    Caring facility feels like home

I placed my mom here and I'm very pleased - the facility feels like home: clean, comfortable and welcoming. Staff and leadership are caring, experienced and attentive (nurses/CNAs call residents by name), meals and activities are solid, and administration is responsive, so our family has real peace of mind; I recommend it.

    Summary of reviews

Reviews of Pine Ridge Health & Rehab show a strongly polarized picture: many families describe compassionate caregivers, effective therapy services and a well-attended memory-care unit, while others report systemic operational problems that affect safety and quality of life. Positive accounts emphasize committed nursing and CNA staff, long-tenured employees, engaging activities (for example, bingo and other recreational programming), and elements of a clean, comfortable environment that some families found reassuring. Rehabilitation services and emergency-transport responsiveness receive consistent praise in multiple accounts.

At the same time, a recurring pattern of staffing and operational concerns appears across reviews. Families describe inconsistent staffing levels (particularly overnight and on certain halls), delayed responses to call lights, and lapses in routine personal-care activities such as bathing and continence assistance. These operational weaknesses are frequently linked to medication-timing delays, missed checks, and longer wait times for assistance. Several reviews also raise sanitation and odor-control concerns in particular areas, and describe maintenance issues related to an older, sometimes confusing facility layout.

Management and leadership are another polarizing area. Some reviewers credit recent administrative changes and engaged leadership with visible improvements, citing better communication, training, and morale in parts of the facility. Others identify gaps in clinical oversight, unresponsive directors or nursing leadership, high turnover, and inconsistent orientation practices for new staff. Linked to those organizational concerns are reports of poor accessibility by phone and difficulties in obtaining timely information, which erode family confidence.

Dining and social programming receive mixed feedback. A number of families and residents praise the menus, mealtime variety, and recreational offerings; others find the dining-room ambiance uninviting and describe residents opting to eat in their rooms. Therapy and skilled-rehab services are highlighted as strong points by several families who experienced positive outcomes.

Several accounts reference serious safety and clinical incidents, including resident deaths and critical hospitalizations, and there are notes of investigations and inquiries. These are described as catalyzing increased scrutiny by families and, in some cases, changes in leadership or practice. Given the severity of those notes, prospective families should review the facilityโ€™s most recent inspection and incident records.

In summary, Pine Ridge presents a mixed profile: clear strengths in caregiver compassion, rehab services, and some administrative improvements, alongside persistent operational challenges tied to staffing, clinical oversight, sanitation in certain areas, and family communication. Prospective residents and their families would benefit from an in-person tour (including night-shift observation), direct questions about current staffing ratios and clinical leadership, review of recent inspection and investigation outcomes, and conversations with current families in the units of interest to corroborate the most relevant, up-to-date information.

About Pine Ridge Health & Rehab

Pine Ridge Health & Rehab sits at 706 Pineywood Road in Thomasville, North Carolina, and serves as a nursing home with 140 skilled nursing beds, where folks can get both short-term rehab and long-term care all under the same roof, and you'll notice a clearly marked main entrance along with helpful signs pointing the way, which can come in handy when you're visiting for the first time. The facility takes care of a range of needs, offering help with daily activities like bathing, dressing, eating, and managing medications, so whether someone's needing a little extra support or dealing with something like Alzheimer's, there's specialized care available, since they run memory care units and have organized individualized programs for residents living with dementia. The health and rehab services include physical, occupational, and speech therapy, all provided by licensed staff, and the care team is made up of nurses, CNAs, and different therapists who watch over residents twenty-four hours a day, working together with the goal of helping each person recover and feel as well as possible while also trying to keep hospital stays to a minimum. Nikki Clements is the administrator in charge, and the management team's known for working to improve both the quality of life and the building's environment, focusing on clean spaces, better decor, and the kinds of activities and menus people can look forward to. Staff here are described as caring, friendly, and helpful, and there's a sense of community since people often work their way up through the team, from jobs like floor tech to maintenance director if that's a path they want to take. Pine Ridge Health & Rehab accepts both Medicare and Medicaid, stores and gives out medications as needed, and has an environment where the staff aim to create a secure, healing atmosphere for everyone, young and old, whether folks are coming in for short-term therapy or staying long-term due to ongoing needs. The review score averages out to 1.8 from about forty reviewers, so experiences do seem to vary, but there are those who say residents look happy and well cared for, while others mention the professional and team-focused attitude of the employees. The place takes an interdisciplinary approach, tailoring care to each resident's physical, social, and emotional needs, hoping to help them feel dignified and comfortable, with a structure that supports a sense of belonging throughout each stage of aging.
