    Clean caring staff excellent rehab

I'm very satisfied with this clean, well-run facility - the staff are caring, respectful, and true advocates for residents. Nurses and aides (special thanks to Jaz and Victoria) gave compassionate, attentive care and recovery-focused therapy (PT/OT/ST) that helped me improve quickly; the food, activities, and team communication were great. I highly recommend this community for anyone needing skilled nursing or rehab.

    Summary of reviews

The reviews for Smithfield Manor Nursing and Rehab are mixed but show two clear themes: many families and residents praise the clinical rehabilitation and day‑to‑day caregiving, while a smaller set of reports raise operational and conduct concerns that merit further inquiry.

Care quality and therapy: The facility is frequently described as strong on short‑term rehabilitation and restorative care. Reviewers highlight structured PT programs, measurable recovery outcomes, coordinated therapy services (PT, OT, ST), and staff who push for progress. Several families credited the therapy team and nursing staff with meaningful functional improvements and relatively quick recoveries for post‑acute patients.

Staff and day‑to‑day caregiving: Many accounts emphasize compassionate, long‑tenured aides and nursing staff, teamwork across shifts, and staff who advocate for residents. Positive comments single out individual caregivers and describe respectful interactions, regular hygiene assistance, and close family communication in many cases. At the same time, other reviews describe operational weaknesses: staffing shortages that reduce floor coverage, inconsistent responsiveness to call lights and family inquiries, uneven personal‑care scheduling (including bathing), and occasional concerns about staff conduct and tone. A few reviews raised serious allegations about individual staff behavior; those accounts are outliers but should be investigated by families and regulators as appropriate.

Dining and activities: Dining receives generally favorable remarks, particularly where a dietitian is involved and menus are palatable to residents. However, there are isolated mentions of meal‑service problems and inconsistencies in how meals and medications are coordinated. Activity programming and therapy‑driven engagement are described positively, with residents participating in recovery‑focused exercises and social options.

Facilities and environment: Several reviewers describe the building as clean and well‑run, but others characterize portions of the facility as outdated or institutional in feel. Management has indicated plans for remodeling to address atmosphere and appearance, which aligns with reviewer suggestions for environmental improvements.

Management and administration: Operationally, the facility appears to be responsive in some areas (therapy organization, planned renovations, dietary involvement) but less consistent in others. Notable administrative concerns include billing and accounting irregularities (delayed or confusing statements) and communication gaps between staff and families. These administrative issues, combined with staffing variability, can affect family confidence even when clinical care is strong.

Notable patterns and considerations for families: If your priority is rehabilitation and structured therapy, Smithfield Manor appears capable and often effective. Prospective residents and families should, however, investigate current staffing levels, ask about call‑response protocols, review medication‑administration safeguards, and clarify billing practices before placement. For any serious conduct concerns mentioned in a small number of reviews, request documentation of staff background checks, complaint investigations, and the facility’s process for incident review. Overall, the facility demonstrates solid clinical strengths with some operational and administrative areas that would benefit from targeted improvement.

About Smithfield Manor Nursing and Rehab

Smithfield Manor Nursing and Rehab sits at 902 Berkshire Road in Smithfield, NC, right within walking distance of Johnston Medical Center, and the place takes care of seniors who need long-term nursing or rehab care and has a total of 160 certified beds, all dually certified for Medicare and Medicaid, which means residents with those coverages can use their benefits here. The facility is a family-owned, for-profit corporation, with Carolyn Arnn having managed since December 2005 and ownership shared among several members of the Arnn and Maness families, and every owner holds a specific share. The center runs under Fair Housing and Equal Opportunity standards and accepts Medicaid for seniors who need help paying for care, plus long-term care insurance options might be available. Smithfield Manor provides 24-hour skilled nursing care and has a staff of qualified nurses and physicians on site, and there's a lower nurse turnover rate here compared to the state average, with nurse hours per resident a bit above the usual for North Carolina. For therapy, residents get physical, occupational, and speech therapy in the rehab wing, with private rooms for recovery if needed, and there's wound care, post joint replacement therapy, and Vital Stim Therapy among the specialized services. You'll find help with everyday things like bathing, dressing, eating, and there's a team for social work and psychological needs, along with hospice care, religious services, and activities every day. Beauty and barber services are on request, laundry happens every day on-site, and the pharmacy service means the facility stores and dispenses drugs safely. Residents get individualized care plans made for their needs, and meal services include therapeutic diets. There are private rooms available, family councils that are active, and a full-time admissions department to help with moving in. Smithfield Manor reports regularly on inspection deficiencies-there've been 11 cited in the latest reports-and holds to federal care standards, aiming to provide for most needs right on the campus, with a big mix of medical, therapy, administrative, and nursing services all under one roof.
