    Caring professional staff, dignified care

I placed my mom here for a long stay and was very satisfied. The staff were caring, professional, and treated her with dignity and gentleness - they felt like family in a down-home setting, cleanliness was acceptable, and even after the ownership/name change the care remained very good; I would have kept her longer.

    Summary of reviews

Overall impression: Reviews reflect a mixed picture in which the facility's therapy services and certain frontline caregivers receive strongly positive comments, while operational, managerial, and safety-related issues generate significant concern. Several families praised the physical and occupational therapy teams and transportation staff and described long-stay experiences in which staff felt like family and resident dignity was prioritized. At the same time, multiple patterns indicate systemic weaknesses that prospective residents and families should evaluate further.

Care and clinical services: The rehabilitation/therapy program is consistently identified as a strengthโ€”therapists are described as competent, caring, and effective in helping residents progress. Direct-care staff (aides and some nurses) are also frequently described as caring and gentle, particularly in long-term placements. However, reviews indicate chronic understaffing and staffing instability that affect nursing coverage and day-to-day assistance. Reported consequences include delays in responding to toileting and feeding needs, aides distracted during care tasks, and instances where residents missed meals. These descriptions suggest gaps in staffing levels, task supervision, and on-shift workflow management that can affect basic activities of daily living.

Staff, teamwork, and conduct: There is a notable contrast between accounts praising compassionate caregivers and accounts describing poor teamwork and professionalism. Several reviews cite nurses arguing in front of residents, staff making disparaging comments, and inconsistent on-shift communication. These items point to problems with team cohesion, supervision, and expectations for professional conduct. While some families report that staff handled residents with dignity, others report concerns about staff tone and behavior that warrant attention to training, supervision, and culture-building.

Dining and activities: Food quality is described inconsistentlyโ€”some reviewers call it acceptable, while others describe inconsistent meal service and poor food quality. Missed meals and unreliable meal-service continuity are specific operational risks noted in reviews. Activity programming is not frequently discussed in the summaries provided; transportation services receive positive mention, which supports access to off-site appointments and social outings when available.

Facilities and environment: Reviews include recurring cleanliness and odor concerns in some areas and reports of missing personal items from rooms. These point to lapses in housekeeping consistency and in personal-property management. While parts of the facility are described as clean and homelike by some families, variability across units or shifts appears to be an issue. The summaries also reference changes in ownership/management and a facility name change, which may be relevant when assessing current conditions.

Management, administration, and safety: Several reviews raise management and administrative concerns, including perceived unprofessionalism, weak discharge planning, and limited communication about transfers and billing. There are multiple reports of attempted unauthorized charges, disputed billing, and questions about how personal property is securedโ€”indicating weaknesses in financial controls, billing transparency, and resident-privacy protections. Reviews also describe a social-worker role that did not provide expected discharge updates or transitional planning, suggesting gaps in care-coordination processes.

Notable patterns and considerations: The collection of summaries shows a polarized experienceโ€”some families are highly satisfied and emphasize compassionate care and effective therapy; others report operational failures that materially affected resident safety, dignity, and financial security. Key patterns to investigate further include staffing levels and scheduling, supervision and culture among nursing personnel, consistency of housekeeping and property safeguards, meal-service reliability, and the facility's current billing and privacy controls. Given the reports of administrative change, prospective residents and families should confirm current ownership, recent corrective actions (if any), and up-to-date inspection or compliance records before making placement decisions.

    About St. James Living Center

St. James Living Center sits in Saint James, Missouri and offers a wide range of care for older adults, including independent living, assisted living, memory care, skilled nursing, hospice care, and even home care, so you can see it covers pretty much everything folks might need as they age. It takes Medicare and Medicaid, so people with different financial situations can use their benefits. The staff provides help with basic daily needs like bathing, dressing, taking medicine, and getting around, and there's a healthcare-related alert system in place for emergencies at any hour, plus a 24-hour call system and regular supervision to help keep everybody safe. There's an on-site chef for meals throughout the day, including menus for allergies and diabetes, and folks can eat restaurant-style or pick all-day dining, while private rooms come furnished and have options for private bathrooms and kitchenettes, along with cable TV, Wi-Fi, phone, and air conditioning, which many folks will find important.

For activities, the center offers things like movie nights, music, arts, games, and fitness, along with a library and a garden outdoors for walking and programs when the weather's good, not to mention transportation to doctor appointments with accessible vans, parking, and regular outings. Residents can use the community kitchen, arts room, game room, or fitness room, and folks who need extra help, like for memory issues or those who can't walk, can get specialized care in their wing, with extra supervision and tailored programs, including medication management and personalized care plans for each person.

When it comes to inspections and staffing, St. James Living Center has had 36 total deficiencies and has reported issues with infection control and care planning, along with a nurse turnover rate of 64.8% and just 3.21 nurse hours per resident per day, and its health inspections rate as average but both quality and staffing are rated much below average, which may be worth considering for families thinking about long-term care here. The management has been run by Ltc Management Services Llc since September 2016 and the place is affiliated with James & Judy Lincoln. There's help with moving in, housekeeping, laundry including dry cleaning, and special transportation and parking services, so residents get a good amount of support for daily life. Specialized care, such as adult day services, home health, and outside services brought in by request, are part of what makes this place flexible in meeting unique needs. The center wants every resident to reach their highest well-being-physically, mentally, and emotionally-with programs led under a doctor's supervision and a focus on comprehensive long-term care.
