    Excellent rehab care; compassionate staff

I'm very pleased with the care my family and I received. The rehab program and PT/OT team were outstanding, and nurses and admissions staff (special thanks to Hazel Findlay) were compassionate and went the extra mile. The facility is clean and secure with large rooms, lovely grounds and a covered outdoor area, family-friendly visiting, and tasty, plentiful meals. Overall a very good place close to home for continued rehab.

    Summary of reviews

Reviewers present a mixed but focused picture of Hazel Findlay. Positive commentary centers on individual caregivers and therapy staff, the facilityโ€™s outdoor areas, and certain operational elements such as housekeeping and visitation flexibility. At the same time, multiple reviews identify recurring operational weaknesses that prospective residents and families should evaluate carefully.

Care and staff: Many accounts praise compassionate, attentive nurses and aides, with multiple mentions of staff who go the extra mile. The occupational and physical therapy teams receive frequent commendation for effective rehabilitation work and supportive rehab programming. However, these strengths are tempered by reports of inconsistent staff performance and professionalism across shifts or units. Common operational concerns include delayed responses to assistance requests and occasional coordination glitches that affect scheduling and continuity of care. Reviewers also raised concerns about medication-timing inconsistencies that warrant direct inquiry during a tour.

Dining and activities: Feedback on meals is mixed. Some reviewers describe meals as tasty and plentiful, while others identify limited dietary accommodations (notably for residents needing diabetic-friendly options) and variable food quality. Activity programming likewise varies by unit: creative and engaging offeringsโ€”arts, outings, and an on-site Massโ€”are noted, but other residents experienced a more limited, repetitive schedule (bingo, cards, occasional outings). Families should ask about individualized engagement plans and diet-management protocols.

Facilities and environment: The campus and grounds, including covered outdoor areas and flowered landscaping, receive positive mention, as do reports of a clean and secure entrance and common spaces. Conversely, the physical plant shows signs of age: outdated wings, inadequate climate control or lack of air conditioning in some rooms, and a generally clinical or โ€œhospital-likeโ€ room setup that limits personalization. Room size and a large dining room are positives, but reviewers also point to limited therapy space in places and inconsistent availability of designated rehab rooms.

Management and notable patterns: Several operational themes recur: gaps in oversight and supervision, variability in how different units are run, and coordination/communication issues between staff and families. There are also more serious claims in the reviews, including allegations of managerial misconduct and legal concerns; these are significant and should prompt direct questions to administration and review of licensing and inspection records. Infection-control and quarantine handling was raised as a concern in a subset of accounts, suggesting prospective families should ask about current infection-prevention policies.

Recommendation for families: Hazel Findlay demonstrates clear strengths in individual caregiving and therapy services, alongside pleasant grounds and some strong operational elements. At the same time, the facility appears to have variability in staffing, diet accommodations, physical plant condition, and management consistency. Prospective residents should schedule an on-site visit that includes: meeting nursing and therapy staff, reviewing medication and dietary protocols, checking room climate control and customization options, asking about staffing ratios and supervision, and requesting recent inspection/licensing records. Doing so will help determine whether the facilityโ€™s strengths align with the residentโ€™s clinical and lifestyle needs.

    About Hazel Findlay

Hazel I Findlay Country Manor in St. Johns, MI, offers several kinds of senior living, including long-term care, assisted living, retirement services, and nursing care. You'll find specialized care for Alzheimer's and options for respite and hospice stays. Hazel Findlay covers a lot, with help for daily living, medication management, and skilled nursing. People can use rehab services like physical, occupational, and speech therapy, plus cardiac therapy, pain management, stroke recovery, and orthopedic and neurological rehabilitation. The staff help with nutritional counseling, diabetes care, recreational therapy, respiratory needs, and pharmacy services, aiming to make things comfortable for everyone. There's a focus on a peaceful setting, but they don't shy from the work-nurses and assistants staff the facility at above average levels, which means more help is often close by compared to many other places. They do daily activities and have therapy equipment on site, and Clinton Commons is on the grounds, too. Hazel Findlay is a non-profit and takes Medicaid and Medicare. Visiting runs each day from 8:30 AM to 6:00 PM, with options for different hours if you arrange beforehand or use the buzzer system. Infection control includes COVID-19 testing if wanted. The center earned recognition from Newsweek as one of America's Best Nursing Homes for 2024 and also got a Bronze Commitment to Quality Award from the American Health Care Association. Hazel Findlay works under a continuing care model they call "Close to Home." There aren't many reviews, but people can find a photo gallery and more details online. CALD renewal and refresher courses, a Certified Assisted Living Director program, and regulatory resources for HFA & AFC are part of the offerings for those who need them. You won't find a lot of frills described, but the basics and key services get attention, and they stick to clear routines and support systems for seniors and families.
