operational inconsistencies tied to staffing, interdepartmental communication, dining consistency, and policies governing personal items and access. Prospective residents and families should consider an in-person tour, ask about current staffing ratios (including overnight coverage), medication and pain-management protocols, meal planning, property-access policies, and the facility’s process for handling complaints or serious conduct concerns before making a placement decision.

About Clearstream Rehabilitation and Nursing Center

Clearstream Rehabilitation and Nursing Center sits at 240 E North St in Hastings, MI, and folks will find a skilled nursing and rehab facility with several care options, like Active Adult Living, Independent Living, Assisted Living, Memory Care, Home Care (Non-Medical), Adult Day Services, Skilled Nursing for long-term care, and Hospice Care so seniors with different needs find a spot here, with a special transitional care unit and a separate long-term care area, plus a secured memory care neighborhood for people needing more supervision. Nurses provide around-the-clock care, and the place gives physical, occupational, and speech therapy, and people say the staff is dedicated and knows families and residents well, especially since many come for short-term rehabilitation before heading home from the hospital. Medicare and Medicaid are accepted, and they do offer Medicare-Certified Home Health Care if you qualify.

Folks need to know that government reports show Clearstream Rehabilitation and Nursing Center has a rating much below average for overall quality, health inspections, and staffing, even though it gets an above average score for quality measures. Records show it's had 44 total cited deficiencies, including three infection-related ones, and it's been cited for failing to meet resident rights, proper care planning, and dietary practices, plus some violations where residents weren't always given a safe, clean, or comfortable environment. The nurse turnover here is higher than the state average, with about 66.3% versus 46.9%, and the daily nurse hours per resident are below the state average, coming in at 3.38 instead of 4.0 hours. There's space for 98 certified beds, though the last count shows about 84 residents a day, but right now, they aren't accepting new patients and clinic staff speak English. For ownership, Renewal Healthcare LLC owns the facility outright, while Eliyahu Freund, Moshe Gottlieb, and Joshua Koenig hold most indirect stakes. The facility is for-profit and follows inspection rules set by the Centers for Medicare and Medicaid Services. There's no posted information on who manages or employs staff. Some people know Clearstream Rehabilitation and Nursing Center for the range of care it provides and the therapy programs available, but folks should also be aware of the noted problems identified in inspection reports.
