    Outstanding nursing, rehab, caring community

I had a very positive experience here. The nursing and rehab teams were outstanding and helped my loved one recover quickly; administration was hands-on and responsive. Rooms were clean and quiet, meals were good, and daily prayer/Shabbat services plus lively activities made it feel like a caring, supportive home.

    Summary of reviews

The reviews indicate a facility with a clear strength in rehabilitation services but notable variability across other operational areas. Many families and former patients emphasize excellent physical and occupational therapy, personalized, goal-driven rehab plans, and therapy teams that help residents regain function quickly. Rehabilitation leadership and some therapists are described as conscientious and effective, and multiple accounts highlight timely discharges when goals are met.

Nursing and direct-care staff observations are mixed. Numerous reviewers praised individual aides and nurses as compassionate and attentive; however, recurring themes include inconsistent staffing levels, a busy or chaotic nurses’ station, and concerns about the tone and conduct of some staff. Several accounts describe delays in RN oversight and in responding to call signals, which contributors framed as gaps in clinical responsiveness and continuity of care. Reports also suggest variability in staff training and clinical competency across shifts.

Dining and dietary services show inconsistent performance. Some guests reported meals arriving on time and tasting acceptable, while others described cold meals, poor presentation, and failures to honor special diets. The pattern suggests uneven meal-service processes and unreliable dietary accommodation rather than a uniformly managed dining program.

Activity and social programming is a relative strength. Recreation staff receive frequent positive comments for offering diverse activities, crafts, religious services, and social events that create a positive atmosphere for many residents. Families noted accessible communal spaces and opportunities for family involvement during visits.

Facility condition is described as uneven. Several rooms and wings have been renovated and are presented as clean and refreshed, but other parts of the building are characterized as older, crowded, or in need of updating. Multiple reviewers raised sanitation and cleanliness concerns in common areas and some units, and ongoing renovations produce a mixed environment when touring the facility.

Management and administrative interactions are likewise variable. Admissions staff and some administrators are described as helpful and communicative, and some reviewers praised proactive administrative support with financial guidance and discharge planning. Conversely, other reviewers described dismissive intake interactions, inconsistent follow-through, and insufficient availability of casework support. A few reviews raised serious concerns, including allegations of missing belongings and concerns following a resident’s death; these reports point to gaps in asset-security controls and in family communication during critical clinical incidents.

Overall pattern and practical guidance: Northern Metropolitan appears to offer high-quality, intensive rehabilitation and strong recreational programming, backed by individual staff members who are compassionate and effective. At the same time, there is meaningful variability in staffing consistency, cleanliness in certain areas, dietary reliability, nursing oversight, and administrative responsiveness. Prospective residents and families should consider an in-person visit focused on current staffing ratios, nursing coverage patterns, cleanliness inspections, dietary accommodation procedures, security for resident belongings, and protocols for family communication during incidents. Asking for recent infection-control audits, staff-training schedules, and examples of recent discharge timelines can help assess the current operational consistency at the time of placement.

    About Northern Metropolitan

Northern Metropolitan Residential Health Care Facility sits at 225 Maple Ave in Monsey, NY, and has earned recognition as a public charity with the EIN 13-3070759, so you'll find it's focused on providing skilled nursing home services, adult daycare, and extra nutrition programs for older folks, but what strikes you is that they've built up several levels of care, offering Assisted Living, Memory Care, Independent Living, in-home care, and a Residential Care Home option all rolled together, which means people can move in for different reasons-maybe they need serious help or maybe they're living on their own but want some help nearby, and if memory issues crop up, they offer special programs for that too, including more hands-on caregivers than you'd expect, with nurses, therapists, and clinical specialists trained for just about any condition you can think of, from heart issues and dementia to orthopedic care, HIV/AIDS, COVID-19, stroke, or wound care, even ventilator support. Folks come in for short-term rehab or long-term stays, and there's something called RehabStrong™ that aims to get them back on their feet, with expert therapists who put together personal rehab plans and use the latest rehab tech so it's very results-focused, which makes recovery more about what actually works-plus, they've got five-star accommodations, free bedside phones, free TV, and complimentary WiFi for every room, so comfort's part of the deal. You'll notice the building has air conditioning, wi-fi, wheelchair access, parking, and outdoor seating, so visitors and residents can get around easily. There's a real focus on keeping everyone safe with strong COVID-19 protocols, from health screenings and visitor testing (including rapid and PCR tests) to vaccine encouragement, social distancing, and extra support for folks in end-of-life or emergency situations-it's all about following the rules from CMS and NYSDOH to the letter, and making sure visits happen safely, especially for things like urgent care and compassionate needs. Adults who aren't full-time residents can join the Adult Day Care and get access to meals, friendship, and activities, which keeps people engaged and helps form a community where neighbors, parents, and grandparents feel included. The staff uses electronic medical record systems and the latest charting tools so everyone stays on the same page-from lab staff to urgent care people to the nurses who work around the clock. The goal, from what you'll see, is to offer a safe, caring place where people get the help they need, whether that's a quiet spot for independent living, regular activities to keep busy, or specialized medical care for those tougher days, all while focusing on the residents' dignity and comfort. The place keeps business hours most days from early morning to late evening on weekdays and a bit shorter on weekends, making it easy for families or visitors to plan a trip, and while they've published prices for transparency, you won't find specific rates online just yet, but you can get more info through a free consumer service that checks reviews and details.
