    Summary of reviews

Reviews of Hugo Health & Rehabilitation describe a facility with clear strengths in hands-on caregiving and community life alongside notable operational variability. Many comments praise nursing and therapy teams for attentive, compassionate care and rehabilitation support; several reviewers emphasized a warm, home-like atmosphere that includes resident pets and events that lift morale. Leadership and an administrator were singled out positively for creativity, community engagement, and for organizing memorable programming that staff and residents enjoy. Multiple accounts also describe staff who go above and beyond and a friendly resident community.

At the same time, a number of operational concerns recur across reviews. Admissions and intake procedures are described as inconsistent, with families encountering conflicting information and poor coordination during placement. Front-desk responsiveness and phone handling are frequently cited as problematic, including instances where callers were disconnected. Reviewers also noted variable staff professionalism and communication tone, which appears uneven across shifts and employee groups. Staffing levels were characterized by some as insufficient, and several comments point to gaps in medication-management workflows and in wound/pressure-injury management that warrant careful inquiry.

Dining and activities present a mixed picture. Positive reports highlight lively, memorable events and engaging activities that support resident well-being. Conversely, other reviewers described meal-service quality issues, including temperature and preparation inconsistencies. Activity programming is similarly variable: some families find it robust and community-building, while others report little organized engagement for residents.

Facility upkeep and business practices show signs of both improvement and concern. A number of reviewers praised recent improvements in cleanliness and organization after staffing changes, but separate comments raise sanitation and cleanliness issues in certain areas. Financial and billing clarity was flagged by some families as an area needing improvement, with unclear charges cited. Operationally, reviewers also raised challenges related to bed availability and placement coordination, including placement distance from family and an assertion by one reviewer that an external review was cited to decline admission.

For prospective residents and families, the pattern suggests strong interpersonal care and community strengths that coexist with variability in operational reliability. Recommended areas to evaluate during a visit include: current staffing ratios and turnover, medication administration and wound-care protocols, sample meals and kitchen practices, the admission and placement process, billing transparency, front-desk responsiveness, and the regular activity calendar. Meeting the administrator and observing an activity or therapy session can help gauge whether the positive leadership and culture improvements reported in some reviews are consistent across the facility.

    About Hugo Health & Rehabilitation

Homestead of Hugo is a large, for-profit, corporate-owned skilled nursing facility that offers both short-term rehabilitation and long-term care services. With a capacity of 124 beds, the home serves a diverse range of residents and participates in both Medicare and Medicaid programs. It is not a part of a continuing care retirement community, focusing its offerings instead on residents in need of specialized medical assistance and day-to-day support. The facility features nurse staffing levels that account for an average of 3 hours and 23 minutes per resident each day, with a noted consistency in staffing over weekends as well as weekdays. This aims to provide residents with stable and continuous care, regardless of the day of the week.

The care provided at Homestead of Hugo encompasses a variety of health and wellness needs, from medication administration to help with daily living activities such as mobility, eating, and personal hygiene. Residents benefit from medical oversight and are supported by a team trained to assist those recovering from hospitalizations due to conditions like stroke, heart attack, infections, or accidental injuries. The facility reports that 47.7% of residents are able to return home after their rehabilitation stays, reflecting its dual focus on temporary recuperative support and long-term residency as needed.

Homestead of Hugo places attention on preventative care through initiatives like the administration of annual influenza vaccinations, with 82.8% of residents receiving this protection. Approximately 85.9% of the communityโ€™s residents can maintain self-care abilities during their stay, underscoring efforts to preserve independence wherever possible. The rate of falls resulting in major injuries is documented at 1.6%, and 10.8% of residents in short-term rehabilitation experienced infections requiring hospitalization. Emergency room visits account for 8.7% of short-term stays, and the overall frequency is noted at 1.7 visits per 1000 patient days.

The facility has undergone multiple regulatory health and safety inspections, with recent focus on areas such as food safety, staffing adequacy, timely and accurate resident assessments, and the management of medical and non-medical care. Reports identified areas for improvement, including food procurement and preparation, maintenance of sufficient nursing staff, and development of thorough care plans. Staffing includes requirements for a registered nurse presence and a qualified dietician to ensure residentsโ€™ nutritional and medical needs are addressed. The home also maintains a program for infection prevention and control, and there have been formal processes to correct past areas of concern. Over the past three years, the facility has paid several fines, reflecting accountability to enforced standards and regulations.

Homestead of Hugo stands as a significant provider of skilled nursing care, dedicated to supporting both short-term rehabilitation and the ongoing needs of its long-term residents. Through comprehensive care services, consistent nurse staffing, and ongoing quality improvement initiatives, the facility aims to promote the well-being, dignity, and safety of each resident, fostering a supportive environment tailored to individual health and lifestyle requirements.
