    Caring staff, warm home, improving

I'm impressed by the genuinely caring, kind staff-especially the dietary aide. The team is awesome (kudos!), creating a warm, home-like environment with great resident care and positive changes underway. I'm very satisfied overall; a few small improvements would make it outstanding.

    Summary of reviews

Reviews present a mixed picture of care and operations at the facility. Positive comments highlight caring interactions from many direct-care staff and dietary aides, a home-like atmosphere in some areas, and indications that management changes and employee compensation adjustments are underway to improve staffing and operations. At the same time, multiple concerns recur across reviews about operational reliability and the consistency of resident care.

Care quality and clinical oversight: Families and some staff describe delays in routine personal care such as bathing and incontinence support, alongside accounts indicating gaps in clinical monitoring and wound-care management. These patterns are associated with downstream problems in resident comfort and skin/wound outcomes in some cases. Prospective families should ask about current nursing and therapy staffing levels, care-plan review processes, and specific protocols for wound assessment and pressure-injury prevention.

Staffing and staff conduct: Several accounts point to staffing shortages and role overload, with individual staff members expected to handle multiple responsibilities. Reviewers also describe variability in staff professionalism and responsivenessโ€”while many direct-care employees receive praise for compassion and one-on-one attention, other comments note inconsistent communication tone and slow response to family inquiries. Employee-side remarks indicate competitive pay and some positive team changes that may support recruitment and morale.

Dining and activities: Dietary aides receive positive mentions for being attentive and kind, but overall food quality is described as inconsistent and impacted by dietary restrictions or menu limitations. Social services and organized recreational offerings appear uneven; some reviewers cite shortcomings in social-service responsiveness. Families should clarify how the facility accommodates special diets and how activity programs are scheduled and individualized.

Facility condition and infection-control concerns: Reviews raise sanitation issues, odor-control deficiencies, and pest-control concerns in certain areas. These observations suggest the need for more consistent environmental services and a clear infection-control and pest-management plan. Inquiring about recent environmental inspections, corrective-action steps, and routine housekeeping audits will be important for families assessing risk.

Management, communication, and visitation: A common theme is uneven leadership responsiveness and communication with families. Several reviewers describe difficulty obtaining timely answers from administrative staff; a few accounts describe confrontational visitation situations. The facility is reported to be undergoing management changes in some reviews, which reviewers indicate may be driving improvement efforts. Prospective residents and families should ask about current leadership, family communication protocols, visitation policies, and any recent corrective actions or regulatory findings.

Overall assessment and recommended next steps: The facility demonstrates strengths in individual staff compassion and some positive operational changes, but consistent concerns about staffing, basic personal care timeliness, clinical monitoring, environmental sanitation, and communication warrant careful inquiry. Visitors should conduct in-person tours at varied times (including evenings/weekends), request staffing ratios and recent quality-audit results, review wound-care and infection-control protocols, and speak directly with the nursing leader about specific care needs before making placement decisions.

About Heritage Hills Living & Rehab

Heritage Hills Living & Rehabilitation Center in McAlester, OK is a medium-sized facility with 81 beds that offers both short-term rehabilitation and long-term care services. The facility participates in Medicare and Medicaid, making it accessible to a wide range of individuals seeking care. Heritage Hills is not a part of a continuing care retirement community, but it does offer a range of services for residents in need of specialized care.

The facility has received a below-average rating for both short-term rehabilitation and long-term care based on recent data. This indicates that there may be areas for improvement in terms of the quality of care provided to residents. Despite this, Heritage Hills is equipped to handle post-acute care for patients recovering from a hospital stay, such as after a stroke, heart attack, infection, or accidental injury.

Heritage Hills has been closely monitored by the Centers for Medicare and Medicaid Services at various points in recent years due to documented patterns of poor care. This monitoring ensures that the facility is held accountable for providing appropriate care to its residents. Recent inspection reports have highlighted instances where the facility has failed to meet health and safety standards, including issues related to resident care plans, infection prevention, and resident rights.

In terms of staffing, Heritage Hills has shown consistency with having a registered nurse on site for at least 8 hours every day. This level of staffing helps to ensure that residents receive the necessary medical care and attention. The facility also has a high percentage of residents who are able to maintain their independence in daily activities, indicating a commitment to promoting resident well-being.

Overall, Heritage Hills Living & Rehabilitation Center provides a range of services for individuals in need of short-term or long-term care. While the facility has areas for improvement based on recent data and inspection reports, it remains a viable option for those seeking quality care in the McAlester, OK area.
