    Severe neglect understaffing unclean facility

I had a horrible experience. My loved one was repeatedly neglected - long call-light waits (20-30+ min), missed hygiene and meds, delayed hospital transfer and even a resident found on the floor. The building is old, smelly and unhygienic (sticky rails, trash, urine/feces on clothing), management unresponsive, phone lines often unanswered and staff communication is inconsistent. That said, several CNAs, LPN Alice, Ronald the med-aide and Kay were caring, helpful and went above and beyond, and rehab therapy seemed good. Overall, friendly frontline staff can't overcome chronic understaffing, poor administration, safety and cleanliness issues - I cannot recommend this facility.

About Willow Park Health Care Center

Willow Park Health Care Center is a nursing home and retirement community, run by Care Providers Oklahoma, where Lucretia Parkey acts as the administrator and Sandi Ghannam works as the social services director, and they both help look after the daily needs and well-being of the people who stay there. The center has a focus on nursing home care for seniors, and they also offer options for assisted living, independent living, and memory care, so there's a lot of choices, and you'll find senior apartments and home care too, which means you can pick what suits your needs best. Their staff have a reputation for being friendly, helpful, and kind, which helps everyone feel welcome and comfortable, and there's always a warm atmosphere. The facility gives out nutritious meals, planned by chefs and meal planners to make sure everyone gets a balanced diet with good taste and the right nutrients. Licensed nurses and aides provide 24-hour care for seniors with physical or mental health needs, and the facility has a wide range of medical services, like skilled nursing, long-term care, short-term rehabilitation, speech therapy, occupational and physical therapy, and they even provide respiratory and cardiac care, wound management, pain control, medication help, intravenous therapy, and diabetes management. Willow Park Health Care Center supports patients coming from hospital stays or with serious conditions such as stroke, surgery recovery, fractures, spinal cord injuries, or respiratory illness, and it offers both long-term stays and short-term rehab for those looking to go back home. The facility has a team with a medical director, a consulting pharmacist, and a consulting dietician, and they focus on teaching patients and families how to manage care at home, which is an important part of the process. The resident rooms are spacious with private showers and restrooms, which folks say are the largest in Navarro County, and there are transportation services and daily activities to keep residents engaged-these activities are meant to help people stay happy and social, and they've even gotten awards for community events and engagement. The center provides hospice and palliative care when needed, along with memory care, telehealth services, and respite care for family members who need a break from caregiving. Social services and support for emotional needs are available, with Sandi Ghannam focusing on residents' personal and social well-being. Lab and x-ray diagnostics are available onsite, and there are many amenities like dining options and organized activities. All told, Willow Park Health Care Center gives seniors a full range of services from help with daily life to specialized medical care, all in a setting that tries to keep things comfortable and engaging for its residents.
