    Compassionate, clean, homey skilled care

I'm very pleased with the caring, professional staff and clean, odor-free facility - my grandmother received compassionate nursing, attentive CNAs and excellent physical therapy, and staff responded quickly and kindly to needs. The social, homey atmosphere (porch seating with outdoor views, bird feeding and a friendly Tom cat), TVs for movies, vending nearby and convenient Walmart location made visits comfortable, and the team was accommodating to family needs, including a swift hospice room arrangement.

    Summary of reviews

St. Joseph Manor Health & Rehabilitation presents a mix of strengths and operational weaknesses in the reviews provided. Many descriptions highlight compassionate direct-care staff, including nurses and CNAs, and positive experiences with weekday physical therapy and hospice accommodations. The facility is frequently described as clean and secure, with accessible features such as automatic doors, porch seating with outdoor views, and a social atmosphere that includes on-site entertainment, vending, bird-feeding, and pet presence. Its location near retail amenities is noted as convenient for families.

Care quality and staff interactions are a prominent positive theme. Reviewers commonly characterize nursing and caregiving staff as kind, encouraging, and accommodating; families describe supportive responses during end-of-life situations and appreciation for staff flexibility when arranging hospice or family visits. Rehabilitation services are singled out as a strength, with several favorable remarks about physical therapy. At the same time, there is a recurring operational concern about inconsistent responsiveness: reviewers describe instances where residents were not checked on regularly or access to basic needs (for example, water) was delayed. This suggests variability in monitoring and availability across shifts or times of day.

Dining and housekeeping comments are mixed. Positive mentions of a clean, odor-free environment sit alongside serious sanitation and food-quality concerns in individual accounts. Those accounts point to lapses in meal quality and room-cleaning consistency, indicating variability in routine housekeeping and food-safety practices rather than uniformly poor performance. Prospective families should ask specific questions about kitchen oversight, meal-service protocols, and housekeeping schedules during a tour.

Activities and facilities are generally described favorably. The facility offers communal spaces, porch seating with views for socializing and quiet observation, TVs for movies, and small amenities like vending machines. The presence of animals and bird-feeding contributes to a home-like, social environment. Physical accessibility features are in place, and reviewers note a secure atmosphere.

Management and communication show a mixed pattern: some reviewers praise prompt problem resolution and helpful staff, while others describe uneven communication and follow-up. The contrast suggests that while there are effective processes and staff members who respond well, those practices are not uniformly experienced by all families. For decision-making, it would be prudent to discuss staffing patterns, shift coverage, and communication protocols with facility leadership and to request references or speak with current families when possible.

Overall, St. Joseph Manor appears to offer strong hands-on caregiving and rehabilitation in a pleasant, accessible setting, but prospective residents and families should be attentive to operational consistencyโ€”particularly in resident monitoring, housekeeping, and meal qualityโ€”and verify how the facility addresses those areas during different shifts and service times.

About St. Joseph Manor Health & Rehabilitation

St. Joseph Manor Health & Rehabilitation sits at 1317 North 36th Street in Saint Joseph, Missouri, and is one of those places that's focused on health care and rehabilitation, offering a wide range of services for older folks who need different levels of care, including skilled nursing, memory care, short-term rehabilitation, and long-term care, and you'll find 110 certified beds in the building, usually with about 61 residents staying on an average day, and it's managed by Beverly Miller and affiliated with Reach Ltc. The staff helps residents with daily activities like bathing, eating, and dressing, and there's always someone on hand for nursing and support, 24 hours a day, with 3.90 nurse hours available per resident each day, though there's a nurse turnover rate of 50.0% noted. Folks staying here get care in private rooms that come with beds, nightstands, overbed tables, windows, and TVs, and you'll notice the reception area looks cheerful and open, while the main entrance shows some nice landscaping, benches, a covered walkway, and an American flag.

Skilled nurses, an award-winning Wound Care team, and different therapy professionals work together for treatment and rehabilitation programs that help people regain strength and independence, and the care's adjusted based on what each resident actually needs, whether that's short stays after hospital visits, long-term nursing, or adult day care, and some residents need more help with things like memory care or subacute rehabilitation, so there are specialized units set up for that. There are regular activities like bingo, crafts, church services, and chances to get out for community events, and they've even got Miss Ruby, a therapy dog, helping with the emotional side of care, and people who want quiet time can take part in services for meditation or worship. The staff aims to show respect and consider everyone as they go about their work, and you'll spot transportation services for medical appointments, lounges for relaxing, and safety features like emergency call systems throughout the building.

They do keep track of residents' health and provide medication management, as well as therapy options like physical, occupational, and speech therapy, but according to recent inspection reports-including a standard inspection on March 21, 2025-there have been 57 total deficiencies across several reports, with points that need attention in areas like proper infection control, accurate resident assessment and care planning, and dietary staffing, including five infection-related deficiencies, so folks who like to know about these things may want to look up reports for the latest information. The place tries to keep a safe and welcoming environment, with services for both medical and nonmedical daily needs, and offers support for those who are very frail or dependent on care, and its staff works to help residents meet goals for health, confidence, and feeling part of the community.
