    Compassionate staff made transition home

I placed my loved one at this family-owned Youngstown facility and am very pleased. Owners Lisa and Chris, administration and staff (especially rehab director Missy and social services Ivorie) are compassionate, attentive, and truly put residents first; the skilled therapy team made the transition home smooth. It's welcoming, family-oriented with nice community events - I highly recommend it.

    Summary of reviews

Overall impression: Reviewers present a bifurcated picture of Maplecrest Nursing Home. Many families and professionals praise the facility for its therapeutic offerings, personable staff, and family-owned atmosphere; others raise operational concerns around communication, medication oversight, cleanliness, and management responsiveness. The facility consistently earns positive remarks for its rehabilitation services and certain aspects of day-to-day care, but reported experiences appear to vary across shifts and individuals.

Care and staff: The strongest, most consistent strength described is direct caregiving and therapy. Reviewers highlight skilled therapists and knowledgeable rehab leadership who support strong outcomes for residents returning home. Social-services staff are also frequently credited with facilitating smoother transitions to community settings. At the same time, there are recurring descriptions of uneven staff coordination and variable conduct: some families encountered delayed responses, curt interactions, or difficulties getting consistent information. There are also medication-management and psychiatric-oversight concerns raised by a subset of reviewers; these relate to clinical decision-making and communication about psychiatric consultation and diagnoses and warrant specific questions during any evaluation.

Dining and activities: The facility's activity program and community events receive positive remarks as contributors to a family-friendly culture. Multiple comments note an active calendar, engaging social opportunities, and generally favorable meal service. These elements appear to enhance resident quality of life for many families.

Facilities and cleanliness: The physical plant is frequently described as attractive and well maintained, with pleasant grounds and comfortable common areas. However, some reviewers noted sanitation or odor concerns in particular areas, indicating variability in environmental maintenance. Prospective families should assess cleanliness of both resident rooms and high-traffic common spaces during a visit.

Management and notable patterns: The family-owned management style is often cited as a benefit, with owners and administrators personally involved and approachable in many accounts. Conversely, other reviewers perceived selective enforcement of policies, dismissiveness when raising issues, and inconsistent follow-through on discharge assistance. There are also workplace-culture concerns, including allegations of discriminatory behavior, that some prospective residents and families may want to explore with administrators. Given the mix of positive and negative experiences, it is advisable to tour the facility, meet therapy and social-services leaders, ask for details on medication-review processes, staffing levels across shifts, cleaning protocols, and examples of discharge-planning outcomes to determine fit for a particular resident.

    About Maplecrest Nursing Home

Maplecrest Nursing Home sits at 400 Sexton Street in Struthers, Ohio, and has been family-owned and locally operated since 1966, which feels like you can tell once you step through the door and notice that comfortable, home-like setting they've got because it never feels too big or too cold or anything like that. The facility holds a CMS 5-Star rating, which means high standards in care, and the staff are a team of licensed professionals who focus on making sure everyone gets personal, compassionate care no matter if someone's there for long-term care, skilled nursing, or just a short stay for good old rehabilitation with physical, occupational, or speech therapies. The focus stays on each resident's needs, so they make personalized care plans and try to meet physical, emotional, and spiritual needs in ways that show real dignity and respect, which sometimes is hard to find these days. There's memory care for people with Alzheimer's or dementia, and they also have hospice and palliative services for those who need extra comfort-plus, if someone needs a break, respite care is available, too. Around the clock nursing care is in place, and the team works closely with physicians and families to give consistent and thoughtful help no matter what comes up day or night. Meals are prepared onsite with attention to good nutrition, and folks can keep busy with social activities and programs that help residents stay connected in ways that matter to them. Maplecrest is proud to remain a smaller, family-run place where staff hope to build strong bonds with both residents and families, and the facility's earned various accreditations through the years. With a simple community setup and shared neighborhood spaces, residents have the chance to enjoy senior living that emphasizes comfort, safety, and a sense of belonging above everything else.
