    Professional, caring staff; mom improving

I'm very satisfied with the care my mom received. The staff were caring, professional and communicative; medical attention, timely medication updates and the excellent physical therapy helped her improve and she's expected to go home soon. The facility is clean, welcoming (lovely family room and aquarium), with good activities and a reassuring, recovery-focused atmosphere.

    Summary of reviews

Reviewer impressions of Meadow Park Rehab & Healthcare are strongly polarized. A large subset of reviewers highlight the facility's rehabilitative strengths: physical and occupational therapy teams, wound-care clinicians, and rehab-driven care plans are frequently credited with measurable functional gains and successful discharges home. Many families also describe compassionate, patient-centered caregiving from CNAs and nursing staff, a welcoming reception and family room, engaging activities (music, games, pet therapy) and a sense of community among residents and employees.

At the same time, a substantial number of reviews identify operational and safety weaknesses. Understaffing and inconsistent staffing patterns are a recurrent theme and are linked to delays in routine care, inconsistent medication timing and missed clinical communications to families. Several reviews describe problems with discharge coordination, transport and billing logistics, and a lack of transparency around discharge paperwork and medication instructions. There are repeated concerns about how the facility manages personal property and belongings, including missing or mishandled items.

Dining and clinical-support services show mixed performance. While some reviewers note improved and regular meal offerings and residents looking forward to meals, others describe inconsistent meal delivery, late evening medications, and inappropriate diabetic-diet selections. Infection-control and outbreak management is another important pattern: reviewers cite an outbreak among staff and patients and uneven implementation of containment measures. Safety and maintenance issues are also raised, including missing or malfunctioning safety equipment (bed rails, room rails), elevator reliability problems, and crowded multi-occupancy rooms with shared bathrooms that can complicate privacy and infection control.

Facility physical condition and housekeeping elicit divergent comments. Several visitors and families praise clean, well-kept communal spaces, an attractive reception area and well-equipped patient rooms. Conversely, other reviewers describe sanitation concerns, odor concerns in some areas, infrequent housekeeping in particular rooms, and an overall need for building updates in parts of the campus.

Management and communication practices are another area of variation. Some reviews describe supportive, responsive administrators and helpful front-desk staff; many others point to poor phone and electronic-communication reliability, slow responses to family inquiries, inconsistent follow-through on complaints, and perceived gaps in staff training and accountability. A small number of reviews reference serious individual clinical events and concerns following a resident's deterioration or death; these comments amplify the themes above (notification delays, emergency response variability, discharge-process gaps) rather than describing a consistent clinical pattern across the facility.

In summary, Meadow Park appears to provide high-quality rehabilitative services and has staff who deliver compassionate, effective care in many cases. Prospective residents and families should weigh those strengths against recurring operational concerns—particularly staffing consistency, medication and discharge processes, housekeeping/sanitation in some areas, multi-occupancy rooming, and communication reliability—when evaluating placement. A focused tour and direct questions about staffing ratios, medication-administration protocols, infection-control practices, property-loss policies, and discharge coordination can help clarify whether the facility's strengths align with an individual resident's needs.

About Meadow Park Rehab & Healthcare

Meadow Park Rehab & Healthcare gives many choices for seniors, offering assisted living, memory care, independent living, and skilled nursing all in one place, so you'll find help with daily tasks like bathing, dressing, transfers, and taking your medicine, and there's always someone on-site or just a call away, with nurses there 12-16 hours a day and a call system for 24-hour supervision. The community's got strong rehab services too, with short- and long-term stays that focus on recovery after illness, surgery, or hospital stays, and with a full team of board-certified doctors, skilled nurses, and therapists-there's physical therapy, occupational therapy, speech therapy, respiratory therapy, plus physiatry, prosthetic and orthotic help, and even language pathology, and their gym has modern equipment that helps people work toward getting back home and living as independently as possible. You'll find memory care units with 24-hour support, specialized programs for memory improvement, and a team that works together to offer programs and activities, plus behavioral rehabilitation for adults and mental wellness programs. Meals are a big part of life here, with restaurant-style dining, special menus for different needs like diabetic or kosher diets, and a certified Glatt Kosher kitchen that makes food to fit every resident, and you'll also see social events from ice-cream socials to religious services. Rooms come fully furnished with private bathrooms, air conditioning, cable TV, Wi-Fi, and kitchenettes, which is handy, and there's housekeeping, linen services, and even laundry for everyday ease. In terms of spaces to enjoy, there's a large dining room, arts room, fitness and wellness center, gaming and movie room, small library, spa, garden, and outdoor patio, and both resident-run and staff-planned activities fill the calendar, with outings, music programs, exercise, community events, and chances for everyone to join in. For getting around, the place offers its own transportation options so you can get to appointments and do errands in the area, and since it's located near the Division of Vascular/Interventional Radiology at Long Island Jewish Medical Center and the Queens Crossing Pharmacy, people often find visits convenient. The staff speaks eight languages and is available around the clock for clinical needs, and you'll also notice internal and external case management, recreation and social service departments, home safety evaluations, and both independent living and respite care so caregivers can rest. Meadow Park is for-profit and privately owned, accepts Medicare and Medicaid, and has a family and resident council, so everyone gets a chance to have their say, and the community is part of a larger Continuing Care Retirement Community, so folks can move between levels of care as their needs change. The place also has a 4-star rating from Medicare and provides care from English-speaking caregivers, along with certified primary care physicians and consulting specialists on site, and puts a big emphasis on patient comfort, self-care, and education every step of the way.
