    Attentive compassionate staff and care

I placed my loved one at Maple City and I'm very pleased. The nurses, CNAs and therapy/rehab team were attentive and compassionate, providing excellent daily care and end-of-life comfort when needed. Communication was clear, admissions and sign-in were smooth, and the front desk and staff-especially Jen and Amanda-were extremely helpful and facilitated visits during COVID. I would recommend Maple City for warm, genuine, well-staffed care.

    Summary of reviews

Reviews of Maple City Rehabilitation and Nursing Center present a mixed picture. Many family members and visitors praise the direct-care staff, noting compassionate bedside care, attentive CNAs, and a strong rehabilitation team that delivers effective therapy and supports recovery goals. Several accounts describe warm receptions, helpful front-desk processes, and streamlined admissions, and there are positive mentions of supportive end-of-life care and clear communication when clinical updates are needed.

At the same time, there are recurrent operational concerns that appear across multiple reports. Understaffing and frequent reliance on agency nurses are commonly cited, which reviewers link to delays in medication administration, slower response to resident needs, and uneven supervision—particularly for residents with cognitive impairment. Reviewers also raise sanitation and linen-management deficiencies and odor concerns in some areas, along with signs of deferred facility maintenance such as drafty windows and damaged fixtures. Several comments indicate inconsistent infection-control practices and disorganization in daily workflows.

Management and administrative practices draw both praise and criticism. Some reviewers specifically note improvements under new management and effective communication from certain staff, while others describe difficult interactions with social-work or HR personnel and slow or confusing discharge and transfer processes. These contrasting accounts suggest variability in administrative responsiveness and in the consistency of staff conduct.

Dining and activities receive relatively few direct comments in the set of summaries provided; the strongest nonclinical theme centers on rehabilitation and therapy services, which are frequently highlighted as a strength. Prospective residents and their families should plan an in-person tour that explicitly evaluates housekeeping procedures, odor control, and the physical condition of rooms and common areas; ask for current staffing ratios, how the facility manages agency nurses, and policies for medication administration and infection control; and request recent inspection or quality reports. These steps will help clarify whether the facility’s clinical strengths are adequately supported by stable operational practices and building upkeep.

About Maple City Rehabilitation and Nursing Center

Maple City Rehabilitation and Nursing Center, once called Hornell Gardens, sits at 434 Monroe Avenue in Hornell, New York, and has been managed by Bridgett Chasko since September 2014. The center provides 114 licensed and certified beds, serving people who need skilled nursing or rehab services, and it belongs to the Hurlbut Care group under the ownership of Robert Hurlbut. Folks staying here might notice some special terms and features-like their Guest Services team, always available day or night, to help guests settle in, handle requests, or even assist with gift shopping, and there's the wander guard platform that offers safety with wander management and staff emergency alerts, aiming to keep people safe who might be at risk of wandering. The nursing staff offers about three hours of care per resident each day, which is below the state's 3.7-hour average, and staff turnover runs high, around 58.1%. The facility's received 25 deficiencies on recent inspections, with notes about areas like care planning, responding to residents' needs, and following care orders.

The center uses its own electronic medical records system so staff can get to resident information quickly, helping coordinate care for people who may need special attention, whether it's for dementia, pain management, wound care, or stroke recovery, plus there are dental, eye, and podiatry services, pharmacy support, and various therapies including physical, occupational, and speech therapy. Maple City takes care to accommodate people in private or shared rooms, with amenities like full-time doctor availability, personalized menus, beauty and barber services, free cable and phone, plus daily newspapers and magazines, which can help folks feel more at home. The center has plenty of specialized clinical services-from orthopedic rehab, joint replacement recovery, amputee support, to IV therapy, oxygen needs, and comfort or palliative care.

Therapy services run the gamut, including mobility help and recreational programs, and there are staff and resources to provide regular physician care and laboratory testing. For those who enjoy activities, there's therapeutic recreation, and safety gets a boost thanks to the wireless nurse call system and alarm escalation. While Maple City's goals focus on comfort and well-being with high hospitality standards, past citations for gaps in responding to resident preferences and planning show there's still room for improvement. The address and basic information remain clear, and for people looking for care in this part of New York, Maple City offers a full range of nursing and rehab services with unique features and support systems.
