    Welcoming dependable staff excellent rehab

I've been impressed by the facility upgrades and steady growth - the dedicated, welcoming staff genuinely care and work hard to improve residents' quality of life. Their rehab team is outstanding (convenient to Atrium/Novant), wound care and certified nurses deliver excellent results, and clinical/dietary care is strong. I would entrust a family member to this dependable, accommodating team and their new secured memory-care unit.

    Summary of reviews

Reviewer feedback for this facility is mixed, with a clear pattern of strong clinical and therapy services juxtaposed against operational and environmental concerns. On the positive side, the facility receives consistent praise for its short-term rehabilitation capabilities: therapy teams are described as effective and dependable, there are documented strengths in wound care, and an experienced certified treatment nurse is cited as a clinical asset. The facility maintains a dedicated rehabilitation unit and has added a secured memory-care unit intended for residents who require skilled-level services. Its location near Atrium and Novant hospitals is a logistical advantage for families and clinical coordination.

Care quality appears variable. Several accounts praise clinical outcomes and dietary attention, while other reports raise concerns about medication-management practices and gaps in assistance with activities of daily living, including inconsistent bathing and toileting support. Some reviewers describe a pattern of adverse events and injuries that point to potential weaknesses in incident-prevention and transfer-safety processes; prospective families should seek recent incident data and fall-prevention protocols during a tour.

Staffing and interpersonal conduct are areas of divergence. Many comments highlight caring, welcoming frontline staff and a dependable rehab team. Conversely, reviewers also report insufficient CNA coverage on some units, frequent staff turnover and unexpected layoff events, and variability in staff professionalism and tone. These operational staffing issues are linked in reviews to delayed responses to resident needs and to family-contact problems (for example, difficulty obtaining callbacks or assistance with resident communication devices).

Dining and daily routines elicit concerns about inconsistent mealtime assistance and dining-safety oversight. Alongside positive notes about dietary attention, there are descriptions of lapses in supervision during meals that suggest the facility may need clearer protocols for mealtime assistance for residents who require help.

Facility condition and management practices show both investment and unresolved issues. Multiple reviewers note ongoing renovations and upgrades meant to improve quality of life, while others cite maintenance problems such as broken equipment and peeling finishes. Sanitation and odor concerns in certain common areas and units recur in the comments, indicating inconsistent environmental controls. There are also serious governance-related remarks, including an allegation of rating manipulation and worries about management transparency and staffing-change processes; these raise questions about institutional stability and oversight.

Notable patterns for prospective residents and families: rehabilitation and specialized clinical teams are the facility's relative strengths, and recent capital and program investments (including a memory-care unit) suggest active development. However, persistent operational issuesโ€”especially around staffing consistency, environmental cleanliness, family communication, medication oversight, and maintenanceโ€”warrant direct inquiry. Recommended next steps for families: schedule an on-site visit during mealtime and therapy hours, request current staffing ratios and turnover data, review recent inspection and compliance reports, and ask about specific protocols for medication management, bathing schedules, incident prevention, and family communication policies.
