    Bright clean facility; compassionate staff

I placed my mom here and feel very satisfied - the facility is clean, bright, and welcoming, with on-site rehab and a large PT gym that helped her regain mobility. Nurses, aides and admissions staff (including Kim, Fatima, Kira, Enoch, Danielle, Marta) have been attentive, compassionate and communicative, meals and activities keep residents engaged, and I have real peace of mind.

    Summary of reviews

Clear Creek Nursing and Rehabilitation Center elicits a strongly mixed set of impressions. Many families and residents praise the physical plant and clinical rehabilitation services: the building is frequently described as clean, bright, and modern, with roomy accommodations, barrier-free showers, a large therapy gym, and on-site rehab services. Specific clinical strengths are evident in progressive therapy outcomes and wound-care management; several accounts single out dedicated clinical staff (including wound-care nursing) who produced measurable improvement. Admissions and front-desk staff are generally described as welcoming, and routine social programming (bingo, holiday meals, chapel services) and family communication receive positive comments.

At the same time, recurring operational weaknesses create a clear pattern of concern. Staffing levels are a consistent theme: families describe shortages that are most acute at night and on weekends, variable performance between day and night shifts, and frequent use of agency staff. Those staffing gaps are linked to inconsistent responsiveness to call bells and care requests, delays in assistance with toileting and repositioning (incontinence-care delays and pressure-injury risk), and uneven medication-administration timeliness. Several reviewers also raised safety-related concerns such as inadequate supervision leading to falls or worsening pressure injuries; these indicate gaps in clinical oversight and care continuity rather than isolated facility features.

Administrative and management problems are another repeated pattern. Multiple accounts describe billing and insurance-claims errors, poor follow-up on family inquiries, long telephone hold times, and front-office responsiveness issues. Laundry and personal-property handling problems — missing clothing and need to replace items — were noted across reviews and point to weaknesses in property-management processes. There are also mentions of inconsistent infection-control and pest-management practices and variable food quality: while some residents find meals well-tailored and satisfactory, others describe bland seasoning or inconsistent presentation.

The net impression is of a facility with substantial strengths in infrastructure, rehabilitation services, and certain committed clinical staff, paired with operational and management shortcomings that can materially affect resident experience and safety. Prospective families should weigh the facility’s therapy capabilities and clean, modern environment against documented risks tied to staffing patterns, administrative reliability, and property management. When evaluating Clear Creek, consider asking specific questions about night/weekend staffing ratios, medication-administration protocols, laundry/property procedures, how leadership communicates with families after an adverse event, and the facility’s processes for pest control and infection prevention. Direct observation of shift changeover, a therapy session, and mealtime can also help clarify whether the positive clinical attributes are matched by consistent day-to-day operations.

You'll find the staff to be fully trained, friendly folks who help with medical needs and everyday activities, whether someone needs round-the-clock nursing after an illness or just a little extra help getting around.

Though there are times when the center isn't open to new residents, it stands as a steady option for both short-term recovery and longer-term living, with 24-hour support and rehabilitation services.
