    Impeccable care and restorative rehab

I'm very pleased with most aspects of this facility: impeccable skilled nursing, approachable and professional therapists and nurses, a very clean, home-like environment, appetizing meals, prompt and caring staff (Catrina, Tanya, Amanda, Danielle), and strong rehab and activities that helped my mom regain function. I have peace of mind and would recommend it.

    Summary of reviews

The reviews for Edgewood Manor Rehabilitation and Healthcare Center present a mixed but instructive picture. Many families and visitors describe a facility with strong clinical strengths: skilled nursing and rehabilitation services are frequently characterized as professional, effective, and responsive. Therapists and nurses are commonly described as approachable and attentive, and admissions and first impressions are often positive. Reviewers also praise an inviting, home‑like atmosphere, good housekeeping, and a generally clean interior environment.

Dining and social programming are another consistent strength. Meals are described as appealing and nourishing, and the campus supports a range of daily activities and seasonal programming that residents and families appreciate. The facility is noted as dog‑friendly, which some families cited as enhancing the community feel. Several comments emphasize family‑focused care and staff who listen and adjust care plans when needed.

Alongside these positives are repeated operational concerns that prospective families should evaluate closely. Clinical monitoring gaps appear in several areas: medication administration and documentation, accurate delivery of therapeutic diets, and routine weight/vital‑sign monitoring. Therapy continuity and scheduling have been described as inconsistent at times, and there are mentions of insufficient on‑hand patient equipment (for example, bedside commodes or functional scales), which can affect daily care and clinical follow‑up.

Staffing and management issues are an important pattern in the feedback. While many reviewers praise individual caregivers, others report persistent understaffing and workload strain that they believe has contributed to declines in service consistency. Some families perceive management as prioritizing financial considerations over operational needs, and there are serious allegations of discriminatory treatment that merit attention and direct inquiry during a tour or intake process.

Facility features receive generally favorable comments for cleanliness, décor, and a pleasant rehabilitation environment, but perceptions vary across areas and over time; some reviewers describe variability in sanitation and maintenance. Outdoor amenities appear limited in parts of the campus, with comments about lack of shade and sidewalks affecting residents’ ability to use exterior spaces comfortably.

For prospective residents and families: observe med‑pass routines, ask to see how diets and therapeutic diets are verified, confirm how weights and other clinical indicators are tracked, and inquire about equipment availability and replacement policies. Ask for current staffing ratios, turnover data, and details about the facility’s complaint and nondiscrimination policies. A focused tour that includes a therapy session, a dining observation, and conversations with both nursing leadership and direct‑care staff will help validate whether the facility’s strengths align with your priorities and whether operational concerns have been addressed.

About Edgewood Manor - Port Clinton

Edgewood Manor Rehabilitation and Healthcare Center sits in Port Clinton, Ohio, right on the shores of Lake Erie, and serves the Ottawa County community with a mix of care choices, and while it has 80 certified beds and about 61 residents on an average day, it's affiliated with Crown Healthcare Group and operates under Covenant Care, too. This family-run center has a staff size somewhere between 51 and 200 workers, and people will find short-term, long-term, intermediate, and skilled nursing care available here, along with subacute, cardiac, and orthopedic rehab, plus stroke recovery and memory care programs for folks who need support with Alzheimer's or dementia. You'll also find home health, outpatient therapy, transitional care, respite care, hospice care, and emergency care services here, making the place prepared for a lot of different medical needs, and they'll help with medication monitoring and meal preparation, while offering some community and recreational activities intending to keep things lively and supportive for the residents.

Now, the center has nurse coverage at about 3.41 hours per resident each day, which is just below Ohio's average, and the nurse turnover rate is a bit higher than other Ohio facilities, sitting at 53.7%. Past inspection reports from March 2025 and November 2024 note 34 separate deficiencies, including two tied to infection-related problems, and the place has been cited for concerns like how medication and drugs are stored and labeled, as well as for care issues affecting people who are continent or incontinent, and these reports highlight some matters with infection control and pharmacy services that the state found. Even with these issues noted on inspections, Edgewood Manor says it aims for compassion and tries to keep the place warm and homelike, and the campus can be seen through a video tour for families who want a look before stopping by. They support both people recovering from illness or surgery and folks staying for a longer time, and the staff uses medical tools and therapy to help residents heal and feel as comfortable as possible with amenities focused on their everyday well-being. The admissions office is ready for questions, so anyone wanting to know more can reach out that way.
