    Compassionate communicative staff and rehab

I've been very pleased with my loved one's care here. The admissions, nursing, rehab and therapy teams (shoutout to Charles, ShaDonna and Mimi) are caring, communicative and present - rehab strengthened him, memory-care is thoughtful, and staff kept us updated through COVID. Rooms are clean and secure, activities are engaging, and maintenance and management made the move-in smooth. Overall a safe, compassionate place with only minor room for improvement.

    Summary of reviews

Reviews of ProMedica Goerlich Memory Care Center Sylvania present a mixed but detailed picture. Many families praise the facility for its compassionate, resident-focused caregiving, strong communication pathways with family members, and well-organized admissions and administrative support. Several accounts emphasize effective, targeted rehabilitation (PT/OT), responsive therapy teams, and rehabilitation outcomes that improved residents’ strength and function. The environment is frequently described as clean, calm, and well maintained, with private rooms, onsite grooming services, secure wandering-prevention systems, and pandemic-era visit accommodations that some families found reassuring.

Staff-level feedback is predominantly positive around respectfulness, empathy, and day-to-day interpersonal care: reviewers commonly describe nursing and aide staff who are personable and attentive, as well as recreation teams that provide engaging memory-focused programming (singing, conversation, and social activities). Admissions and management staff members are repeatedly noted as helpful, knowledgeable, and effective at coordinating insurance and home-care transitions.

However, there are recurring operational concerns that prospective families should weigh. A pattern of inconsistent staff conduct and professionalism emerges in several accounts, ranging from curt or inappropriate communication tone to more serious allegations regarding conduct and resident dignity. Related clinical concerns include gaps in medication administration and uneven responsiveness to residents’ daily needs. Housekeeping and meal-service experiences appear variable: while many describe a clean facility, others report sanitation-related inconsistencies and uneven meal quality or presentation.

Management and organizational issues also appear as a notable theme. While some reviewers commend the administrative team, others express frustration about inconsistent family accommodations and unclear alignment between advertised memory-care specialty services and the facility’s capacity to manage advanced needs. One review thread references an impending closure and the emotional and logistical disruption that can introduce for residents and families; this highlights a continuity-of-care risk that merits direct inquiry during tour and intake.

Recommendation: Prospective residents and family members should arrange an in-person tour, ask for current staffing ratios and turnover statistics, review medication-administration and incident-reporting protocols, and request recent inspection or quality reports. Inquire specifically about how the facility supports complex dementia and Parkinson’s needs, how they train and supervise staff on professional communication and dignity-preserving care, and what contingency plans exist for organizational changes. The facility shows clear strengths in compassionate caregiving, therapy services, and environmental maintenance, but also demonstrates variability in operational consistency that families should clarify before placement.

About ProMedica Goerlich Memory Care Center Sylvania

ProMedica Goerlich Memory Care Center in Sylvania focuses on care for people with Alzheimer's, dementia, and memory loss, and the place feels pretty peaceful, especially for those with these needs, so you can expect the nursing staff and therapy staff to really understand what it means to look after someone going through stage five dementia, and you'll see that the staff checks in often and shows a lot of patience and kindness, which is important. The medical director's a neurologist, which makes a difference when you're thinking about memory care, and there's a strong nursing presence, too, with 12 to 16 hours a day of on-site nursing support and a 24-hour call system, so if someone needs help, help comes quickly. The focus stays on making sure people feel safe and engaged, which is why you see secure memory care areas, outdoor gardens for walks or sitting, and entertainment spaces like a movie theater and music programs, plus plenty of scheduled activities, therapy groups, and events, and it feels like there's always something going on during the day, whether it's art, music, or time in the library.

The center's small and non-profit, and part of the larger ProMedica health system, which is pretty well regarded, and the community holds good grades on inspections, with an A+ for recent checks and high marks for nursing care. Skilled nursing, rehabilitation, and therapy services are all offered, including physical, occupational, and speech therapy, plus special support for people dealing with mental illness and more complex needs, so folks living with dementia and Alzheimer's have care plans tailored just for them. People can get help with things like bathing, dressing, taking medicine, and moving around, even if they're not able to walk, because the building supports non-ambulatory care, and the staff speaks English.

Residents have a choice of studio or semi-private rooms, and the place provides three meals daily plus snacks, and they keep in mind nutritional counseling, especially for those who need help with eating or special diets. There's daily help for activities of daily living and transfers, if needed, and post-acute care is available for people who need extra support after leaving the hospital, so families can look into short-term rehab, long-term stays, or even respite care if it's needed for a break. Adult day care and night services are available at an hourly rate, so some people come only for part of the day, and the rates are straightforward. People who need Medicaid, Medicare, or private insurance will find the center accepts all those.

Everyday life here encourages socializing and mental engagement, with cognitive and social activities designed for memory support, and residents have access to an emergency alert system, fitness facilities, activity rooms, and secure outdoor spaces. The center is focused on secure, attentive care for individuals with memory issues, working with individualized plans and therapy programs. Staff get special dementia training and support, plus family and professional caregivers can join educational programs. There's a resident council so people have a way to share their concerns, although there's no separate family council. The occupancy runs high, but the staff-to-resident ratio stays strong, and everyone's focus remains on respectful care, safety, and quality of life for people living with memory loss.
