    Kind, communicative, family-like care experience

I moved my mom into Eagle Pointe and have been very pleased - the staff treated us like family: kind, patient, compassionate caregivers and an outstanding activities team with lots of outings. Laura the social worker handled insurance and hospice coordination smoothly, admissions and nursing were helpful and communicative, the facility is updated with a calming backyard, and overall it's a very positive, family-like experience I'd recommend, with only minor room for improvement.

    Summary of reviews

Overall impression: Reviewers describe Eagle Pointe Skilled Nursing & Rehab Center as a facility with many relational strengths and a generally updated physical plant, but with some recurring operational concerns. Positive comments emphasize the caregiving culture — staff are frequently described as compassionate, attentive, and family-like. Several families highlighted smooth care transitions from rehab to long-term placement and praised the facility’s social work and admissions staff for handling logistics, arranging services (for example, on-site salon appointments), and coordinating hospice and insurance matters.

Care and clinical oversight: Caregivers and nurses receive consistent praise for day-to-day support, individualized attention, and communication about changes in condition. That said, there are intermittent reports indicating uneven clinical decision-making and communication gaps with families. These comments suggest variability in responsiveness and clinical consistency rather than a uniformly poor standard of care. Families moving residents in from other facilities frequently noted relief and appreciated patience with difficult transitions, though some residents experienced emotional adjustment challenges during the move.

Staff, conduct, and management: Many reviewers praised specific staff and leaders (named social workers and admissions staff were singled out for helpfulness). At the same time, multiple comments point to inconsistent professionalism: examples include negative phone interactions, staff engaging in politicized conversations in resident areas, incidents that family members perceived as public embarrassment of residents, and at least one allegation of derogatory language directed at a transgender resident. These items indicate gaps in staff training or culture around resident dignity, privacy, and inclusiveness. Management presence is described as courteous by some families, but other accounts note uneven communication and occasional perception of being talked over or not listened to.

Activities and programming: The activities department is repeatedly cited as a strength. Reviewers mention frequent outings, engaging events, and active posting of activities (e.g., on social media), which families found helpful for staying informed and for resident engagement. This programming, combined with a generally friendly resident community, contributes to a lively small‑town atmosphere.

Facilities and services: The building and grounds receive positive comments for being updated and well maintained, with a calming backyard and accessible on-site services such as a salon. The facility also appears prepared to serve bariatric residents, which some families found beneficial. However, there are sanitation concerns tied to cleaning-protocol limitations (specific restrictions on certain disinfectants were mentioned), which have affected perceptions of cleanliness in some areas.

Notable patterns and considerations: Strengths center on relational care, transition support, activities, and physical environment. Primary areas for improvement are consistent professional conduct, clearer and more responsive family communication, uniform clinical practices, and review of cleaning protocols and staff training on resident dignity and inclusivity. Prospective residents and families valuing active programming and a compassionate staff may find Eagle Pointe a strong option; those particularly sensitive to staff professionalism, privacy practices, or specific expectations about population mix should discuss those topics directly with facility leadership during a tour and pre-admission conversations.

About Eagle Pointe Skilled Nursing & Rehab Center

Eagle Pointe Skilled Nursing & Rehab Center sits over at 87 Staley Rd in Orwell, Ohio, where folks can find a skilled nursing and rehabilitation center with 60 certified beds and about 55 people living there each day, and while the building gives a cozy, home-like feeling, it's got all the usual care options like long-term care, secure memory care for Alzheimer's and dementia, respite care for short stays, and therapy for those needing physical, occupational, or speech help. There's a nurse on-site for 12 to 16 hours daily and staff who stay all day and night, with a call system for emergencies, and families will find a team that includes nurses, doctors, social workers, and nutritionists working together to look after the residents, even though the nurse turnover rate sits at 45.5% and each resident is scheduled to get 3.29 nurse hours each day. Eagle Pointe is owned by Pointe Woods Investment LLC, and the indirect owners-Jeffrey Goldstein, Alexander Sherman, and Samuel Sherman-each hold a third share, and Alexander Sherman and Samuel Sherman have managed the place since spring of 2017, and while the center is affiliated with Aom Healthcare, it does have 22 deficiencies from recent inspections, including a few issues with providing special eating tools, trauma-informed and culturally competent care, and not always following residents' care goals or treatment orders. Medicaid and Medicare are accepted, and Eagle Pointe is VA-approved, so residents can pay in different ways. The facility isn't taking new residents right now, but it tries to make life comfortable with services like help with bathing, dressing, and mobility, housekeeping, laundry, move-in help, and family support, and you'll find community activities, an arts room and library, games, music, movie nights, outdoor programs, a garden, walking paths, a spa and wellness room, and different rooms for relaxing or joining events. The dining setup is restaurant-style, inside a dining room, and includes options for those who need special diets, like folks with diabetes or allergies, and most rooms come with private bathrooms, cable TV, Wi-Fi, air conditioning, furnished spaces, and a kitchenette, and residents who can't walk or get around by themselves can still get the care they need. English is spoken by the staff, and the profile for the center is kept up to date online, with different ways to view and save the information for yourself if you need it. Even with the focus on creating a caring space, the center's history includes deficiencies around how care is delivered, so families may want to check recent reports as they consider options for skilled nursing care in the area.
