    Friendly staff and outstanding therapy

I stayed at Laurels of Toledo and was impressed - the staff were friendly, professional, and attentive; the facility and rooms were clean, secure, and comfortable with nice amenities. Therapy/rehab (AlterG, skilled PT/OT) was outstanding, activities plentiful, admissions and family communication were smooth, and I felt well cared for - I'd recommend it for short- or long-term needs.

    Summary of reviews

Reviews for Divine Rehabilitation and Nursing at Toledo present a mixed but clarifying picture: many families praise the facility's rehabilitation services, admissions staff, and a number of genuinely attentive caregivers, while other reviewers describe significant operational shortcomings. A consistent positive theme is the therapy program — reviewers frequently highlight skilled PT/OT staff, modern rehab equipment (including an Alter G treadmill), and a rehab wing that has been renovated and described as a strong driver of successful discharges home. Admissions and marketing personnel are regularly characterized as helpful and knowledgeable, and several accounts note a straightforward admission experience.

Care quality impressions vary. Numerous families describe compassionate, attentive nursing and aide support and positive short-term rehab outcomes. Conversely, a set of reviews raise concerns about inconsistent nursing performance across shifts and units, and specific operational weaknesses such as medication-administration gaps and inconsistent management of chronic conditions (for example, diabetes care). Staffing levels emerge as a recurring theme: reviewers indicate periods of understaffing and high staff-to-resident ratios that correlate with delayed responses and missed care tasks.

Dining and nutrition feedback is similarly mixed. Some reviewers appreciated the dining atmosphere and regular activities around meals, while others cited inconsistent meal quality, small or unbalanced portions, and inadequate accommodation of therapeutic diets. These accounts suggest that nutrition management and special-diet implementation are areas needing clearer protocols and oversight.

Activities and social programming are frequent strengths. The facility is noted for a range of activities, birthday and social events, and family-oriented programming. Communication channels — including email, phone updates, and periodic virtual meetings — are praised by several families for keeping them informed and involved in care planning.

Facility condition and amenities receive largely positive comments: many reviewers describe clean, modern rooms with contemporary furnishings, private phones and TVs, secure entrances, and good parking. At the same time, descriptions point to variability between different wings or areas of the building, with some sections consistently described as more updated and better maintained than others. A number of reviews also raise sanitation and incontinence-care concerns in specific areas, indicating inconsistency in environmental services and personal-care routines.

Management and administrative patterns are mixed and appear to have evolved over time. Multiple reviewers note a change in ownership or leadership in recent years that some say has improved morale and the appearance of the facility. However, other reviews describe rigid visitor policies, front-desk unresponsiveness, scheduling and discharge coordination problems, and instances where family members experienced difficulty resolving grievances. There are also mentions of events that prompted regulatory attention and contentious visitor interactions; these accounts suggest room for improvement in grievance resolution and transparency.

Taken together, the reviews portray a facility with clear strengths in rehabilitation, admissions, and many individual caregivers, alongside systemic operational issues that include inconsistent staffing, medication and chronic-disease management, variable cleanliness, and communication gaps. Prospective residents and families should weigh the facility's strong therapy offerings and renovated spaces against the potential variability in day-to-day nursing care and housekeeping, and should seek specific assurances about staffing levels, medication protocols, dietary accommodations, and visitation policies during their tour and intake discussions.

About Divine Rehabilitation and Nursing at Toledo

Divine Rehabilitation and Nursing at Toledo sits at 1011 N Byrne Rd, and the place runs 24 hours a day every day of the week, so there's always someone around to help out. The facility has a skilled nursing center along with a physical medicine and rehabilitation clinic, offering options for people who need anything from short-term rehab after surgery to longer stays for ongoing care. They've got an in-house MD or nurse practitioner who's on site six days a week along with 24/7 direct admissions from local hospitals, which means people coming from the hospital can get in quickly when space is available, but they're not accepting new patients right now. Rehabilitation is available seven days a week, with therapists using modern equipment like the NASA-developed Anti-Gravity Treadmill called the Alter G, and they work with licensed staff in physical, occupational, and speech therapy. Guests have private rooms with ensuite bathrooms, cable TV, and WiFi, plus an accessible therapy gym, bright hallways for easier walking, and lounges to sit or visit with others. A secure memory care unit features private suites and showers, designed for people living with Alzheimer's and dementia, and the staff in that unit have special dementia care training. The whole place has an atmosphere meant to make residents comfortable, from private rehab suites to a professional chef making meals that fit dietary needs, and regular laundry and housekeeping services included. Residents also have a calendar full of social activities, outings, and events to keep up a sense of community, with gathering spaces and an enclosed courtyard outdoors. Divine Rehabilitation and Nursing at Toledo offers a broad range of specialized programs, like memory care, dialysis support, lymphedema therapy, bariatric care, hospice, orthopedic rehab, tracheostomy care, and stroke or cardiac recovery, as well as behavioral health and psychiatry. The team follows personalized care plans and provides things like medication management, nutritious meals, transportation services, and even a beauty salon for hair appointments. There's a medical director who's a gerontologist with a solid local reputation and gives daily oversight, helping manage both short and long stays. They keep a culture where folks can take part in social and recreational options, schedule activities and outings, and have places to walk, relax, or meet up throughout the building. They speak English and try to help everyone feel as at home as possible, and though they have only one employee on record, the licensed caregivers and staff seem dedicated. Divine Rehabilitation and Nursing at Toledo is listed as a health care facility with a broad business category that includes nursing home services, senior care, adult family homes, and assisted living, all working together to offer post-acute care for many situations. They have a website at divine-toledo.com for more details, and visitors can check out their special rehab programs and therapy options. This facility isn't BBB accredited, but it keeps a focus on providing care in a setting that's designed for comfort, safety, and attention to personal needs.
