    Attentive staff excellent care welcoming

I had a very positive experience - attentive, genuinely caring staff (Connie, John and many others) made it feel like family and went above and beyond. Skilled nursing, excellent wound care, and effective PT/OT in a large, relaxing therapy space helped me improve and meet my goals; management and support teams (housekeeping, kitchen, maintenance, dietitian) were responsive. The remodeled, home-like, secure wings with family events and a private courtyard are welcoming; I recommend it, with just a few minor areas for improvement.

    Summary of reviews

Majestic Care of Perrysburg elicits strongly mixed impressions: many families and residents praise warm, family-like interactions and an interdisciplinary approach to rehabilitation and wound care, while others describe operational inconsistencies that affect care reliability. Strengths include a dedicated memory-care unit with secure entry, an apartment-style layout with a private courtyard, remodeled private rooms, and visible management engagement. Therapy services (physical and occupational), in-room PT options, and coordinated support from dietary, maintenance, and housekeeping are repeatedly noted as contributors to successful short-term rehab stays and positive discharge outcomes.

Staffing and direct-care quality are areas of clear divergence. Numerous accounts highlight attentive aides, skilled nurses, and individualized bedside manner that create a home-like atmosphere and meaningful resident relationships. Conversely, reviewers describe inconsistent staffing levels and training, leading to delayed call-button responses, uneven personal-care routines (notably bathing and toileting assistance), and variable hygiene or odor concerns in some areas. Several families identified breakdowns in accountability and safety practices; a small number described a serious safety-related incident resulting in hospitalization and potential legal follow-up.

Dining and nutrition emerge as a recurrent operational pain point. While the facility has multiple dining areas and staff support, meal quality is described as inconsistent — some residents appreciate certain menu items, but others report limited variety, unappealing meals, and weight loss concerns during stays. Activity programming is a relative strength: residents benefit from gardening, pet days, seasonal events, and family-oriented dinners that reinforce a community atmosphere, especially within memory-care programming that emphasizes individualized plans and secure, familiar spaces.

Facility condition and management responsiveness present a mixed picture. Renovated wings, colorful resident areas, and clean, easy-to-navigate common spaces receive praise, and several reviewers commend visible leadership and positive interactions with social work or directors. At the same time, some families encountered rooms in need of maintenance, crowded hallways or small rooms, occasional odor concerns, and inconsistent housekeeping. Communication is uneven — many families experience transparent, helpful staff communication, while others note confusion or delays in treatment updates and care-plan paperwork.

Overall, prospective residents and families should weigh the facility’s strong rehabilitation and memory-care offerings and many reports of compassionate staff against documented operational inconsistencies. For residents with high-dependency needs, families should verify staffing ratios, toileting and bathing schedules, and safety protocols. A focused tour that checks private-room conditions, dining, and response times to call buttons, plus direct conversations about individualized care plans and contingency procedures, would help assess fit for a given resident’s needs.

    About Majestic Care of Perrysburg

Majestic Care of Perrysburg is a small, corporate-owned nursing home in Wood County with 75 beds, and when looking around the place, you can see they try to cover a lot of needs, whether someone's needing short-term rehabilitation after a hospital stay or is looking for long-term care with 24-hour nursing for more serious health problems. They help with daily tasks like bathing, dressing, managing medicines, and moving around, with staff on hand at all hours and a 24-hour call system in all rooms, which have private bathrooms, air conditioning, cable TV, and Wi-Fi so folks can stay comfortable. There are different care sections, like assisted living in "Commons at Perrysburg," and memory care for people with Alzheimer's or dementia in a secure unit, with care focused on keeping residents safe and involved through memory-enhancing activities and specialized programs.

They've got skilled nursing and therapy services, including physical, occupational, and speech therapy, along with wound care and respiratory therapy, plus outpatient therapy for people who need it after going home. You'll find daily activities planned by both staff and residents, from art and music programs to movie nights, fitness classes, and trips outside the community, and residents can spend time in spaces like a game room, a small library, an arts room, a movie theater, or outdoors where there are gardens, walking paths, and gathering spots for events or just sitting. Folks who like a quiet moment can use the wellness center or spa room, and there's also a computer center, beauty salon, private suites for rehab patients, and scheduled transportation for appointments or errands.

For meals, residents eat in a dining room where there's restaurant-style service, with menus that work for special diets and a chef handling meal prep, taking care with allergies and other needs. Housekeeping and linen services, laundry, and move-in help are available along with management of resident funds and careful handling of money during discharge. The place stays on top of safety, with regular inspections, infection control, and routine health checks, and there's a registered nurse on shift daily with a nursing director overseeing things.

Residents can stay on a respite basis for a few days up to 30 days if families need a short break from caregiving, with furnished rooms for comfort. There are community programs to foster dignity, choice, and open communication, helping people feel respected and supported while they recover or adjust. The staff is trained to work with older adults, whether their needs are simple or complex, and offer both personalized care and transition assessments within 48 hours of admission or if a resident's health changes. Majestic Care of Perrysburg takes Medicaid, Medicare, and private insurance, so families can choose how to pay. They also offer hospice care focused on comfort and emotional support at the end of life. In all, it's one of those communities that try to cover all the bases for seniors who need daily support, skilled medical help, or just a place that's easy to live in as needs change with time.
