    Summary of reviews

The Mulberry at Waverly elicits strongly divided impressions. Many reviewers describe a warm, family-like culture in which staff treat residents as individuals, leadership is visible and responsive, and social programming is robust. Positive comments emphasize attentive nursing leadership, an organized admissions experience, active holiday and community engagement, tidy rooms, pleasant outdoor spaces, and staff who remember and relate to residents. Several families specifically praised nurse leadership and front-line caregivers for listening and resolving concerns.

Conversely, a number of reviews raise substantive operational and clinical concerns. These center on inconsistent care quality between units or shifts, with specific issues implying weaknesses in medication administration, delayed attention to hygiene needs, and episodes of weight loss or nutrition-related decline. Reviewers also described delays in diagnostic testing and limited follow-through on abnormal results, as well as variable rehabilitation outcomes where therapy did not produce expected progress. Together these point to gaps in clinical escalation, therapy oversight, and day-to-day care continuity.

Communication and administrative processes are another recurring theme. While some families report strong, solution-oriented communication from leadership, others describe abrupt phone interactions, difficulty obtaining medical records, and unclear or inconsistently enforced visiting policies. There are also allegations of record-keeping inaccuracies and concerns about missing personal items with unclear reimbursement procedures. Reports of staffing qualification questions and payroll/labor disputes suggest potential impacts on staff morale and operational stability.

Facility and service-level observations are mixed. Many reviewers note clean, well-kept rooms, reasonable cost, and a homelike atmosphere; others point to sanitation inconsistencies and occasional professionalism issues among staff. Activities programming and community partnerships are commonly highlighted as strengths that enhance resident engagement. Prospective residents and families should weigh the polarized feedback: the facility can provide compassionate, engaged care under attentive leadership, but there are documented operational risks related to clinical consistency, communication, record management, and staff stability.

Recommendations for families considering Mulberry at Waverly include: observe multiple shifts during a tour to assess consistency of care; ask for specifics on medication-safety protocols, staffing ratios, and therapy outcome metrics; request the facility's visiting policy in writing and verify current enforcement; review procedures for personal belongings and reimbursement; and confirm processes for medical-record access and clinical escalation. Direct conversations with current families, and verification of staff credentials and any outstanding labor or regulatory actions, can help clarify which aspects of the facility experience are most likely to affect a prospective resident.

    About The Mulberry at Waverly

The Mulberry at Waverly sits as a single-story building with a classic brick and siding look, a well-kept lawn, and an American flag out front, making it easy to recognize from the street, and has room for 54 residents with an average of about 44 living there each day, with care focused on nursing home services that follow federal standards, though detailed information about who manages or oversees the place isn't available. The Mulberry at Waverly offers skilled nursing care along with help for daily living, memory care, independent and assisted living, short-term and respite stays, plus services like case management, scheduled activities, local transportation, and a beautician on site, and tries to keep things social and aimed at resident wellness, with the grounds and programs meant to make residents feel welcome and cared for.

There's a range of senior care services provided, including hospice support, home health, and even home care, and the staff is supposed to deliver personal care through individualized plans, with social services and support and referral help for families and residents. There are quality efforts focused on both daily needs and encouragement for social and recreational activity, so people have chances to mix, move, and enjoy programs, while support for health and comfort is a main focus, and there's an ongoing push for compassion and wellness.

The nursing staff provides about 3.49 hours per resident each day, which is less than the Nebraska state average of 4 hours, and there's an adjusted total nurse staffing rate of 3.15 hours, with RN staffing at about 0.49 hours per resident each day, and a similar figure for weekend coverage. Nurse turnover is high at 87.5%, compared to the state average of 51.6%, and there's a posted deficiency for not keeping daily records of nurse staffing as required, plus documented problems with nutrition and food safety, where inspections found that food didn't always come from approved sources or get handled by the right food standards.

Inspections have found 38 deficiencies, covering problems with some physician and nursing services, and 6 infection-related citations, reflecting issues with infection control and health safety. The facility has care ratings from federal inspectors: a 1-star staffing and health inspection rating, 2 stars for quality measures, and a 107.0 weighted health survey score, as recorded under CMS Certification Number 285143. The Mulberry at Waverly is a for-profit operation focused on operations that fit senior living communities, but full details about how the facility is managed or run day-by-day aren't available in any public report.
