    Very satisfied with mother's care

I'm very satisfied with the care my mother receives here. Administration is responsive, staffing and care have noticeably improved, and the building is clean and odor-free. The staff are friendly, helpful and engaged-Ronald (Med Aide), Kay and LPN Alice went above and beyond-rehab therapy and activities are strong and the community feels family-oriented.

    Summary of reviews

Overall impression: Reviews are mixed with a pronounced split between families who describe compassionate, helpful direct-care staff and those who document recurring operational weaknesses. Positive remarks highlight attentive CNAs, helpful licensed nurses and medication aides, strong rehabilitation/therapy programming, and a family-oriented atmosphere. At the same time, there are consistent operational concerns that families and prospective residents should investigate further before placement.

Care quality: Direct-care staff receive frequent praise for compassion and hands-on assistance; named employees are often singled out for going beyond duty. The facilityโ€™s therapy services and rehabilitation programming are also cited as strengths. Conversely, reviewers describe variability in basic care delivery: inconsistent assistance with bathing and continence needs, slow or irregular response to call lights, missed or delayed medication administration, and instances of delayed clinical follow-up or hospital transfer. These patterns suggest uneven adherence to routine care schedules and clinical monitoring protocols.

Staff and communication: Staff friendliness and individual dedication are common positives, but there are recurring concerns about professional communication and staff identification. Several accounts mention staff not wearing visible name badges, inconsistent introductions, and communication that family members perceived as dismissive or unprofessional. Phone accessibility is a frequent complaintโ€”long hold times and unanswered callsโ€”and family communication about clinical changes and incidents is described as uneven. Staffing levels are characterized as short or inconsistent, which reviewers link to many of the service delays they experienced.

Dining and activities: Food quality and dietary accommodation are areas of concern for multiple reviewers, with comments about inconsistent meals and limited responsiveness to special dietary needs. In contrast, activity programming and events receive favorable comments from families who noted resident engagement and coordinated activities. Therapy and rehabilitation services are often highlighted as positive aspects of the facilityโ€™s offerings.

Facilities and sanitation: The physical plant is described as dated and institutional in parts, with an older layout and shared bathroom arrangements noted as suboptimal for privacy and comfort. Sanitization and odor control emerge as recurring issues in some accountsโ€”furniture cleanliness, hallway fixtures, and trash management are specifically mentionedโ€”while other reviewers describe recent improvements in cleanliness and pest/odor control under newer administration. Prospective families should assess current conditions in person and review recent inspection or remediation documentation.

Management and oversight: Reviewers describe inconsistent management responsiveness. Some families say leadership is attentive and has implemented staffing and cleanliness improvements; others report slow or inadequate resolution of complaints, inconsistent oversight, and burdensome administrative practices around trust accounting. There is at least one serious allegation of staff theft that would warrant independent verification through regulatory records or facility administration.

Notable patterns and takeaways: The facility exhibits a pattern of polarized experiencesโ€”strong, compassionate bedside care from specific staff members and therapy teams, alongside systemic operational weaknesses in cleanliness, timeliness of basic care, communication, and facility upkeep. Improvements tied to administrative change are mentioned, suggesting recent initiatives may be addressing some problems. Prospective residents and families should prioritize direct observation of current staffing levels, call-response times, hygiene and sanitation practices, medication administration protocols, dietary accommodations, and managementโ€™s processes for handling complaints and incidents. Reviewing the most recent state inspection reports and asking the facility for current staffing ratios and cleaning/maintenance logs is advisable prior to placement.

About Willow Park Health Care Center

Willow Park Health Care Center is a nursing home and retirement community, run by Care Providers Oklahoma, where Lucretia Parkey acts as the administrator and Sandi Ghannam works as the social services director, and they both help look after the daily needs and well-being of the people who stay there. The center has a focus on nursing home care for seniors, and they also offer options for assisted living, independent living, and memory care, so there's a lot of choices, and you'll find senior apartments and home care too, which means you can pick what suits your needs best. Their staff have a reputation for being friendly, helpful, and kind, which helps everyone feel welcome and comfortable, and there's always a warm atmosphere. The facility gives out nutritious meals, planned by chefs and meal planners to make sure everyone gets a balanced diet with good taste and the right nutrients. Licensed nurses and aides provide 24-hour care for seniors with physical or mental health needs, and the facility has a wide range of medical services, like skilled nursing, long-term care, short-term rehabilitation, speech therapy, occupational and physical therapy, and they even provide respiratory and cardiac care, wound management, pain control, medication help, intravenous therapy, and diabetes management. Willow Park Health Care Center supports patients coming from hospital stays or with serious conditions such as stroke, surgery recovery, fractures, spinal cord injuries, or respiratory illness, and it offers both long-term stays and short-term rehab for those looking to go back home. The facility has a team with a medical director, a consulting pharmacist, and a consulting dietician, and they focus on teaching patients and families how to manage care at home, which is an important part of the process. The resident rooms are spacious with private showers and restrooms, which folks say are the largest in Navarro County, and there are transportation services and daily activities to keep residents engaged-these activities are meant to help people stay happy and social, and they've even gotten awards for community events and engagement. The center provides hospice and palliative care when needed, along with memory care, telehealth services, and respite care for family members who need a break from caregiving. Social services and support for emotional needs are available, with Sandi Ghannam focusing on residents' personal and social well-being. Lab and x-ray diagnostics are available onsite, and there are many amenities like dining options and organized activities. All told, Willow Park Health Care Center gives seniors a full range of services from help with daily life to specialized medical care, all in a setting that tries to keep things comfortable and engaging for its residents.
