    Compassionate attentive care, peaceful stay

I had a very positive stay - staff were welcoming, compassionate, and attentive, with excellent nursing and rehab teams who gave personalized care and kept my family well informed. The facility is clean, beautiful (the greenhouse is a nice touch), management is responsive, and meals felt home-cooked, which gave me real peace of mind. I'd choose this place again.

    Summary of reviews

Reviews of Advanced Healthcare Center are highly polarized, with distinct clusters of positive and negative experiences. Many families praise the facility's rehabilitation services and therapy teams, and multiple accounts describe compassionate, attentive caregiving and effective wound-care expertise. Some reviewers also highlight strong family communication, a welcoming atmosphere, a functioning activities program (including a greenhouse amenity), and instances of leadership stepping in to make rapid improvements.

At the same time, a substantial number of reviews raise operational concerns that recur across domains. Staffing inconsistency is a frequent theme: reviewers describe frequent call-offs, long nurse response times, and insufficient aides for routine care tasks. These staffing gaps are associated with delays in attending to resident calls, missed or late medications, and inconsistent assistance with personal-care schedules. Several accounts also cite gaps in clinical processes—particularly related to catheter and continence care, wound oversight, and infection-control practices—which caregivers and families view as safety risks.

Cleanliness and maintenance present a mixed picture. Some areas and units are described as clean and hotel-like, while other accounts indicate sanitation issues, odor concerns, and pest-control problems in portions of the campus. Dining and food service are similarly inconsistent: while home-style meals and adequate dining are noted in positive accounts, others report delayed meal delivery, small portions, and irregular meal service that sometimes requires families to supplement residents’ food.

Communication and management practices are another area of divergence. Positive reviews describe supportive leadership, responsive managers, and clear family engagement. Conversely, negative reviews point to poor communication with families and POAs, lost or mishandled personal items, reimbursement promises that were not fulfilled, and staffing/ payroll or HR instability that reviewers link to operational decline. A few reviews reference legal action and more serious allegations; those items indicate isolated but serious concerns that prospective families may want to explore further with regulators and the facility directly.

Activity programming and therapy are consistent bright spots for many visitors and residents, with staff in those departments frequently receiving praise. Given the mix of experiences, prospective residents and families should verify current staffing ratios, infection-control protocols, medication-administration procedures, personal-property policies, and recent inspection or enforcement records during tours. Asking about recent management changes, turnover rates, and concrete corrective actions for sanitation and clinical-process gaps will help clarify whether the facility’s strengths align with a specific resident’s needs.

    About Advanced Healthcare Center

Advanced Healthcare Center sits at 955 Garden Lake Pkwy in Toledo, Ohio, and gives a wide range of care options for both short-term and long-term needs, so folks can get help with medical recovery, ongoing health issues, memory care, or even hospice and palliative care, and while the clinic location right now isn't taking new patients, the main skilled nursing center is open every day from 9 in the morning until 9 at night and has English-speaking staff on site. The place has a big focus on Physical Medicine and Rehabilitation, which means there are therapy services for people who've had strokes, brain injuries, joint replacements, or even spinal cord injuries, plus there's help for lots of other health problems like Parkinson's Disease, multiple sclerosis, fractures, and neurological disorders, while they also can manage day-to-day things like migraines, acute sinusitis, respiratory infections, pharyngitis, abdominal pain, hypothyroidism, and even acne or eczema, which means people don't have to figure things out on their own if they don't feel well.

Residents and families get involved in planning care thanks to those interdisciplinary team meetings that always try to tailor each care plan to the person, and the care team has a range of staff-pediatrician Dr. Sanford Kimmel even works there, and there are attending physicians from local health systems who handle everything from psychiatry, neurology, wound care, and even infectious diseases, and the nurses work around the clock, ready to help with medication, pain management, or special needs like ventilators, BiPAP/CPAP support, or tube feeding, which is something not everywhere can do.

People who stay here have access to skilled nursing and rehab services, including physical therapy, occupational therapy, speech therapy, and pain management, though the choices don't stop at just therapy rooms-there's a rehabilitation gym, an occupational therapy kitchen, specialty equipment like bariatric beds or lifts, and even a Wii Therapy System for some lighter exercise, while the ADL suite set up like an apartment helps folks get ready to go home, so those who want to regain independence get all sorts of useful practice.

For those who need a break or a place to recover for a short stretch, Advanced Healthcare Center has respite care, short-term and private options, post-surgical recovery, and programs focused on everything from stroke to orthopedics to frailty, and if someone is struggling with things like ADD/ADHD, bipolar disorder, schizophrenia, Huntington's disease, or dementia, there are trained specialists and programs that deal with those, and all the staff get education in caring for people with Alzheimer's or memory challenges.

People here get three meals daily cooked by a professional chef with special diets if needed, and there are always alternative dishes for different tastes, and the Orange Spot Bistro is open for folks who want a change of scenery, while the A360 Lounge comes with iPads and computers, and there's a greenhouse on site full of banana and palm trees, plus shared outdoor patios and spaces for group games, crafts, or shopping trips. Residents can have their hair done at the salon or barber shop, laundry and housekeeping are taken care of, medications are managed by staff, and there's transportation for appointments or outings.

The facility accepts Medicare, Medicaid, and private insurance, and has contracts with most managed care plans, and offers private pay choices as well, so families don't have the trouble of figuring out what works. Amenities like 24-hour staff, private rooms, specialty mattresses, an on-site wound care nurse, and weekly physician visits round out the care, and with Home Health and Hospice, Independent Living, Assisted Living, Nursing Home, and At-Home Care all under one roof, families can find the support they want, whether it's a temporary recovery stay or a new permanent home.

Advanced Healthcare Center belongs to CommuniCare, a family-owned group, and tries to make the building and grounds supportive for guests and their families with programs and daily life built to support both emotional and physical health.
