    Caring, home-like facility; highly recommended

I toured and chose Pathstone Living and feel very comfortable - the staff are consistently caring, friendly, and available 24/7, with attentive CNAs and nurses and strong rehab support. The building is tasteful and home-like (not a nursing-home feel) with a cafe, lounges, garden, aviaries and aquariums, chapel, salon, and nicely decorated rooms with private baths. Meals are delicious and nutritious with choices and patient dining staff, and activities/entertainment (music, dances, walks, games) are offered regularly. Visits and family access are easy, emergencies were handled professionally, and overall I'm very satisfied and would recommend it.

    Summary of reviews

Ecumen Pathstone elicits a wide range of experiences. Many reviewers praise the campus: the property, landscaping and common spaces (café, chapel, lounges, bird aviaries) are described as attractive and tastefully updated. The facility offers private rooms with private baths, on-site rehabilitation and therapy services, an in-house salon and fitness/therapy amenities that support positive short-term rehab outcomes for some residents. Families and residents commonly cite engaging activities — musical performances, games, walks and frequent programming — and several note that these offerings contribute to resident wellbeing.

Care quality descriptions are mixed. Positive accounts emphasize attentive CNAs and hands-on caregiving, reports of sustained mental clarity and successful rehab stays. Conversely, other accounts raise clinical-safety concerns: delayed discovery of falls, subsequent hospitalizations, and weight/health declines for some long-stay residents. These observations suggest variability in day-to-day clinical oversight, particularly in situations requiring timely monitoring or rapid intervention.

Staffing and communication also show divergence. Supporters describe warm, helpful staff, strong therapy teams, and professional emergency handling with follow-up contact. Critics describe inconsistent staffing levels, staff distraction (including cellphone use), and delays in responding to needs. Several reviews highlight gaps in family notification and follow-up after significant incidents, and at least one mentions concerns following a resident's death and the handling of that event.

Dining and nutrition receive mostly favorable comments for menu variety, alternative selections, and helpful dining staff; many families describe nutritious meals and accommodating service. However, other reports point to inconsistent meal delivery, missed or delayed supplements, and at least one instance where prescribed nutrition needs were not met. These contrasting accounts indicate that meal-service reliability may vary by shift or unit.

Activities and the physical environment are frequently cited as strengths. The facility's programming, musical events, lounges and outdoor areas are repeatedly described as contributing to a homelike atmosphere. That said, housekeeping and sanitation practices appear inconsistent across reports: isolated cleanliness issues, odor concerns after carpet cleaning, and pest-control concerns were mentioned. Such items suggest variability in housekeeping oversight and environmental maintenance.

Management and value perceptions are polarized. Some families commend the admissions process, online family access, flexible visiting and low-friction short-term rehab stays. Others describe management explanations for care gaps, dissatisfaction with resolution of concerns, and a perceived mismatch between fees and the quality of long-term care. A few reviewers also questioned the transparency of quality metrics and ratings.

Notable patterns: positive experiences cluster around short-term, rehab-focused stays with active therapy involvement, while negative experiences more often involve long-term or memory-care settings where supervision, housekeeping, and nutrition continuity were flagged. Prospective residents and families should observe staffing levels during tours, ask about fall-monitoring protocols and memory-care supervision, inquire about nutrition and supplement administration policies, review housekeeping and pest-control procedures, request examples of incident-communication practices, and examine state inspection/complaint histories. Taken together, the reviews suggest that Ecumen Pathstone offers strong amenities and successful rehab outcomes for many, but also exhibits operational inconsistencies that families should evaluate carefully when considering long-term placement.

    About Ecumen Pathstone

Ecumen Pathstone is a non-profit senior living community that sits near the Minnesota River bluffs, and you'll notice the views are nice and there's a peaceful feeling with the big old trees and Scandinavian-style buildings. The community offers a wide range of care, including Independent Living, Assisted Living, Memory Care, Long-Term Care, Short-Term Transitional Care, and Respite Care, and they even have Home Care, Adult Day Services, Hospice, and Therapy right on campus, so folks can get help with daily things or more specialized support as their needs change. The care model here focuses on helping seniors stay healthy and as independent as possible, with personal care plans and teams trained for both Assisted Living and Memory Care, and if you have memory issues like Alzheimer's or dementia the place offers specialized services in closely monitored areas with building security and personalized, mind-invigorating activities to keep you engaged and safe.

You'll find 48 apartments and 146 private rooms, and some apartments have walk-in showers, in-unit laundry, private patios or balconies, stainless steel appliances, granite countertops, and everything's pretty much ready for you to move in, with utilities included and recent renovations making things easier to get around for folks who might need a bit more help. The campus is pet-friendly, and there's walking paths and nearby parks if you like going outside, plus lots of shared spaces indoors-like a library, exercise room, and recreation areas-where you can meet with others, read, or join in one of their daily social, educational, or entertaining activities aimed at keeping everyone involved and uplifted. For meals, there's a Café Catering and the Ecumen Store for snacks and essentials, and they handle things like light housekeeping and laundry, so you don't have to worry about chores unless you want to.

Staff is always on call, with 24-hour nursing care and help for everything from medication to dressing, bathing, and getting around, including skilled nursing if you need it. There's structured wellness programs, rehabilitation therapy, and support for those coming in for short stays or recovering from illness, and if you want to stay connected to your faith or get spiritual care they offer Christian care right on site. Security features include controlled building access, concierge staff, and smoke and carbon monoxide detection systems in all the units. There are also amenities like barber and beautician services, and you can invite friends or family to share private gathering spaces. Residents and visitors can take a virtual tour online to see what the apartments and common spaces look like ahead of time, and the community connects people through things like a Pen Pal Program that brings different generations together.

Ecumen Pathstone is proud of its community-centric atmosphere, where the goal is to help people of all backgrounds live full lives with energy and a sense of security, and they do things like memory care research and other care innovations to improve quality of life. Payment options are flexible, including Medicare, Medical Assistance, Veterans Benefits, private insurance, and private pay, so you've got choices. The campus has sister communities nearby, such as Ecumen Sand Prairie and Ecumen Prairie Hill, offering more Assisted Living and Memory Care if you need a different setting. U.S. News named it one of the Best Nursing Homes for 2019-20, and you'll often see residents, families, and team members working together to make sure everybody feels included and taken care of, with both long-term and short-term options available whenever needs change.
