    Compassionate staff excellent rehab care

I'm very pleased with the compassionate, professional staff - nurses, aides, therapists and social worker Kat (and Jai) who go above and beyond - and the facility is clean, well-maintained and welcoming. Rehab, personalized family-like care, good meals and active programming helped my stepmother thrive; management is responsive and I recommend this community.

    Summary of reviews

The reviews for Majestic Care of Toledo present a mixed picture with clear strengths in rehabilitation, individualized caregiving, and facility upkeep, alongside recurring operational concerns. Many families and residents describe compassionate, attentive nurses and aides, strong therapy services (physical and occupational), and a generally clean, well-maintained building with private-room options and pleasant communal areas. Social services and select staff members received repeated praise for advocacy and coordination, and several accounts highlight successful short-term rehab outcomes and improved mobility following therapy.

Care quality shows variability. Positive reports emphasize attentive nursing care, regular wound and bandage maintenance, effective medication management in some cases, and individualized therapy plans that facilitate discharge home. Conversely, a number of families described delays in medication administration, issues reaching providers promptly, and gaps in clinical-incident handling that led to external emergency care for some residents. There are also accounts of inconsistent bathing and personal-care schedules and concerns about resident-safety practices (for example, call lights not always within reach or slower response times), which suggest uneven implementation of routine care processes across shifts or units.

Staffing and professionalism are polarizing themes. Many comments praise specific caregivers and teams for going above and beyond, creating a family-like environment and delivering respectful, person-centered interactions. At the same time, reviewers reported variability in staff demeanor and conduct, including unprofessional communication and inattentive behavior on particular shifts. Several families pointed to reliance on agency or temporary staff and noted that care quality and responsiveness were sometimes worse on overnight or second shifts, indicating consistency issues related to staffing models.

Dining and activities receive generally favorable remarks. The facility is credited with varied menu choices, accommodations for dietary needs (including lower-sugar options), and an active programs department offering puzzles, games, and events. Some critiques focus on meal-service logistics — meals served at incorrect temperatures or occasional mismatches between chosen and provided meals — which may reflect operational challenges in kitchen-to-resident delivery rather than menu planning.

Facilities and cleanliness are often described positively (clean rooms, friendly housekeeping, well-kept common spaces and courtyard), but there are intermittent sanitation and odor concerns in some areas that families raised. The memory-care unit and COVID-era infection-prevention practices were noted as strengths by several families, though reviewers also flagged inconsistent infection-control follow-through in other instances.

Management and administration show mixed impressions. Positive experiences include accessible leadership, thoughtful admissions/transfer coordination, and assistance with billing or administrative questions. Counterbalancing this, other families experienced delayed responses from management, billing errors, abrupt discharge processes, and perceived lack of transparency following incidents. These patterns suggest variability in leadership responsiveness and administrative consistency.

Overall, Majestic Care of Toledo demonstrates notable strengths in rehabilitation, individualized caregiving, and a generally pleasant physical environment. However, persistent themes — inconsistent staffing, call-response delays, medication-administration lapses, variable staff conduct, and uneven administrative communication — warrant careful inquiry by prospective residents and families. When evaluating the facility, consider asking for current staffing ratios by shift, medication-administration and incident-response protocols, examples of recent quality-improvement actions, and the facility’s plan to ensure continuity of caregivers (including use of agency staff). Touring the units across different shifts and speaking with therapy staff and social services can help determine whether the facility’s strong elements align with an individual resident’s needs.

    About Majestic Care of Toledo

Majestic Care of Toledo, formerly known as Genacross Lutheran Services Toledo before December 17, 2021, sits in the Toledo, Ohio area and is part of a network of Majestic Care Communities that stretches across Indiana, Michigan, and Ohio, offering services through places like Jefferson Pointe, Sheridan, Bedford, Bloomington, Connersville, and even Carmel Assisted Living, so there's a bit of history behind it since the corporation began way back in 1860 and got incorporated in 1997. The facility's known for assisted living, independent living, memory care, skilled nursing, and a whole range of rehabilitation services, which includes short-term and long-term skilled nursing care, respite care, hospice care, and specialized dementia care, all supported by a Care Team made up of trained, licensed, and friendly staff with a nurse on staff part-time. You'll find about 135 employees here, working under Ms. Cathleen Voyles-Baden, the Executive Director, and they aim to deliver consistent, round-the-clock assistance every day of the year, putting an emphasis on person-centered care that's warm and supportive. Residents get assistance with bathing, dressing, and meal preparation, as well as medication reminders, and can join daily activities tailored to meet their needs-things like arts and crafts, music programs, religious services, facilitated outings, and social or educational events meant to keep folks engaged and connected, not to mention animal visits with pet services and shopping trips using community transportation. People staying short term or for rehabilitation have access to a modern therapy gym and services like outpatient therapy, with physical, occupational, and speech therapy supporting recovery or day-to-day living, plus a balance therapy program and a special MVMT Unit focused on short-term rehabilitation. For those needing long-term care, skilled nursing, or help during the last stages of life, there are private rooms, spacious accommodations, and hospice care focused on pain management and emotional support. Residents' health needs get addressed through a wellness clinic and health maintenance program, and there's financial guidance available, with the facility accepting Medicaid, Medicare, and private pay, even offering VA aid help and home sale assistance for those who need it. Units come furnished and include kitchen appliances, plus several comfort features like a veranda, a chapel, a gift shop, mailboxes, and rooms set up with handicap-accessible bathrooms that offer safety rails and seated showers; heat and air conditioning controls are in the rooms, letting folks set things how they like. There's communal dining but also privacy, and the buildings aim for a home-like, comfortable feel, trying to make things as close to a private house as possible. Majestic Care of Toledo is certified by the Ohio Department of Aging under designation 2285R, though it's not BBB accredited and doesn't have all the details publicly available about every service or amenity, but it's got reliable, friendly staff on hand and encourages folks to come by for a tour, meet the Care Team and other residents, and see the place for themselves.
