    Outstanding care and excellent dining

I had a very positive rehab stay - the staff were professional, attentive and genuinely caring, therapists were outstanding, and the facility is beautiful and impeccably clean. Dining was excellent with well-presented, tasty meals in a pleasant dining room. I felt heard and well cared for and would return for rehab.

    Summary of reviews

Advanced Health Care of Henderson is described primarily as a rehabilitation-focused, short-stay skilled nursing facility with a modern, hotel-like environment and a wide array of on-site amenities. Many families and patients praise the facility’s physical plant — clean, recently built spaces, private suite-style rooms with kitchenettes and large televisions, an open dining room, and amenities such as a gym, barber/beauty shop, library, and small theater. Social programming (ice cream socials, bingo, events) and meal presentation receive frequent positive mention, and numerous observers characterize the facility as conducive to recovery and comfort.

Clinical and caregiving strengths are well documented in the reviews. Reviewers consistently highlight compassionate and attentive RNs, CNAs, and therapy staff; physical and occupational therapy teams receive repeated praise for effectiveness, patience, and individualized progression. Several case managers and administrators are singled out for responsive communication and coordination, and drivers/transport staff are often described as helpful for appointments and discharge logistics. Overall, the facility’s short-term, rehab-oriented model and robust therapy resources attract positive feedback from patients aiming to return home.

At the same time, the reviews reveal recurring operational concerns that prospective residents and families should evaluate. A common theme is variability in responsiveness to call lights and time-sensitive needs; these delays are linked in some accounts to discomfort and missed timely assistance. Medication-management lapses and inconsistent timing of medications are described, as are instances of missed or delayed wound and ostomy care — indicating potential gaps in clinical follow-through and documentation. Several families raised concerns about discharge planning and care transitions, including premature or poorly coordinated discharges and unclear handoffs to home health; a small number of accounts referenced regulatory-level complaints related to discharge practices.

Dining and dietary services are generally praised for quality and variety, but reviewers also describe inconsistency in meal delivery, menu-format confusion, and occasional shortfalls in dietary accommodations such as insufficient fiber or fruit options. Sanitation and incontinence-care processes are another area of variability: while many note an impeccably clean environment, others describe episodes of inconsistent hygiene assistance and cleanliness practices. Transport and appointment coordination received mixed comments — drivers are appreciated, yet scheduling and pick-up coordination have been problematic for some families.

Management and leadership perceptions are mixed. Multiple reviewers commend accessible administrators and supportive case managers who facilitate good family communication and placement transitions. Conversely, other accounts describe gaps in higher-level accountability, inconsistent responses to clinical concerns, and perceived pressure related to private-pay financial considerations. Staff conduct and communication style are also variable — several reviewers praise specific employees by name for professionalism and empathy, while others raise concerns about tone and bedside communication from some staff members.

In sum, Advanced Health Care of Henderson appears to offer a strong rehabilitation environment with many institutional strengths: well-regarded therapy teams, a modern and clean facility, engaging activities, and numerous staff who deliver personalized, compassionate care. However, the facility shows operational variability in responsiveness, medication and wound-care continuity, discharge planning, and some aspects of management communication. Prospective residents and families may benefit from confirming specific operational safeguards during admission — for example, plans for medication oversight, wound-care protocols, expected call-response times, dietary accommodations, and a clear, documented discharge and home-health plan — and identifying the on-site staff members who will be responsible for those areas during a stay.

About Advanced Health Care of Henderson

Advanced Health Care of Henderson provides short-term nursing and rehabilitation care for people coming out of the hospital and needing a place to recover before going home, and it also helps those who need skilled nursing or intermediate care because they're frail or need extra help. The facility has 38 certified beds and usually cares for about 37 residents each day, which shows it's almost always at capacity, and you'll find private suites with their own bathrooms and showers, adjustable beds, pressure-relieving mattresses, private phones, televisions, and high-speed internet so residents have privacy and comfort. There's round-the-clock nursing care with licensed nurses on site seven days a week, and the physician's always on call, which adds peace of mind for families. Advanced Health Care of Henderson's got a strong team, with nurses, therapists, social workers, and home health aides who all work together to cover everything from medication management and wound care to help with bathing and daily tasks, plus there are specialized therapy programs for rehabilitation, with physical, occupational, and speech therapists on hand for conditions like stroke or injury, and the staff provide personalized treatment plans to make sure every resident gets exactly what they need to regain their independence.

The facility's linked with Advanced Health Care and is affiliated with the Larry H Miller Corporation and a number of trusts and entities, and it operates as a for-profit corporation. They get Medicare certification and offer both inpatient and outpatient rehabilitation services for people who need therapy at different stages of recovery, and you'll find home health aides providing care under the supervision of nurses and therapists. The nurse turnover rate sits at 42.9%, which is a bit better than the state average, and residents get 5.77 nurse hours per day, which is higher than most places in the state, so residents get a good amount of attention. The place has an outdoor area, a library with a saltwater fish tank, a salon and barber shop, a roomy private dining area with a restaurant-style meal service supervised by a dietitian equipped for special diets like for diabetes or heart problems, and a cozy fireplace and patio where people can relax or visit with family in a homey setting.

The inspection records show 15 different deficiencies, including trouble with making sure the food comes from approved sources and is handled properly, which was marked as a G-level seriousness and affected a few residents with actual harm but not immediate danger, so that's worth noting. Inspectors cited them for a quality of life and care deficiency because of failure to follow treatment and care orders as well as a violation of infection control standards, with one infection-related deficiency on file. Most other issues were rated as D or lower, meaning no actual harm occurred but there was potential for more than minimal harm. Even with these issues, the facility keeps its focus on patient comfort and safety, offering amenities like a beauty salon, an attractive private dining room, a reading area, and comfortable seating for visitors. Therapists, nurses, and social workers are part of the team, helping with everything from post-surgery care and rehabilitation to connecting families with community resources and support. They aim to help every resident reach the best recovery possible and make the place feel welcoming and supportive during what can be a hard time.
