    Caring, clean facility; responsive staff

I placed my parent here after a tour and I'm very pleased. The RNs, director and caregivers are caring, responsive and proactive - they communicate regularly and often go above and beyond. The facility is clean and well-maintained, with roomy private rooms, home-style meals, lots of activities and real peace of mind; I would recommend it to families.

    Summary of reviews

Stone Brook Assisted Living & Memory Care elicits a mix of strong positive impressions alongside a subset of operational concerns. Many families praised the facility's clean, well-maintained appearance, home-like atmosphere, and roomy private accommodations that include kitchenettes and accessible bathrooms. The dining program, hotel-style dining room, and regular meal service are frequently noted as strengths, and the facility offers a broad activities program—games, movies, crafts, bingo, outings and transportation for errands and medical visits—that contributes to resident engagement.

Staffing and clinical care generate both praise and concern. Numerous reviewers described caring, communicative nurses and leadership who provide proactive updates and strong family communication; some named RNs and directors who were perceived as particularly attentive. At the same time, other accounts describe inconsistent staffing levels, delayed responses to clinical incidents, and apparent gaps in monitoring that have contributed to severe family worry. These comments suggest variability in how consistently clinical procedures and incident-response protocols are followed across shifts.

Personal care and sanitation patterns are another mixed area. Positive notes include housekeeping, general cleanliness, and good maintenance of rooms and common areas. However, there are specific concerns about inconsistent bathing schedules, delays in incontinence-related care, and localized odor issues—particularly within the memory-care unit. Laundry and clothing-management problems (missing or unkempt clothing) were mentioned by some families, indicating gaps in daily personal-care coordination for certain residents.

Activities and social programming are commonly described as robust, with many families appreciating the range of options and the sense of community. A few reviewers, however, felt activity frequency or variety could be improved in some parts of the facility. Facilities and grounds receive frequent praise for pleasant landscaping, roomy common spaces (TV room, library, game room), and effective maintenance, though some interior areas were described as dated and some visitors noted a strong disinfectant odor on tour.

Management and resident-family relations are generally seen as accessible and helpful; on-site ownership, proactive staff, and instances of above-and-beyond care were repeatedly commended. Conversely, some families experienced inconsistent customer-service tone, difficulties with certain staff members, and inflexible visitation practices. There are also isolated accounts of unresponsive administrative follow-up, which contributes to a perception of uneven management oversight.

Overall, Stone Brook presents as a facility with clear strengths in dining, activities, physical environment, and many engaged caregiving staff. At the same time, reviewers point to meaningful operational weaknesses—most notably in staffing consistency, clinical-incident response, and personal-care routines—that prospective families should evaluate. When considering Stone Brook, families may want to ask specific questions about current staffing ratios, fall-incident protocols, bathing and incontinence-care schedules, laundry procedures, memory-care environmental controls, and visitor/visitation policies, and to tour the memory-care unit separately to assess condition and staffing during different shifts.

About Stone Brook Assisted Living & Memory Care

Stone Brook Assisted Living & Memory Care in Denison, Texas, sits on attractive, well-kept grounds and offers seniors a safe, quiet place to live with friendly staff and a family-like community spirit, where folks greet each other and enjoy living together. The ranch-style residence provides both assisted living and memory care, each with different levels of support, so one might see residents who need a little help with bathing or grooming, and others who live with dementia getting extra care and close supervision, and the staff are ready all day and night, with nurses on hand and a doctor on call if needed. Apartments come in several floor plans, including studios, one-bedrooms, and companion suites, some with kitchenettes, private baths, closets, cable TV, Wi-Fi, and emergency call systems, and there are private and semi-private rooms for both privacy and companionship, with wheelchair accessible showers for safety.

The place has many comforts, like fireplaced living rooms, TV lounges, a puzzle room, a library, a beauty salon with a visiting beautician every week and a barber shop, and guests can find their meals in a dining room that looks a bit like a nice hotel with tablecloths, or take meals in their rooms, with three chef-prepared meals served daily, snacks between meals, and menus that include special diets for diabetes or high blood pressure, along with some international food to keep things interesting. Pets like cats and dogs are welcome, and the grounds have raised gardening beds, courtyards, walking paths, and outdoor gathering spaces for those who like to get some air or work in the garden. There's plenty of parking for residents and guests, and transportation services are available, including complimentary rides for local errands or appointments, plus scheduled trips and outings, so residents can get out for shopping or enjoy group activities.

Residents have daily chances to join arts and crafts, brain fitness classes, exercise like stretching or yoga, games and puzzles, music, gardening, movie nights, holiday events, and community service projects. The full-time activity director organizes devotional services both on-site and off, and makes sure there's something on the calendar to keep folks engaged, so even those coming for a short respite program after an illness or surgery find social time and help with meals and care, sometimes for a day or a few weeks. Stone Brook pays close attention to safety, with secured buildings and technology like alarm bracelets for memory care residents who might wander, with staff trained for Alzheimer's and dementia behaviors, including help with medications, blood sugar checks, and all kinds of assistance, right up to mechanical lifts, incontinence care, hospice, and specialized therapy.

Everyone gets a personalized care plan after a full assessment, with regular updates, and the team focus on dignity, respect, and trying to keep residents as independent as they can be, helping with what's needed but promoting choice and privacy. The all-inclusive rate covers meals, housekeeping, laundry, daily support with activities like bathing, dressing, and reminders, and there's financial guidance for families sorting out costs. The community, licensed as an assisted living facility (ALF#000463), welcomes folks from north Texas and southern Oklahoma, including those needing more care who want to stay in a comfortable setting with flexible help, aging in place, and plenty of chances to enjoy good meals, companionship, and pleasant surroundings.
