    Compassionate attentive short-term rehab care

My mom had a short-term stay at Cedar Manor and I was very pleased. Staff from the front desk to nurses and aides were consistently kind, attentive and communicative - Eva, Judi and Jean were especially helpful with admissions and insurance. Rehab (PT/OT) was outstanding, medications and infection control were handled professionally, and the rooms, grounds and river/garden views were clean and welcoming. Activities were engaging and staff treated residents like family; I felt supported throughout and would recommend Cedar Manor for compassionate, effective care.

    Summary of reviews

Cedar Manor presents a mixed but instructive profile for prospective residents and families. The facility's rehabilitation services (PT/OT/speech) are a clear strength: therapy teams are frequently cited as effective, motivating, and responsible for measurable functional gains that enable discharge home. Many families highlight individualized therapy plans, attentive therapists, and well-equipped therapy spaces. The recreation program is also a positive feature, with varied, engaging activities and holiday programming that contribute to resident well-being.

Care delivery and staffing produce divergent experiences. Numerous reviews praise individual caregivers, nurses, and aides by name for compassionate, hands-on support and reliable medication administration. At the same time, a substantial number of accounts describe operational weaknesses: chronic understaffing, high turnover, and inconsistent nurse responsiveness that manifest as long call-bell delays, irregular toileting and bathing schedules, and concerns about supervision during off-hours. These patterns suggest variability in day-to-day nursing coverage and response reliability rather than uniformly poor clinical practice.

Medication management and clinical communication are additional areas of concern. Several reviewers described delays in medication availability or administration, inaccurate discharge orders, and infrequent or late physician rounds. Families also note inconsistent communication from administrative and clinical leadership about changes in condition, test results, or care plans. Conversely, social work and community-relations staff receive repeated positive mentions for effective insurance navigation, discharge coordination, and family liaison work, indicating pockets of strong administrative support amid broader communication gaps.

The facility environment is generally regarded as clean, renovated in communal areas, and attractively sited with gardens and river views. Therapy rooms and common spaces are well-maintained; however, resident rooms are often characterized as small with limited privacy and in need of occasional maintenance or updating. Dining service is typically timely and nutritious, though some reviews point to variability in meal temperature, menu variety, and vegetable offerings.

Notable safety and conduct themes require attention. Multiple reviewers raised sanitation and room-maintenance concerns as well as isolated allegations of staff misconduct, including alleged theft; these are serious matters that families should explore directly with facility leadership and survey records. Infection-control practices, by contrast, are frequently praised, with reviewers noting active COVID-19 precautions and monitoring.

Overall, Cedar Manor appears to offer strong rehabilitation outcomes, engaged therapy and recreation teams, and several dedicated staff members who provide high-quality individualized care. At the same time, there are recurring operational issues—principally staffing consistency, medication processes, and communication—that can materially affect resident experience. Families considering the facility should prioritize conversations about current staffing levels, medication protocols, toileting and bathing schedules, room accommodations, and mechanisms for clinical updates and incident reporting. Scheduling multiple visits at different times (including evenings/weekends) and asking for named staff contacts can help clarify how the facility's strengths and weaknesses align with a prospective resident's needs.

About Cedar Manor Nursing & Rehabilitation Center

Cedar Manor Nursing & Rehabilitation Center sits at 32 Cedar Lane in Ossining, NY, and operates as a for-profit facility with 153 certified beds, usually caring for around 145 residents a day, and it's run by ownership groups Cedar Manor Holding I LLC and Zbl Cedar Manor LLC, each holding half ownership, and its indirect owners include Joel Zupnick, Zipporah Farkas, Herbert Jozefovic, Joel Leifer, and Joseph Schlanger, all of whom hold either a 17% or 33% stake, though there's no public information about who manages the place or supervises staff on-site. This center offers both short-term rehabilitation and long-term care, and it serves people with dementia, Alzheimer's disease, and those needing complex medical services, like pain or wound management, oral chemotherapy, hemodialysis, peritoneal dialysis, total parenteral nutrition, and even hospice care, and it provides medication, hydration, glucose testing, intensive rehabilitation like physical, occupational, and speech therapy, and IV therapy, as well as respite services for those who just need a short stay to give their regular caregivers a break, and it has outdoor landscaped gardens and an enclosed atrium for residents to spend time in, which is something people like for peace and quiet, so the setting feels calm, and the center tries to provide professional care in what they say is a warm, nurturing place, with admissions open 24 hours every day.

The director is Silvy E. Mathew, M.D., and Angela Taylor is the main contact at the facility. Cedar Manor's staffing gives 3.11 nurse hours per resident per day, which is below the New York state average of 3.7, although its nurse turnover rate is 37.3%, which is a bit better than the state average of 41.2%. The center's certificate is marked as WAIVER, and it's listed under facility ID H042-0000, with a CLIA number of 33D2312056, and its current certification lasts until October 7, 2026.

Inspectors have found a total of 21 deficiencies at Cedar Manor across various visits, and the most recent complaints include a February 25, 2024 complaint with three deficiencies, one for infection control, while another complaint filed June 12, 2024 reported two more deficiencies. Recent inspection reports also show negative findings about how the facility protects residents from abuse, neglect, and exploitation, though these said there was potential for more than minimal harm but no actual harm happened, and the facility was cited for not always reporting suspected abuse or neglect as required by law. There's a record of an infection-related deficiency that means the facility didn't always meet federal rules to stop infections from spreading in the building. Cedar Manor offers skilled nursing and rehabilitation care in an elegant setting, and they have a range of healthcare services focused on promoting recovery and comfort, but their record shows there have been citations and resident complaints that families might want to review when considering options.
