    Warm staff, restored mobility, safe

I'm very pleased with my stay - staff are warm and attentive, nurses and aides know residents by name, and leadership and business office (Mrs. Boatwright, Amarjit) are helpful. Physical and occupational therapy (Natalie, Brad and team) restored my mobility, Activities (Claire) kept days fun with music, bingo and the salon, meals are good, facility is clean, and I felt safe and cared for - I'd recommend it.

    Summary of reviews

Four Chaplains Nursing Care Center elicits strongly mixed feedback. Many families and former patients praise the facility for its robust rehabilitation services, attentive therapy teams, and a caring nursing and CNA workforce that contributed to observable mobility and functional gains. Reviewers repeatedly note effective physical and occupational therapy, concrete walker-to-cane progressions, and therapy staff who are encouraging and professional. Common areas and public spaces are frequently described as clean and welcoming, and several accounts highlight engaged activities (bingo, exercise classes, movies, salon services) that contribute to residentsโ€™ socialization and daily structure.

At the same time, reviews indicate substantial variability in day-to-day clinical care and operational reliability. Staffing levels and responsiveness emerge as recurring concerns: delays in answering call signals, long waits for assistance, and uneven performance between shifts are described. These staffing patterns correlate with reports of inconsistent wound care, hygiene support, and medication communication. Some families experienced problems with discharge planning, medication reconciliation, and inadequate home-transition supplies, indicating gaps in care-continuity processes.

Dining and amenities receive mixed assessments. Many reviewers praised the kitchen for tasty meals and accommodating service; others reported cold, bland, or inconsistent food and occasional missed items. The activities program and on-site amenities are often cited as strengths, but therapeutic availability can be disrupted (for example, during infection-control events), which impacts rehab timelines for some residents. Facility maintenance of common spaces is generally favorable, although resident rooms are repeatedly described as small, occasionally outdated, and in some units not fully wheelchair accessible; shared restroom arrangements reduce privacy for some residents.

Management and leadership impressions are also mixed. Several reviews commend an accessible, involved Director of Nursing and responsive business-office staff who address concerns and coordinate care. Conversely, there are accounts of unprofessional staff conduct and serious interpersonal incidents; one review described an alleged inappropriate supervisory action. Communication shortcomingsโ€”delayed family updates, documentation errors, and occasional difficulty reaching administrationโ€”appear often enough to be a consideration when evaluating placement.

Notable patterns for prospective residents and families: the facility can offer strong short-term rehab outcomes and a supportive social environment when staffing and therapy access are consistent. However, variability in clinical consistency, discharge-planning practices, and infection-control cohorting suggests the need for careful, individualized assessment. For families considering Four Chaplains, recommended due diligence includes in-person observation of staffing levels on different shifts, direct questions about therapy schedules and contingency plans during outbreaks, review of discharge and medication-reconciliation procedures, and verification of accessibility and privacy in specific rooms. Those seeking hospice or continuous high-acuity skilled nursing should probe recent experience and protocols carefully, as end-of-life care and consistent 24/7 clinical responsiveness received notably mixed feedback.

About Four Chaplains Nursing Care Center

Four Chaplains Nursing Care Center, also called Four Chaplains Convalescent Center, sits in Westland, Michigan, and has been around since 1965, serving seniors in the Westland and Wayne County area with a range of nursing care, rehabilitation, and convalescent services. The facility has 103 rooms, some with private suites and shared "Jack & Jill" bathrooms, plus central air conditioning and Wi-Fi, cable TV, and kitchenettes, so folks who stay get a comfortable living space with modern touches like flat screen TVs and restaurant-style dining along with a barbershop, salon, and a library. Folks find both short-term rehab and long-term care here, with skilled nursing and rehab services managed by NexCare, physical, occupational, and speech therapy every day of the week, and RN coverage 24/7, which helps people recover from illness or surgery or get the care they need just day to day, whether that's help with dressing, bathing, medication management, or even transfers and daily activities. The staff stays present around the clock, and there's a 24-hour call system, so support's always at hand, even for those with mobility challenges or folks needing non-ambulatory care support. Safety's a big focus with things like the Clorox Total 360 System, Aquawing Ozone Laundry System, plexiglass shields, UV lighting, and a temperature scanner kiosk. The center offers a family-style atmosphere, daily activities, off-site trips, social activities, regular live entertainment, and spiritual support, keeping things lively, while the community areas include activity and arts rooms, a game room, movie theater, store and restaurant, music programs, and outdoor spaces with paths and gardens. There's also Wi-Fi, a fitness room, a spa/wellness center, housekeeping, laundry, a concierge, transportation, and parking to keep things easy for residents and visitors. Meals are served restaurant-style, but room service is available when needed. Four Chaplains Nursing Care Center runs as a for-profit community, accepts Medicare and Medicaid, and lets residents-through their own Resident Council-have a say in improving life and care in the center, with staff aiming to treat people like family. There are services for respite and hospice for those who need them, along with support to help people transition from hospital to home, and the option for assisted living. With a dedicated team and personalized care plans, folks can expect support with everything from medication to daily routines, and with amenities like salon/barbershop, movies nights, daily programs, music, arts, and outdoor activities, along with rehab, medical, and care services, the center stays focused on making it a safe and supportive place for seniors to live.
