    Clean facility, caring staff, recommended

I'm very pleased with this clean, updated facility - the staff are warm, attentive and genuinely caring. Nursing and rehab teams helped my mom make real progress, and activities, meals and daily walks keep residents engaged. Management and front desk were welcoming and responsive; I'd recommend it for rehab or long-term care.

    Summary of reviews

Overall impression: Reviews present a polarized picture of Greene Meadows Nursing and Rehabilitation Center. Many families and residents praise the facility’s rehabilitative strengths and individual staff members, while a significant number of accounts raise concerns about basic operations, staffing, and environmental care. The result is a mixed profile: strong therapy and some highly regarded caregivers coexist with operational inconsistencies that families should evaluate carefully during a tour and intake.

Care quality and clinical oversight: The facility’s rehabilitation services (physical, occupational, and speech therapy) receive consistently favorable feedback; therapists are described as knowledgeable, engaging, and effective at supporting functional gains. At the same time, there are recurring concerns about medication administration, delayed clinical follow-up, untreated skin integrity issues, and missed or delayed assistance for residents with toileting needs. These patterns point to variability in clinical reliability across shifts and units rather than uniformly poor or uniformly excellent care.

Staffing and staff conduct: Many reviewers single out individual nurses, CNAs, and therapists for compassionate, respectful care and advocacy. Those positives are offset by repeated reports of understaffing, staff burnout, high turnover, and occasional lapses in professionalism or communication. Front-desk and supervisory interactions are described as helpful in some accounts and unprofessional in others, suggesting uneven training and management oversight. Prospective families should ask about staff-to-resident ratios, turnover rates, and weekend/night coverage during placement discussions.

Dining and activities: Dining service quality appears inconsistent. Common themes include cold or reheated meals, timing and portion issues, and occasional lapses in adherence to dietary restrictions. Conversely, the facility offers rehabilitative and social activities that many residents enjoy — therapy-driven group sessions, arts/craft options, and some social events — though several reviewers requested more varied and consistently run recreational programming.

Facility environment and infection control: Portions of the facility are described as clean and modern with comfortable rooms and pleasant common areas; however, other accounts note sanitation lapses, odor concerns, exterior clutter, and pest-control issues in specific units. Infection-control practices (including COVID screening and handling of gastrointestinal illness) are described as variable. Families should evaluate cleanliness, odors, and pest-control measures in person and inquire about recent environmental or regulatory findings.

Management, communications, and billing: Reviewers describe a recent change in ownership and report mixed experiences with management responsiveness. Concerns include inconsistent family communication (phone responsiveness, explanation of medical conditions), surprise billing or insurance confusion, and occasional references to regulatory attention or citations. These suggest uneven administrative processes; prospective residents should request copies of recent inspection reports, readmission and incident data, and clear written explanations of financial responsibilities.

Notable patterns and guidance: The strongest, consistent strength is the therapy/rehab program and several individual staff members whose efforts families repeatedly commend. Recurring operational weaknesses include inconsistent staffing, variable housekeeping and incontinence care, meal-service quality control, and occasional lapses in clinical follow-through. For families considering this facility, recommended next steps include an in-person visit focused on cleanliness and odors, a conversation with the therapy team, review of staffing schedules and medication-management protocols, and inspection of recent regulatory or inspection documentation. Doing so will help determine whether the facility’s rehabilitative strengths align with the level of clinical reliability and environmental quality your loved one requires.

About Greene Meadows Nursing and Rehabilitation Center

Greene Meadows Nursing And Rehabilitation Center sits in Jefferson Heights, NY, and has served the Catskill community since 1975, with a focus on careful, personalized care so everyone who stays there gets treated as an individual, and folks tend to notice the way the building has character and history, since it traces its roots back to the late 1800s with Genesee County, and you can see that charm in the design of the spaces-they're both comfortable and bright, with an effort to make residents feel at home, which is saying something when you've got up to 120 beds spread through fully furnished rooms, most with private bathrooms, kitchenettes, air conditioning, cable TV, and Wi-Fi, so each room feels like a place you might want to settle in, and staff go out of their way to create care plans that fit each resident's health needs, with a full range of skilled nursing overseen by nurses and therapists who are there 24/7, making sure help is always nearby whether you need support with daily activities, medication, or something more serious.

Greene Meadows offers long-term nursing care, short-term rehabilitation, and post-acute rehab, and there's also specialized help for anyone recovering from surgery, dealing with heart problems, orthopedic needs, or in need of hospice support, plus memory care for folks dealing with dementia or Alzheimer's, which involves safe environments and a lot of gentle, steady attention, and the therapy part is steady-physical, occupational, and speech therapy, plus nutritional and pain management, because sometimes getting back on your feet means small steps with a lot of help, and they've got a wellness center, fitness room, dining spaces, game room, computer center, and even a little library, so you're not stuck just watching TV, and yes, there are gardens and outdoor areas if you like a bit of fresh air now and then.

Dining is restaurant-style, and they'll prepare meals for all sorts of special diets, including if you're managing diabetes or just have a preference, and there's housekeeping and laundry so you're not worrying over chores, but if you like mixing with folks, they've set up daily activities, clubs, trips, social programs, and even resident-led events, plus transportation for errands or outings, and it's all aimed at keeping life active-but not rushed-while you're there.

For people needing a short stay, maybe while a caregiver gets a break or while recovering from something, respite care is available, so there's always a spot for temporary needs, and people mention the staff really trying to support everyone thoughtfully, with respect for each resident's life and wishes, and while the place isn't brand new, it's clean, well kept, and feels like a regular community with experienced hands on duty at all times, so most families find peace of mind with that arrangement, and reviews rate the place about a 3.2 out of 5, which says there's always room for improvement, but folks tend to like the wide range of services and the calm, steady feeling throughout the building-a good fit for many looking for thorough nursing and rehab care without a lot of fuss.
