    Attentive rehab care and support

I stayed two weeks for knee replacement rehab and found the nurses (especially head nurse Jen), aides and therapy team professional, patient and genuinely resident-focused - PT helped me progress and discharge was efficient. There were plenty of activities and outings, the food was very good, and staff went above and beyond; I felt cared for and supported the whole time.

    Summary of reviews

Park View Senior Health & Living Health Care Center receives a mix of strong praise for its clinical rehabilitation capabilities and hands-on staff, alongside recurring operational concerns. Many reviewers described positive therapy outcomes—particularly for joint-replacement rehabilitation—with physical-therapy teams characterized as coordinated, professional, and goal-oriented. Nursing staff are frequently noted as compassionate and supportive, and some families highlighted individualized care planning, effective discharge coordination, and emotionally attentive end-of-life support. The facility provides on-site skilled nursing and rehabilitation services and offers activities and transportation that families found valuable for resident engagement.

Despite these strengths, a set of consistent operational issues emerges across reviews. Cleanliness and sanitation practices were described as inconsistent, and several accounts referenced odor concerns and housekeeping lapses. Climate control problems (rooms reported as uncomfortably warm) and dated infrastructure, including shower hardware and other fixtures, were raised repeatedly. Reviewers also described delays in responding to nurse-call requests and personal-care assistance, which they associated with staffing shortages and variability in staff responsiveness. These operational patterns can affect timely bathing, toileting assistance, and other basic-care routines.

Clinical practice concerns include variability in monitoring and documentation; a few reviews noted gaps in wound care follow-up and laboratory monitoring. Communication and administrative processes were also uneven: families praised some case-management and discharge experiences but cited other instances of misinformation, billing surprises, and inconsistent communication about equipment or plan details. Dining received mixed comments—some residents enjoyed the food and menu assistance, while others experienced limited portions, substitutions, or inconsistent meal execution. Activity programming and transportation were consistently cited as positive contributors to resident quality of life.

For prospective residents and families, Park View appears to offer effective, rehabilitation-focused clinical care when therapy and attentive nursing are priorities. At the same time, families should inquire specifically about current staffing levels, housekeeping routines, climate-control in rooms, policies on bathing and call-response times, and billing practices before admission. Asking for recent quality reports, a tour of typical resident rooms and bathing facilities, and contact with the therapy team can help set expectations and identify whether the facility’s operational strengths align with the resident’s needs.

Park View Senior Health & Living Health Care Center sits right next to Buffalo Lake in Buffalo, Minnesota, offering a quiet setting close to Sturges Park and a few blocks from downtown. The community provides many choices for living and care, with services ranging from independent and assisted living to memory care, skilled nursing, and even hospice. The center has more than 120 beds for skilled nursing care and offers both short-term rehab and long-term stays, so someone can recover and go home or move in for more support if needed. There's an adult day center for people who live at home but benefit from daytime help or social time, and staff are known for being friendly, joyful, and patient, including people like Nurse Jen who make a difference with their care.

Residents might enjoy organized transportation for trips to local restaurants, attractions, and nearby parks, or spend time in the gardens, on the patio, or relaxing in a sunroom or lounge. The apartments and suites are built for comfort, and some have full kitchens so people can keep cooking if they like, but there are also three nutritious, chef-prepared meals every day served in the dining room. There are wellness programs, fitness centers focused on balance and movement, a whirlpool spa room, as well as a library for quiet reading or learning. The in-house beauty salon and barber shop provide personal care right on site, and the activities calendar offers something different every day-clubs, Bible studies, men's groups, entertainment, spiritual activities, and educational seminars.

Park View offers health care services around the clock, including skilled nursing, help with daily needs like dressing or taking medicine, therapies such as physical and speech therapy, and extra support for people with diabetes, incontinence, or mobility problems. The memory care section uses special routines and layouts to help reduce confusion and encourages residents to stay engaged, all with staff who have been trained to support those with Alzheimer's or dementia. Emergency call systems and secured building entry promote safety and peace of mind for residents and families.

There are several places of worship, hospitals, pharmacies, doctors, and restaurants right nearby, so families don't have to go far for anything important. Both the staff and the environment at Park View focus on supporting each person's abilities and independence for as long as possible while adjusting care as needs change over time. The community is part of Elim Care, which specializes in helping seniors and their families find resources and solutions for long-term care. Overall, Park View stands out for its active daily life, lakeside location, and careful attention to a wide range of health and living needs.
