    Bright facility, compassionate, dedicated staff

I moved my father here and have been very impressed with the turnaround under new management - the building is bright with new flooring, therapy rooms, and a family-like, small-town feel. The nursing team and CNAs (Tammy and others) truly care, go above and beyond with wheelchair assistance, resident programs and themed days, and keep families updated (Jenny even took photos). Staff are compassionate, professional and dedicated; I see a very bright future for this community.

    Summary of reviews

The reviews present a polarized portrait of Stillwater Skilled Nursing and Rehabilitation: many families and visitors praise the direct-care teams and therapy staff, while others describe significant operational and infrastructural shortcomings. Strengths are concentrated in hands-on care โ€” nurses, CNAs, and therapists are frequently described as compassionate, attentive, and willing to spend extra time with residents. Activity programming, including themed days and music, and a small, community-like atmosphere are additional positives that families cited when care is working well. Recent leadership and visible cosmetic updates (new flooring, refreshed spaces) have been noted as contributing to an improved environment in parts of the building.

Clinical and staffing patterns are inconsistent across accounts. While direct-care staff receive high marks in many descriptions, there are repeated concerns about reliance on PRN or partโ€‘time nursing, insufficient full-time STNA coverage, and delays in clinical responsiveness (for example, wound treatment scheduling and other time-sensitive care). These inconsistencies point to variable experiences by unit and shift rather than uniformly poor or uniformly excellent care. Reviewers also raised issues with discharge coordination, paperwork processing, and a privacy-related documentation lapse.

Dining and environmental services show operational weaknesses. Multiple accounts indicate inadequate kitchen staffing and management, which has impacted meal quality and service continuity; one area of the campus was noted as lacking its own kitchen capacity. Housekeeping and laundry processes appear inconsistent, with intermittent linen availability and sanitation concerns in some common and care areas. The facilityโ€™s physical plant โ€” including plumbing, ventilation, parking-lot conditions, and occasional waterโ€‘service interruptions โ€” was described as aging and in need of targeted renovation and maintenance.

Administrative and safety-related processes merit particular attention. Several reviews describe poor phone responsiveness, long hold times, and weak follow-up with families. There are also serious administrative concerns: billing transparency, belongings security, and documentation-handling were cited as problematic, including allegations of theft and billing irregularities. Pharmacy access and medication-dispensing authorizations (electronic dispensing systems) were also identified as gaps that affect timely medication delivery. Finally, reviewers expressed unease about mixing higher-acuity psychiatric/dementia populations without clearly distinct care pathways.

Taken together, the pattern suggests a facility with notable strengths at the point of care and renewed momentum under new management in some areas, alongside persistent facility-level challenges tied to infrastructure, staffing models, kitchen and housekeeping operations, and administrative transparency. Prospective residents and families should consider on-site assessment focused on staffing levels by shift, kitchen and linen-management plans, maintenance and renovation timelines, pharmacy/medication workflows, security and billing policies, and how the facility separates and supports residents with differing care needs. Visiting during activity hours and speaking directly with therapy and nursing leadership can help gauge whether the facilityโ€™s recent improvements align with a householdโ€™s clinical and operational expectations.

About Stillwater Skilled Nursing and Rehabilitation

Stillwater Skilled Nursing and Rehabilitation sits in Covington, Ohio, about 6.3 miles from Piqua, and does a steady job providing skilled nursing and rehabilitation care for seniors, and the staff there are around day and night to help with medical needs, things like IV therapy, wound care, respiratory help-including trach care-dialysis, pain management, feeding tubes, and chest drainage, plus they take time to support palliative care and make sure daily personal care doesn't fall behind, and they don't turn off the lights on weekends or holidays when it comes to therapy, because their team works seven days a week, handling different recoveries from surgery, stroke, heart problems, and broken bones or joint replacements. The facility isn't huge, but it's described as a beautiful place by some families, with a big rehab gym and studio room options, and the people living there can get help with daily life and join in on activities even if they need a lot of hands-on care, plus their community feels welcoming, with a focus on individual needs. Stillwater has amenities like washrooms, fitness centers, pools, patios, hot tubs or spas, and offers several types of residential options in the greater property, including pet-friendly apartments, townhomes, and single-family homes, with some units offering features like air conditioning, though the exact details can shift. They take care to meet legal protections for the LGBTQ community, making sure residents feel respected no matter their identity or orientation. Residents or patients who need short-term stays for rehab or longer-term care for ongoing needs both find support, and the staff focus on helping everyone work towards a meaningful, fulfilling life, which is the way they put it, offering extended and respite care when families need a break. Their ranking stands as the second highest rated community in Covington, with one family giving a strong 8.0 score and an overall average of 3.2 out of 17 reviews, so people seem to think it does well, but as always, the best thing is for families to visit, see the rehab rooms and community spaces, and talk to staff and residents, since no place is the same for every person, though Stillwater does keep its promise of round-the-clock skilled nursing, specialized programs, and a safe place to recover or live when needs change.
