    Compassionate, dedicated rehab and care

I'm very impressed with the compassionate, dedicated care my husband received - phenomenal aides and nurses who consistently went above and beyond. The rehab team and gym were top-notch; with their help he's walking again. Staff, social work and leadership coordinated meds, transfers and outpatient care, were always available, and treated our family like family. The facility is clean, meals are good, and long-tenured staff show real expertise - I highly recommend this warm, responsive place and would return.

    Summary of reviews

The submitted reviews present a polarized picture of Best Care Health and Rehabilitation. A substantial body of feedback praises the facility for its rehabilitation capabilities, compassionate bedside staff, and family-focused approach. Therapy servicesโ€”both inpatient and outpatientโ€”are frequently described as skilled, goal-oriented, and instrumental in returning residents home. Many families highlight individual nurses, aides, and therapists who provided attentive, empathetic care, stayed late for visits, and coordinated with social work and discharge planning to arrange home services.

At the same time, numerous reviews raise significant operational and safety concerns. Recurring themes include inconsistent responsiveness to call lights and monitoring systems, delays or gaps in medication administration, and variable recognition of acute medical events that led to urgent transfers. Several comments point to maintenance and equipment issues (nonfunctional call buttons, out-of-order bathrooms) and inconsistent sanitation or incontinence-care practices. There are also serious allegations concerning security of personal belongings and attempted unauthorized financial access; these are described as incidents that prompted law-enforcement involvement in at least one account.

Dining and activities receive mixed feedback. Multiple families report hot, palatable meals and an active activities program; others describe poor meal quality, limited soft-food options, and slow service. The activities program and therapy gym are generally cited as strengths, contributing positively to rehabilitation outcomes and social engagement. Facility condition is described variably: some reviewers call the campus attractive and well-kept, while others note dated rooms, overcrowded memory-care units with double beds and shared baths, and areas in need of updating.

Management and staff culture show both strengths and variability. Several reviews commend proactive administrators and supportive leadership who communicate effectively with families and address concerns. Conversely, other accounts describe inconsistent communication, moves or changes made without family notification, mishandling of personal items, and variability in staff tone and professionalism. There are indications that new management and long-tenured personnel have improved some areas, but perceptions of care quality remain uneven across reviewers.

Notable patterns: the facility is frequently credited for strong rehabilitation outcomes and for individual staff members who go above and beyond, yet there are repeated operational weaknesses that relate to safety, communication, and environmental maintenance. Prospective families should consider on-site observations focused on call-button functioning, medication-administration procedures, security measures for personal belongings, staffing levels during different shifts, the layout and occupancy of memory-care rooms, and the facilityโ€™s protocols for recognizing and transferring residents with acute changes in condition. Asking for recent inspection findings, incident-response logs, and examples of quality-improvement initiatives may help clarify how the facility is addressing the concerns raised in these reviews.

About Best Care Health and Rehabilitation

Best Care Health and Rehabilitation sits at 2159 Dogwood Ridge Road in Wheelersburg, Ohio, and is known as Best Care Nursing & Rehab Center, offering a skilled nursing facility with both short-term and long-term nursing care options, so people come here for help after hospital stays, for intensive medical care, and for support with daily life when needed. The building has 110 certified beds with an average of 69 residents each day, and the team provides specialized services, like memory care for Alzheimer's and dementia, hospice and palliative care, and a full range of skilled nursing and rehabilitation support, including physical, occupational, and speech therapy, all handled by a rehab team focusing on personalized treatment plans. Residents can choose between private or semi-private rooms, which come furnished with private bathrooms, kitchenettes, cable TV, Wi-Fi, air conditioning, and daily newspaper services-some suites even have flat-screen TVs and telephones.

The staff gives 3.79 nurse hours per resident per day, which is slightly higher than the state average, and the nurses are available for 12 to 16 hours each day, with supervision and a 24-hour call and emergency alert system to make sure residents feel safe, though the nurse turnover rate is just a bit above the state average at 52.1%. Best Care accepts both Medicare and Medicaid, and the staff focuses on supporting residents with daily activities like bathing, dressing, eating, and transfers, including care for those with limited mobility and medication management for complex needs, always overseen by skilled nursing professionals.

For comfort, the facility offers many extras, with a movie theater, activity room, fitness room, wellness spa and sauna, and a library, while the outdoor areas have walking paths, a community garden, and common spaces for relaxing or taking part in community-sponsored outdoor programs. The dining room serves a range of meals with options for special diets, including those sensitive to allergies and for people with diabetes, and housekeeping and laundry services are included, which helps people focus on recovering or enjoying their days without worry. Transportation is available for appointments or outings, and there's parking for those who need it.

Residents often find things to do here, with daily schedules for arts and crafts, music, movie nights, and other organized programs, with some activities led by the residents themselves, which can help everyone stay engaged and connected. There are also special programs and organized activities to promote mental well-being.

The ownership belongs to Medical Rehabilitation Centers, LLC, and Lexington Health Management is affiliated with Best Care, while Brenda Campbell, Mohammad Kalo, and Elizabeth Townsend have managed the place since early 2023. The facility doesn't belong to a Continuing Care Retirement Community, and staff speak English. Best Care's services span short-term rehab, long-term care, post-surgical recovery, complex medical care, post-acute respiratory, cardiac, and orthopedic care, as well as respite stays, all tailored to each person's own recovery or care needs.

Best Care does have a record of inspection deficiencies, with 40 cited over time, including 11 listed in the October 2024 report, such as one infection-related concern, a pharmacy service deficiency about drug storage and labeling (F0761), and a resident rights deficiency on participation in resident or family groups (F0565), so families may want to review these inspections for clarity. Residents get supplied with unopened supplies and private or semi-private living spaces, and the team says they're dedicated to a nurturing and warm environment where they try to adapt care to individual needs, focusing on recovery, comfort, and personalized support, though like all facilities, there are things to keep an eye on and improvements to make. Currently, Best Care isn't accepting new residents but will provide information on next availability if asked.
