    Excellent attentive nursing and rehab

I'm very pleased with the excellent nursing and rehab team - staff were friendly, professional, and gave my mother attentive, personal TLC. The facility is very clean, easy to access, and provides high-quality care; I would recommend it.

    Summary of reviews

Overall impression: Reviews reflect a facility with clear clinical strengths paired with operational and communication weaknesses. Many comments emphasize strong nursing care and a well-regarded rehabilitation program, while concerns focus on responsiveness, staff conduct, and certain environmental-safety issues.

Care quality and clinical services: Multiple comments praise the clinical teams โ€” particularly nursing and physical therapy โ€” for providing effective rehabilitation and attentive clinical care. Families and volunteers describe staff as capable and compassionate in many interactions; the facility appears to deliver a rehabilitation-focused model that benefits patients needing PT and related services. At the same time, there are repeated concerns about delays in assistance with activities of daily living (for example, toileting and comfort needs) and slower-than-expected responses to patient calls, which suggest variability in hands-on caregiving during some shifts.

Staff and management: Staff competency and kindness are commonly noted, especially among ancillary and volunteer personnel. However, several accounts describe inconsistent professionalism and problems with tone or phone conduct during family communications. Management-level operational gaps are implied by uneven staffing coverage, slow call-light response times, and problems with phone call handling. These issues point to staffing-model and communication-system weaknesses rather than uniformly poor clinical skill.

Facilities, safety, and cleanliness: The physical plant receives mixed feedback. Many reviewers note clean, well-maintained areas and the absence of strong odors in commonly used spaces. Conversely, there are specific concerns about environmental safety and maintenance โ€” most notably wet-area management in bathing/shower spaces and water intrusion โ€” that raise potential slip/fall and hygiene-control issues. Overall, cleanliness appears acceptable in many areas but inconsistent between units.

Dining, activities, and ancillary services: There is limited direct commentary about dining or organized activities. Rehabilitation programming and volunteer engagement receive positive mentions, indicating an emphasis on therapeutic activity. Recreational and social programming beyond rehab is not widely described in the available comments.

Notable patterns and considerations for families: Strengths center on clinical and rehabilitation capabilities and many positive interpersonal interactions with staff. Operational weaknesses cluster around responsiveness (call-light and ADL assistance), communication systems (phones and family contact), variable staffing levels, and specific facility maintenance/safety concerns. Prospective residents and families may wish to ask management about current staffing ratios, call-light response metrics, phone/communication procedures, bathing-area maintenance and safety protocols, and recent cleanliness audits to assess whether the operational issues raised have been addressed.

About Sanctuary of the Ohio Valley

Sanctuary of the Ohio Valley sits on South 5th Street in Ironton and has served its community for over 15 years, always trying to keep things comfortable, friendly, and respectful for its residents. This is a non-profit retirement home with both skilled nursing and intermediate care for people who need regular help, covering everything from personal care to long-term nursing and also short-term rehabilitation after a hospital stay. People can choose between private and semi-private rooms, staying for the long haul or coming in for just a short break with respite care, and everyone gets an individualized plan based on what they need, written up by experienced medical staff and therapists. This place has registered nurses, LPNs, and CNAs working around the clock, plus therapy teams for physical, occupational, and speech therapy-some folks just need help after a stroke, injury, or when they're trying to get more mobile again, while others need extra support as they age. Sanctuary of the Ohio Valley provides plenty of basics, like medication management, nutritious meals, help with bathing, palliative care for people with long-term serious illness, and even homemaking and activity planning. There's a good amount of programs meant to help residents stay social and active, and the building itself is air-conditioned, accessible for wheelchairs, and has parking and nice outdoor landscaping. Family and resident councils allow people and their families to get involved in community matters, sharing what's working or what could be better. The facility's inspection results show an A+ for keeping things up to standard, with overall care quality scores often at A or A-minus across the board, though its most recent overall grade was a B-minus, which is still considered good. After so many years, Sanctuary of the Ohio Valley has built a solid reputation for offering reliable help to older adults, with attentive and compassionate care squarely focused on both physical and emotional well-being, all supported by a steady staff, on-site physicians, and a Medical Director. The community's goal is always to keep things safe, secure, and as home-like as possible for seniors with all kinds of needs.
